At the origins of Saint-Petersburg state pediatric medical university: the contribution by an outstanding pediatrician K. A. Rauchfuss

Abstract

Based on archival and published materials, the article describes the activity of an outstanding pediatrician and one of the founders of pediatrics in Russia K. A. Rauchfuss (1835-1915). His contribution to clinical pediatrics, training of pediatricians and the development of new children’s hospitals with regard to the age-related pathology, as well as the style of his management and principles of building relationship with colleagues, patients and their relatives is consi-dered. The role of K. A. Rauchfuss in the organization of the Children’s Hospital in Memory of Sacred Coronation of their Imperial Majesties (Saint-Petersburg, 1905) and his idea to open the Institute for the Protection of Motherhood and Infancy are highlighted. At present, the Saint-Petersburg State Pediatric Medical University is taking steps to commemorate the name of K. A. Rauchfuss, particularly to restore the memorial plaque that was put on the wall of one of the pavilions in 1909.

Diagnosis of celiac disease in routine clinical practice

Abstract

During 10 years there has been conducted the purposeful clinical and laboratory investigation of 1775 children aged from 6 months to 18 years. In all patients antibodies to glyadin and tissue transglutaminase have been defined. 494 (27,83 %) children with positive serological markers have been send to endoscopic investigation with subsequent morphological investigation of biopsy material. In selective sampling of children the morphologically confirmed celiac disease is occurred with the frequency 1 to 40. Morphometric indices of duodenum mucous membrane are characterized with decrease of mucous membrane thickness, height of villi, ratio of villi height to depth of crypts. As a compensation crypts depth and a number of interepithelial lymphocytes are increased. Small changes correspond to manifest form of celiac disease. Probability of morphological confirmation of celiac disease diagnosis is higher in children to 7 years old.

Genetic markers of celiac disease: modern concern

Abstract

According to the results of recent scientific studies there is a strong evidence to characterize celiac disease as genetic disorder associated with antigens of major histocompatibility complex (MCH II) HLA-DQ2 and HLA-DQ8. Different loci of HLA molecules are encoded by specific alleles and their combination determines the individual risk of celiac disease formation. In this article authors discuss data on structure of DQ2 and DQ8 molecules, the frequency of different alleles detection in celiac patients and possible classification of genetic risk of celiac disease formation according to HLA-DQ genotype.

To assess the effect of menopausal syndrome manifestations in the manifestation of «dry eye» examined 63 women . All examined the condition of the ocular surface, tear film stability, severity of subjective discomfort . Revealed that the dry eye syndrome in postmenopausal women occurs predominantly in the mild and moderate forms, with the severity of clinical and functional features of dry eye syndrome significantly correlated with severity of menopausal symptoms .

Features of surgical treatment of nasolacrimal duct stenoses in children

Abstract

42 children at age from 1 to 15 years (36 - with relapse of congenital nasolacrimal duct (NLD ) stenosis and 6 - with canalicular stenosis) bougienage NLD was done with Ritleng stent intubation. The control group included 25 children 1-5 years old with relapsed congenital stenosis of the NLD, the treatment of which was limited to probing the NLD. In the first group full functional effect was observed in 21 children (58.3 %), in 9 (25.0 %) - was light epiphoria, in 1 (2.8 %) - moderate, and in 5 (13.9 % ) treatment was not effective. In the second group a full functional effect was obtained for each child. In the control group lacrimal drainage was restored fully in 16 (64.0 %) children, in 2 (8.0 %) - moderate and in 7 (28.0 %) - severe epiphora was remained. Using of silicone intubation with the Ritleng lacrimal system allows to continuously retain their foramen in stenotic site without causing an inflammatory response and bedsores lacrimal punctum and canaliculus.

