THE ISSUE OF STRUCTURAL ORGANIZATION PERIPHERAL NERVES OF THE BRACHIAL PLEXUS HAS REPRESENTATIVES OF THE CLASS OF AMPHIBIANS AND REPTILES THE COMPARATIVE ASPECTS

Abstract

This paper presents the results of a comprehensive study of the morphological structures of the peripheral nerves that innervate the muscle-flexors and extensors in the middle third of the shoulder from the representatives of classes of amphibians and reptiles. It was found that more difficult to organize are the peripheral nerves and the surrounding connective tissue structures, representatives of reptiles class. It was also obtained morphological confirmation of the bilateral asymmetry in the structure of the neurovascular bundles of peripheral nerve on opposite limbs and all studied vertebrates.

MORPHOFUNCTIONAL CHANGES OF THYROID GLAND THYROCYTES AND C-CELLS IN HYPOTHYROIDISM INDUCED BY ACTINOMYCIN D ADMINISTRATION

Abstract

Thyroidin status and thyrocyte ultrastructure of thyroid gland in case of hypofunction induced by introduction of aktinomicin D were studied in the corse of experiments on laboratory animals using radioimmunologic and electronomicroscopic techniques. Confirmation of hypothyroidism was obtained at the tissue level. Morphometric studies of the nuclear apparatus and organelles of thyrocytes and C-cells cytoplasm were held. The proportion of inert heterochromatin in the nucleus is reduced, and the nucleoli decrease in size and lose their cellular structure. Changes occur in vacuolar-lysosomal system. The presence of synergism in the action of actinomycin D and triiodothyronine on thyrocytes. The ultrastructural changes violating biosynthesis and triiodothyronine transportation in thyrocytes and biosynthesis of somatostatin and calcitonin in C-cells were discovered.

THE STRUCTURAL AND FUNCTIONAL STATE OF THE C-CELLS OF THE THYROID GLAND IN HYPOPHYSECTOMIZED ANIMALS

Abstract

The ultrastructure of the C-cells of the thyroid gland in hypophysectomyzed rats was studied. Morphometric studies of the nuclear apparatus and cytoplasm with organelles C-cells were held at the ultrastructural level. Nuclear-nucleolar ratio and the ratio of euchromatin and heterochromatin were studied in experiment. Area euchromatin increased in the nucleus. The amount of cristae in mitochondria and the average area of mitochondria were estimated to study the energy of C-cells and the energy efficiency ratio compartment was calculate on the basis of morphometric data. The number of mitochondria is increased in the cell. The number of cristae is increased in mitochondria. The energy efficiency ratio compartment increases. Degranulated form of C-cells predominant. The ultrastructural changes indicating C-cells hyperfunctional condition on the background of the thyroid hormone function were discovered.

THE DEPENDENCE OF CENTRAL HEMODYNAMIC PARAMETERS AND LEVEL OF ADAPTIVE POTENTIAL ON THE SOMATOTYPE

Abstract

155 students aged 17 to 20 years were selected for this research. Following the procedure of M. Chernorutskii based on the value of the index Pine, all the subjects were divided into three groups (asthenic, normosthenic and hypersthenic girls). Central hemodynamic parameters (heart rate, blood pressure, stroke volume and cardiac output) and the level of adaptive potential was analyzed. The effect somatotype on functioning of the cardiovascular system was determined. The tendency of increase of blood pressure with an increase in overall body size was identified. The signs of the more advanced cardiovascular compensation abilities were shown in asthenic girls. Adaptive potential index showed a satisfactory adaptation of the cardiovascular system in girls of all body types.

