An effective method of surgical treatment of refractory glaucoma patients using Ex-PRESSTM filtering device

Abstract

Based on data obtained from examination and subsequent follow-up of 47 patients (50 eyes) with refractory glaucoma, an efficacy estimation of a new method of the Ex-PRESSTM filtering device implantation was performed. The data analysis showed that the proposed surgical procedure has a low level of intra- and post-operative complications, is characterized by technical ease, and provides a long term stabilization of the glaucomatous process. Therefore it is possible to draw a conclusion that the Ex-PRESSTM filtering device implantation is an effective method for the treatment of refractory glaucoma.

Efficacy and safety comparison of Xalatamax and Xalatan at their use as open-angle glaucoma monotherapy

Abstract

Objective: comparative evaluation of the hypotensive effect and tolerability of two latanoprost 0.005% preparations —Xalatamax of “Jadran” production and Xalatan of “Pfizer International” production at their use as open-angle glaucoma monotherapy. Materials and methods: in the study, 50 patients took part, divided into two groups depending on medication used. Follow-up period was 12 weeks. During this time, patients were examined monthly. IOP was measured, main ophthalmological and general indices were investigated, biomicroscopy and ophthalmoscopy were carried out. Patients’ compliance was constantly monitored, adverse events occurrence, drug instillation compliance, changes in patient’s state self-assessment were under hold. Study results: the level of generally accepted target pressure was practically preserved during the whole study term in both groups. Visual acuity, perimetric indices and eye fundus did not experience statistically significant changes. All patients showed almost equal compliance to treatment. At the start of the study, there were slightly more complaints to the quality of life in the group, that began Xalatamax treatment. Later on, there was a quality of life increase in both groups. In the Xalatamax group, this increase was most significant and constant. Conclusions: side effects by two types of latanoprost treatment did not significantly influence the patients’ quality of life and their attitude to treatment. The IOP-lowering efficacy of both medications to be compared turned out to be almost equal. When using Xalatamax, there was no statistically significant difference in the severity of side effects, general tolerability, and functional indices, in comparison with Xalatan.

A COMPARATIVE ANALYSIS OF THE RESULTS OF SCLERAL BUCKLING WITH ALLOPLANT BIOMATERIAL VERSUS SILICONE SPONGE

Abstract

A surgical method of rhegmatogenous retinal detachment repair with Alloplant biomaterial is effective and allows to provide a significant visual improvement. Technical simplicity, favorable course of early post-operative period, and lack of complications specific to the majority of episcleral procedures (implant contamination and/or extrusion, buckle displacement, dystrophic scleral changes in the buckling area) are the undoubted advantages of the proposed method, which make it preferable for using in rhegmatogenous retinal detachment.

Immunomorphological features of “classic” and “occult” choroidal neovascular membranes in age-related macular degeneration

Abstract

A comparative evaluation of morphological features and proliferative activity of “classic” and “occult” choroidal neovascular membranes (CNM) in patients with the wet form of age-related macular degeneration was carried out. It was found that the CNM proliferative activity depends on the membrane type. The presence of a high pigment epithelium detachment in patients with “occult” CNM may be due to the presence of a solid avascular scar in the subretinal space.

The results of the keratoconus treatment of different degree by “Alloplant” dispersed biomaterial

Abstract

In this article there are presented the research results of keratoconus index after “Alloplant” dispersed biomaterial insertion. There were examined 27 individuals of different keratoconus phase. The obtained results showed that the postoperative change intensity of keratoconus index depends on the initial level of these parameters and keratoconus degree. The research results testify to the low-gradual improvement of the cornea condition and its optic properties.

The influence of phacoemulsification on the intraocular pressure level in patients with co-existing cataract and open-angle glaucoma

Abstract

Based on the examination and treatment results of 94 patients (100 eyes) with cataract and open-angle glaucoma, an estimation of phacoemulsification with intraocular lens implantation influence on the intraocular pressure level was performed. The result analysis showed that phacoemulsification in patients with co-existing cataract and stabilized open-angle glaucoma could ensure a pressure-lowering effect by continued local treatment consisting in instillations of modern IOP-lowering medications.

The enzymatic vitreolysis — a new trend in the treatment of retinal vein occlusions

Abstract

Vitreoretinal relationships exert a significant influence on the course of retinal vein occlusions (RVO) and post-thrombotic retinopathy. A single intravitreal injection of Actilyse 50 mkg in the period from 1 till 21 days from the disease onset stimulates a posterior vitreous detachment in 30.4% patients with non-ischemic and 24.8% patients with the type of ischemic RVO. The process of transition from the partial posterior hyaloid detachment to the complete one, and complete posterior hyaloid detachment in patients who did not have it from the beginning, was accompanied by a visual acuity increase by a factor of 1.5. The use of enzymatic vitreolysis in patients with RVO increases the probability of reduced macular edema in the early disease period, to increases visual acuity, and decreases the risk of neovascularization development in the posterior eyeball segment.