Methods of integrated diagnostics attention deficit hiperactivity disorderes (ADHD) in children

Abstract

It was proved by cross-correlational analysis of EEG of the cohort children of 3-11 years with attention deficit/hyperactivity disorder (ADHD), called minimal brain dysfunction (MDM). On the basis of comprehensive diagnostics, including - for the cross-correlation analysis of EEG computer (QEEG) in normal and ADHD construct therapeutic tactics in the treatment of children with this syndrome. Using QEEG data, the authors confirm the violations of interaction between structures of the cerebral cortex with ADHD compared to the norm, especially between the frontal and parietal-occipital region, as well as between the right parietal-occipital region with all the other departments of the cortex.

Allergic case rate of children and adolescents in the modern ecological conditions

Abstract

The results of analysis of ecological situation and dynamics of statistics’ (Form 12) case rate at the allergic pathology of children aged 0-14 and 15-17 years in various regions of Russia in 2001-2011 are presented. During recent years analysis of atmospheric air conformity to Sanitary Legislation has been showing steady trend to decrease of quantity of samples exceeding hygienic standards both in the whole of RF and in federal regions. However, it was revealed that in more than 35 territories atmospheric air pollution reached level of 5 and more maximum permissible concentrations. Sanitary state of water reservoirs continues to remain unsatisfactory. The correlation of abundance of bronchial asthma, allergic rhinitis, atopic dermatitis with ecological situation (level of impurity of air and water) of subjects of Russian Federation is established. During the rehabilitation of children with allergic diseases it is recommended to conduct improving of sanitary-and-hygienic indexes of environment, the durable monitoring of health and improvements taking into account complex influence of technogenic pollution on the organism of the child.

Treatment fecal incontinence in children with bulking agents «DAM+»

Abstract

One of the reason of organic and neurogenic fecal incontinence in children is low resting pressure in the anal canal. In functional aspect, one of the main roles plays internal sphincter of the anal canal that provides 50-85 % of basal resting pressure and close of the anal canal. Purposes. The purpose of this article is to study the possibilities of using bulking agent «DAM+» to increase resting pressure in the anal canal in children with fecal incontinence, and evaluate the results of this treatment. Metods. The study has been performed in the Department of Pediatric Surgery in our university since 2007 for 2013. In total, 35 patients, aged 2-18 with organic or neurogenic fecal incontinence were underwent 57 procedure of introduction polyacrylamid gel «DAM+». Patients with overflow fecal incontinence were excluded. Before treatment and after, all patients were assessed clinical examination and anal manometry. Anal manometry was perfomed оn multifunctional apparatus «Menfis 2000». Results. The average quantity of resting pressure in the anal canal before operation is formed 20.98 ± 5.17 сm. (H2O), after operation 32.62 ± 6.63 сm. (H2O), in long-term period 28.07 ± 6.65 сm. (H2O) Conclusion. The clinical efficiency of procedure correlates with values of resting pressure in the anal canal before and after treatment. Implantation of «DAM+» into submucosal layer provides increasing of basal resting tone in the anal canal. Imitation of work of the internal anal sphincter and expansive vascular anal cushions on the other part, prevents patulous anus and provides a hermetic seal, that responds for close anal walls. Statistical data was shown, that the average quantity of resting pressure in the anal canal before operation and after operation has performed at the 5 % significance level. P-values < 0.05 was considered statistically significant.

Abstract

The possibility in principle of the conscious psychophysiological decreasing of the circadian rhythm disorders was researched in the example of intestinal tract evacuation function. The investigation was fulfilled in medical students by the way of additional informing them about chronophysiological regularity importance of defecation rhythm. It is shown that getting them this information promotes three basic effects: increasing the frequency of morning defecation rhythm; diminution the risk of the bradyenteria heavy-stage occurrence due increased probability of its modifying into more easy stages; significant elevation of the sleeping gratification.