THE VALUE OF MICROBIOLOGICAL MONITORING AND DETERMINING THE MOLECULAR GENETIC CHARACTERISTICS OF HOSPITAL MICROFLORA OF PURULENT OSTEOLOGY DEPARTMENT

Abstract

434 microbial strains found in Purulent Osteology Department patients were analyzed. 203 infectious agents were found in patients with purulence after major joint prosthetics, 148 - in patients with chronic osteomyelitis, 83 - in diabetic foot patients. Grampositive microorganisms accounted for 302 strains, gram-negative ones - for 132 strains. Irrespective of nosology, main infectious agents were represented by various staphylococci, a third of which expressed mec A gene displaying a phenotype of methicillin-resistant strains. Ps. Aeruginosa was second in detection rate, among which 9 strains out of 51 had a gene encoding VIM-type Metallo-beta-lactamase production. The third place in detection rate belonged to A. baumanii. 15 strains producing OХА-40-like carbapenemases were found among 29 acinetobacter isolates. 14 out of 19 K. pneumoniae strains produced extended spectrum beta-lactamases. Therefore, similar patterns in infectious agent structure were found in Purulent Osteology Department patients with various infectious processes: staphylococci predominance and circulation of a great number of antibiotic multiresistant microorganisms. A thorough microbiological monitoring enables determining of phenotype and genotype of antibiotic resistance and prescribing a balanced antimicrobic therapy.

INFLUENCE OF VILDAGLIPTIN AND GLIQUIDONE COMBINATION ON P-GLYCOPROTEIN FUNCTIONAL ACTIVITY AND EXPRESSION ON THE BACKGROUND OF NORM AND IN EXPERIMENTAL ALLOXAN-INDUCED DIABETES MELLITUS TYPE 2

Abstract

In the in vivo study on rabbits investigated the effect of the vildagliptin and gliqui-done combination on the background of norm and experimental alloxan-induced diabetes mellitus (DM) type 2 on the P-glycoprotein (P-gp, ABCB1 protein) functional activity and its expression in the tissues of the jejunum, liver, kidney and the blood-brain barrier. Introduction of combination of vildagliptin at a dose of 5 mg/kg and gliquidone 10 mg/kg body weight on the background of norm for 14 days did not result in P-glycoprotein changes. On the 5-day of the cancellation of this combination observed inhibition of transporter protein functional activity without altering its expression. In the combination therapy of vildagliptin and gliquidone alloxan-induced diabetes type 2 showed complete recovery of P-glycoprotein functional activity and expression reduced on the pathology background. On the 5-day of treatment withdrawal again occurred inhibition of functional activity of the transporter protein and a decrease in its expression.

THE MEDICATION INHALE TECHNOLOGY AND ASTHMA CONTROL

Abstract

The article presents a study of the influence of inhalation technology on the control of bronchial asthma (BA). The study involved 59 patients with asthma in the full or partial control, the definition of the complaints, the number of errors when inhaled through a metered aerosol inhaler indicators of respiratory function, levels of exhaled nitric oxide. Following the training sessions on the art of inhalation after 4 weeks again, a repeated examination. Decrease the number of points detected by questionnaire form ACQ-5, which corresponds to an increase in asthma control and also reduce errors by inhalation medicaments by metered-dose inhaler.

CLINICAL MANAGEMENT OF DEEP VEIN THROMBOSIS OF THE INFERIOR VENA CAVA SYSTEM IN PREGNANT WOMEN

Abstract

Objectives. To analyze the treatment of patients with deep vein thrombosis in the inferior vena cava system during pregnancy. Materials and methods. The study is based on the survey results of 23 pregnant women with deep vein thrombosis (DVT) (the study group). The average age of patients was 27,4±5,8 years (M±g). All women were divided into two subgroups depending on the gestation terms: in 9 pregnant patients thrombosis developed in the 2nd trimester, in 14 - in the 3rd trimester. Plasma hemostasis investigations, ultrasound triplex scanning of the lower limbs, fetal ultrasound study with Doppler utero-placental blood flow were performed in all patients. Results. In the 3rd trimester one woman was diagnosed with the missed abortion. The operation of a small cesarean section was performed in this patient. In the other 8 cases of a progressing pregnancy one administered the conservative treatment with the use of elastic compression, direct anticoagulants and drugs improving microcirculation. Clinical management of 14 patients with DVT developed in the 3rd trimester depended on the nature of the thrombotic masses. All patients were diagnosed the occlusive nature of the clot, the floating thrombosis was not detected in any case and the patients were treated conservatively. The delivery by means of cesarean section was routinely carried out in all women in the period of 37-38 weeks. One patient the delivery was complicated by the pulmonary embolism of the peripheral branches. Results. The obtained results have demonstrated the application expediency of the worked out clinical management in treating pregnant women with deep vein thrombosis in the inferior vena cava system.