Visual acuity dynamics after phacoemulsification in patients with “wet” form of age-related macular degeneration, treated by intravitreal angiogenesis inhibitor injections

Abstract

Design: prospective non-randomized experimental longitudinal cohort study. The aim of the study: to evaluate the visual acuity after phacoemulsification in patients with the “wet” form of age-related macular degeneration, treated by intravitreal angiogenesis inhibitor injections. Materials: 48 patients (48 eyes) with the “wet” form of AMD, treated by intravitreal angiogenesis inhibitor injections, and incipient cataract. All eyes in the study had a diagnosis of “wet” AMD and underwent cataract surgery. They were followed for a minimum of two years after the cataract surgery. The mean age of patients was 78.16 ± 5 years. There were 36 female (75 %) and 12 male (25 %) patients. Results: All patients were retrospectively divided into two groups. Group No. 1 consisted of 23 patients — operated within a year of the diagnosis of “wet” AMD, and group No. 2 which consisted of 25 patients who had a diagnosis of “wet” AMD for longer than one year prior to the cataract surgery. The mean preoperative visual acuity in the 1st group was 0.16 ± 0.07 (median 0.15), and in patients of group 2 — 0.23 ± 0.06 (median 0,2). Mean visual acuity immediately before surgery in the 1st group was 0.13 ± 0.04 (median 0.09), in the 2nd group — 0.12 ± 0.04 (median 0,08). After surgery, in the 1st group, mean visual acuity was — 0.28 ± 0.08 (median 0.25), in the 2nd group —0.18 ± 0.07 (median 0,15). The difference between the two groups was statistically significant, p < 0.01, t-test. Statistically significant visual acuity increase after surgery was registered in both groups and was preserved after one and two years, when compared to the data before surgery: for the 1st group — 0.13 and 0.1; for the 2nd group — 0,1 and 0.07, respectively (p < 0,01, Wilcoxon criterion). Conclusions: Phacoemulsification is reasonable for eyes with the “wet” form of age-related macular degeneration. Patients with shorter “wet” AMD duration obtain a higher visual acuity increase after cataract surgery. Statistically significant visual acuity increase after surgery is preserved in the majority of patients during next 2 years of follow-up.

The influence of vitreomacular adhesion on the pathogenesis and course of age-related macular degeneration

Abstract

This article describes the age-related changes in vitreous and different types of posterior vitreous detachment. Possible effect of normal and pathological posterior vitreous detachment (in particular vitreomacular adhesion and traction syndrome) on the age-related macular degeneration course are considered. Vitrectomy and pharmacological vitreolysis potentials in the treatment of vitreomacular adhesion are discussed.

The new advantages and perspectives of Latanoprost (Xalatan, Pfizer, USA) treatment in pseudoexfoliation glaucoma

Abstract

The purpose of this study was to compare Latanoprost and Timolol therapeutic activity in pseudoexfoliation glaucoma (PEG). Forty patients with moderate PEG aged from 62 до 78 were enrolled. The one year treatment included the instillation of one drop of Latanoprost 0.005 % once a day (20 patients) and Timololmaleat 0.5 % twice a day (20 patients). The IOP reduction rate in six months was 22 % and 17 % respectively for Latanoprost and Timolol and in twelve months — 17 % and 13 % respectively. The improvement of visual fields and morphometric parameters in Latanoprost group was observed in twelve months: the changes of perimetric indexes from –5.8 ± 0.23 dB to –4.6±1.18 dB (MD) and from 7.3 ± 0.19 dB to 3.8 ± 0.14 dB (PSD); the cup volume of optic nerve head was reduced significantly: from 0.31 ± 0.13 мм3 to 0.21 ± 0.15 мм3. In contrast to this in Timolol group the parameters have become worse: for rim volume this change was: from 0.25 ± 0.14 мм3 до 0.19 ± 0.08 мм3 and for perimetric indexMD: from 5.30.13 dB to 7.8 ± 0.35 dB. We concluded that Latanoprostmay be recommended as the first choice therapy in PEG.

On the causes of ocular tuberculosis morbidity rate decrease in Russia over the last years

Abstract

The results of an investigation of 2055 patients with confirmed and suspected ocular tuberculosis referred to the Saint-Petersburg Phtisiopulmonology Scientific-Research Institute during different time periods (1986-1991; 2004-2010; and 2012) were analyzed. A system of differential diagnosis and a verification method for ocular tuberculosis were used, which were worked out by the Institute phtisioophthalmologists. Over the last two time periods, in comparison with 1986–1991, the frequency of detection of ocular tuberculosis in patients with suspected tuberculosis significantly decreased; the use of tuberculinodiagnostics and of test-therapy also substantially decreased. Also it was discovered that there was an absence of phtisioophthalmologists in many dispensaries. Altogether, the decrease of ocular tuberculosis morbidity rate in Russian Federation over the last time periods was determined both by insufficient staffing level of phtisioophthalmologists in antituberculous dispensaries, and by the decrease of the quality of ocular tuberculosis diagnosis in antituberculous facilities.

Abstract

Синдром Гарднера представляет собой редкое заболевание, частота которого составляет приблизительно 1 на 22 000. Большинство случаев возникает в результате аутосомно-доминантного наследования из-за мутаций в гене APC (Adenomous Polyposis Coli). Однако около 20% случаев возникают спонтанно. Клинически поражения могут иметь различные размеры и пигментацию, но многие имеют вид «рыбьего хвоста». В одном гистологическом исследовании были выявлены более распространенные изменения, чем ожидалось по клинической картине, а в одном случае в этом исследовании были выявлены гамартомоподобные изменения. В то время как очаги врожденной гипертрофии пигментного эпителия при синдроме Гарднера по гистологической картине похожи на изолированные поражения при врожденной гипертрофии пигментного эпителия и сгруппированные невусы пигментного эпителия («следы медведя»), оба последних состояния являются доброкачественными и не имеют никакой связи с полипами или раком толстой кишки.