Sex differences of the content on the monoamines levels in symmetrical structures of the C3H-A mice brain

Abstract

The Sex differenses in the content and metabolism of dopamine and serotonin were studied in symmetrical brain structures of C3H-A mice. With HPLC the contents of norepinephrine (NE), dopamine (DA), serotonin (5-HT) and their metabolites, such as dihydroxyphenylacetic acid (DOPAC), homovanillinic acid (HVA) and 5-hydroxyindolacetic acid (5-HIAA), were measured in the cortex, tuberculum olfactorium, hippocampus and striatum of both the right and the left hemispheres of the brain in male and female mice. The following sex differences in monoamines and their metabolites in brain areas were found: the NE content was higher in the male striatum and in the female tuberculum olfactorium; in males the DA content in cortex and hippocampus was higher, but in tuberculum olfactorium and striatum was lower than that in females; in females the 5-HT and 5-HIAA levels in hippocampus and tuberculum olfactorium were hither than that in males. In the female left striatum the 5-HIAA content was higher than in males. In males three cases of neurochemical cerebral hemisphere asymmetries were found: 1) the NE content is higher in the right tuberculum olfactorium, 2) the DA level is higher in the right hippocampus, 3) the 5-HIAA content is higher in the left hippocampus. In females the only one case of cerebral asymmetry was found, i. g. the 5-HT level was higher in the right tuberculum olfactorium.

Individual and group peculiarities of adaptation in ontogeny (review and problem statement)

Abstract

А complex of biochemical, hematologic and physiologic parameters determine individual variability that should correlate with general resistance of the organism. Hypoxia, a universal pathologic process is recommended as a model for studying individual adaptation in ontogeny. Hypophysis-adrenals system (HAS) activity may produce decisive effect upon the organism’s response reactions’ trend. HAS activity changes with age. Parallel to these changes resistance to hypoxia also alters. This process is gender-dependant. Individual and group peculiarities of resistance to acute hypoxia at various ontogenic periods should be compared to HAS functional state at respective periods separately for males and females. These correlations would facilitate better understanding of resistance variability general mechanisms characteristic of individual and group levels.

Actual aspects of sepsis and septic shock

Abstract

He review presents an analysis of up-to-date views on sepsis and septic shock. Results of consenting conferences are given with classification of sepsis in adults and children. Mortality indexes are presented in patients’ group with sepsis. Basic pathogenesis links are examined i.e. bacteriemia, microbe toxemia, endo(auto)toxicosis, systemic destructive vasculitis, growing hypercoagulation transforming into coagulopathia, consumption trombocytopenia with trombohemorrhagic syndrome and severe immunesuppression. Pathogenesis of septic shock is divided into processes developing in various organs and tissues as well as into intracellular ones: such as oxidative stress and mitochondrial insufficiency. The role of pathogen-associated molecular images - patterns is portrayed in the development of generalized acyclic infection process. A scheme of up-to-date septic shock therapy is presented. An important role of nitrogen oxide in the development of stabile hypotonia resistant to vasopressin therapy is proven. The potency of nitrogen oxide to produce free-radical peroxinitrite inducing lipids peroxide oxidation in membranes is reflected. Its ability to react with non-hem iron- and zink- containing proteins is mentioned. Key factors contributing to activation of genes responsible for inducible nitrogen oxide synthase operation are revealed. Extremely severe sepsis and septic shock are believed to be the most serious problem of contemporary medicine thus necessitating to produce new medicines affecting the most drug-resistant links of its pathogenesis. In many a countries trials are continuing to introduce new medicines like nitrogen oxide synthase inhibitors for the treatment of patients with septic shock.