COMPLEX DIAGNOSTICS AND SURGICAL TACTICS IN BILATERAL PHEOCHROMOCYTOMA

Abstract

The aim - determination of surgical treatment, and particularly the management of patients with bilateral pheochromocytomas. The paper analyzes the results of a comprehensive diagnostics and treatment of 8 patients with bilateral pheochromocytomas. Pheochromocytoma diagnosis was based on clinical data and the results of ultrasound, CT, and the definition of metanephrines levels. Despite the typical clinical manifestations pheochromocytomas only one patient was diagnosed up to 1 year from the moment of occurrence of the first clinical signs of the disease. In other cases, for various reasons (errors in the diagnosis of primary care physicians, late treatment of patients, long-term independent use of various antihypertensive drugs) was observed late diagnosis of the disease. In all patients preoperative applied selective α-blocker - doxazosin (4-12 mg / day.) To stabilize the state and achieve the desired antihypertensive effect. When surgical treatment pheochromocytomas bilateral localization authors adhere to the two-stage tactics, removing the first stage of a large magnitude on the tumor. The interval between the stages of the operation was 2-13 months. Special attention is attached to the nature of drug replacement therapy. Selected treatment-diagnostic tactics in the late period allowed the authors to achieve good results in most cases. Failure to follow the mode of application of preparations aimed at hormone replacement therapy, one death was observed. With the authors recommend the need for a rigorous medical examination of patients and proper conduct of hormone replacement therapy, the aim of adrenal insufficiency prevention and treatment.

THE FARMAKOGNOSTIC STUDY OF GENUS CERASTIUM PLANT OF CENTRAL RUSSIA FLORA

Abstract

The farmakognostic study of the wild plant flora of Central Russia field chickweed (Cerastiumarvense L.) from Caryophyllaceae family (Caryophyllaceae Juss.), including the morphological-anatomical structure of herb plant, as well as the chemical composition of certain polyphenolic and steroid compounds using high-performance liquid chromatography (HPLC) were made.Identification was carried out by analyzing the substances UV spectra, which were obtained by HPLC. As a result of microscopic examination it was found as having a simple trichomes, druses of calcium oxalate, anomocytictype of stomata, open collateral bundle. Also was defined chemical composition of plant, comprising a polyphenolic compounds: tannin, epicatechingallate, epicatechin, coumarin, luteolin-7-glycoside, rutin, and phytoecdysteroidekdisten (ecdysterone).

SYSTEMIC AL-AMYLOIDOSIS: PROBLEMS OF DIAGNOSIS (LITERATURE REWIEW AND CLINICAL CASE

Abstract

As a result of somatic mutations, an abnormal clone of bone marrow plasma cells is formed in AL-amyloidosis. They produce amyloidogenic light chains of immunoglobulins, which are deposited in the form of insoluble amyloid fibrils in tissues of target organs. The clinical picture of AL-amyloidosis is diverse and determined by a primary involvement of various organs into the pathological process. One of the most severe manifestations is heart involvement. Diagnosis of amyloidosis is based on AL-detection of monoclonal paraprotein by means of immunoelectrophoresis of serum and urine proteins, and also quantifying free light chains of immunoglobulins in the serum. The results of biopsy with histological examination play the decisive role in the formulation of a definitive diagnosis. The goal of the treatment is to inhibit the proliferation of a plasma cells clone in order to reduce the production of immunoglobulin light chains. The clinical case of systemic amyloidosis associated with multiple myeloma is described.