Psychological peculiarities of auditory cognitive defect when psychopathology

Abstract

Work is executed on faculty of normal physiology of Saint-Petersburg state pediatric medical University of the Ministry of health of Russia and St. Petersburg psycho-neurological dispensary N 1. Of the existing Arsenal of psychophysiological methods in this study, we used the original method of studying perception of auditory images and psychological analysis of the status of higher mental functions in patients suffering from paranoid schizophrenia and control group. With increasing levels of anxiety, both healthy subjects and in patients of any information provided may be assessed as important. In such circumstances, the inhibition processes touch flow reduced and insignificant background is estimated sick longer than healthy. Normal perception clean and noisy auditory images has high identifiability (95 %) and efficiency with latent period of 4.2-8.4 C. Taking into account time for вербализацию a decision and biologically significant effect on the perceived information, this time interval corresponds to the physiological norm. The effectiveness of the perception of auditory images in chronic psychopathology is within 40-75 %, and the latent period - within 11.2-24.3 with that 2.6-2.9 times exceeds the normative time of identification. Reduction in speed and efficiency of information-analytical activities in psychopathological disorders caused, apparently, by the decrease of the functional state of stem-reticular structures included in the first structural-functional Department of the auditory analyzer. The dependence of the quality of auditory-cognitive activity not only from the functional state of cortical fields, responsible for attention, perception, recognition, remembering, but also on the state of slightly-stem systems of the brain responsible tone and reactivity of the brain. Registered reliable link of functional reduce nonspecific brain systems in violation of the neurophysiological mechanisms of perception.

On the formation of professional self-determination of future doctors

Abstract

The article discusses the features of professional self-determination of Medical University students of the 1st and 6th courses. A set of experimental psychological methods was used: the method of «Un-finished phrases» which reveals the attitude of medical students to their future profession; questionnaire aimed at assessing the importance of personal qualities in the doctor’s work (students’ opinion); diagnostic technique of value orientations in career «Career anchors». It is revealed that the freshmen have an idealized view of medical practice; graduates’ perception of the medical profession includes communication skills to interact with the patient, they are aware of the impact of their personal properties and needs for the professional activity. Learning process, observing elder colleagues’ interaction with patients and experience of working in health care facilities affect on the development of students’ professional self-determination. In the process of training and education of medical students it’s necessary, given the importance of career orientation «service», to complete its with realistic content, as it is one of the motivating factors for the development of professional competence and further self-determination of a doctor.

Adaptive capabilities under extreme conditions, depending on motivational orientation of Anesthesiologist

Abstract

The aim is to identify the level of adaptive capacity anesthesiologists depending on the motivational orientation. Presented survey data 149 anesthesiologists. Testing was conducted on a multi-level personality questionnaire «Adaptability» and on the test Kuhl. The results showed that most physicians have high adaptive capacity, which is highly correlated with the motivational oriented under extreme conditions. The necessity of developing a program of psychological support to the development of non-motivational deficit.

On the history of pediatric stomatology in Russia

Abstract

Pediatric stomatology became an independent discipline after being a part of general dentistry, so the history of their development are closely linked. The first descriptions of dental diseases in children in the domestic literature refer to the XVIII century. In the late XIX-early XX century a specialized dental care for children was established. AK Limberg should be considered a pioneer of pediatric stomatology in Russia. The article shows the role of pediatric stomatology departments (currently there are 27 of them in Russia) in the development of the specialty. Targeted reduction of dental morbidity of the child population is possible through implementing of a complex program, including the organization of dentist care, prevention and treatment of main dental diseases: dental caries and its complications, inflammatory periodontal diseases, dentoalveolar malformations. Implementation of these measures includes the cooperation of the health system, public education and public catering.

Medical service organization for women and children - the Second World War repatriates

Abstract

Women, elderly and children are those who suffer from wars more than the others. The misery increases in case they are prisoners of war. Women became war victims not only by having been wounded, seek, killed, imprisoned but also as they were driven to Germany as labor force. There they were met by hunger, torment and illnesses. The number of children to be repatriated in the late 1945 was more than 630 000, from which 22 000 were orphans. For the first time in the history of medicine there was organized and tested on practice the system of medical help and treatment pointed at women-repatriates and children, that allowed to save their lives, return them to the army and to work as well as to normal physiological activity.
