Scientific events of the Turner Scientific And Research Institute for children’s orthopedics as a form of continuous medical education for pediatric traumatologists

Abstract
The experience of the Turner Scientific and Research Institute for Children's Orthopedics in educational activities for improvement of the professional knowledge of pediatric physicians was represented. The target audience of the continuous medical education include traumatologists, pediatric surgeons, and doctors of related specialties of Russia that are involved in diagnosis, treatment, and rehabilitation of children with injuries, congenital and acquired diseases of the musculoskeletal system. Since 1986, the Institute has organized 28 all-Russian scientific and practical conferences on topical issues of traumatology and orthopedics of pediatric age in 22 different cities across the country. In the interest of the institute, the school of pediatric orthopedists is constantly working for district orthopedists of children's polyclinics of St. Petersburg, and regular monothematic seminars are performed with the participation of leading Russian experts and visiting lecturers from abroad. These scientific and practical activities improve the professional skills of doctors and help them improve the provision of specialized care to children.

The Order of the Minister of Health of the Russian Federation of 11/11/2013 № 837, with the addendum of 06/09/2015 № 328, approved the provision of a model for the basic principles of continuous medical education for specialists with higher education. Implementation of this model is expected to be provided in the framework of organizations under the authority of the Russian Ministry of Health, which performs the educational activities. The modular principles of constructing educational programs and various types of educational activities are considered, such as a combination of distance education and e-learning [1].
We report on the experience of the Turner Scientific and Research Institute for Children’s Orthopedics for the implementation of educational activities to improve the professional knowledge of orthopedists, traumatologists, pediatric surgeons, and doctors of related specialties in Russia who conduct diagnostic examinations, treatment, and rehabilitation of children with injuries and congenital and acquired diseases of the musculoskeletal system.
Since 1986, the Turner Scientific and Research Institute for Children’s Orthopedics has been an organizer of annual scientific and practical conferences on topical issues of traumatology and orthopedics of early age, in which specialists from all administrative territories of Russia take part [2]. To expand the audience of participants through involvement of regional doctors, 28 scientific and practical events were held in 22 different cities of the country. Pediatric traumatologists and orthopedists were welcomed in megacities (Moscow, St. Petersburg, Kazan, Voronezh, Rostov-on-Don, Volgograd, Izhevsk, Yekaterinburg), cities of the North-West (Vologda, Pskov, Cherepovets, Syktyvkar), central regions of Russia (Yaroslavl, Vladimir, Saratov, Cheboksary, Orel), and Siberia (Kurgan, Barnaul), as well as resorts where rehabilitation of children with pathological conditions of the musculoskeletal system is performed (Svetlogorsk of the Kaliningrad region, Staraya Russa, Gelendzhik).
The conference holding was supported by the Priorov Central Scientific Research Institute of Traumatology and Orthopedics and the Association of Traumatologists and Orthopedists of Russia. The Saratov; Ural; Kazan Research Institute of Traumatology and Orthopedics; the Federal Centers of Traumatology and Orthopedics in Kurgan, Cheboksary, and Barnaul; single-profile departments of medical universities; and the Health Committees of territorial administration provided the most active assistance in organizing events on their territories [2-4].
Approximately 200–250 participants from scientific and educational institutions of Russia and foreign countries and practical doctors attended the conferences. The topics of discussion included a wide range of questions on diagnostics, conservative and surgical treatment of injuries and their consequences, and congenital and acquired orthopedic pathology of children. The aim of the scientific events was the introduction of innovative projects into practical health care. The reports were supplemented with master classes and lectures by leading Russian and foreign orthopedists. Sometimes, one most urgent problem was chosen for a symposium. For example, in Gelendzhik (1997) and Yekaterinburg (2011), the reports were devoted to rehabilitation of disabled children with orthopedic, and orthopedic and neurologic pathology. In Izhevsk (1998), the innovative programs for treatment of large joints were of the highest priority. In Volgograd (2003), the errors and complications in treatment of injuries and diseases of the musculoskeletal system were analyzed. In Barnaul (2015), the main topic was devoted to the pathology of the lower extremities, and in St. Petersburg (2016) the pathologies of the upper limbs in children were discussed.
In Rostov-on-Don (1999), the first professional standards of technology for specialized care for children with the 29 most common orthopedic diseases and the consequences of injuries were discussed at the round table [5]. For each pathological condition, the algorithms for compulsory and additional examination methods, technologies for conservative and surgical treatment, criteria for assessing quality, recommendations for prevention of possible complications, and the program of rehabilitation and dispensary observation were compiled. The standards were developed by leading scientists of the Turner Scientific and Research Institute for Children’s Orthopedics and published in the materials of the conference, which helped doctors of practical healthcare working with the funds of compulsory medical insurance when creating territorial standards for treatment of children.
For many years, the organization of scientific and practical conferences has shown the undoubted benefit of such meetings for pediatric traumatologists and orthopedists from different parts of Russia in terms of professional growth, and communicating and exchanging experiences among specialists with different levels of knowledge. The spirit of creativity, mutual respect, and assistance is consistently present at the meetings, and the social connections arisen are becoming stronger from year to year.
For more than 30 years, the Pediatric Orthopedic School has been working in the Institute for Regional Orthopedists of Children’s Polyclinics in St. Petersburg and the Leningrad Region. Monthly sessions include clinical patient analysis for planning treatment tactics, demonstrating the results of the treatment performed, and lectures on topical issues of the specialty. A high level of professional knowledge is necessary for outpatient specialists who, when examining large contingents of children, detect congenital malformations and diseases of the musculoskeletal system at an early stage, perform rehabilitation of patients after hospital discharge to prevent relapses and complications, and perform preventive medical examinations and rehabilitation of children with disabilities. In 2002, with the participation of the Institute, the Association of Pediatric Orthopedists and Traumatologists of St. Petersburg was established. The school program was supplemented with regular monothematic 1-day seminars with participation of leading Russian experts and guest lecturers from foreign countries (United States, Austria, Germany, Israel, Finland, and so forth). Approximately 100 orthopedists from different regions of Russia, Ukraine, and Belarus come to every seminar, except for doctors from St. Petersburg and the Leningrad region [6].
From 09/29/2016 to 09/30/2016, with the support of the Association of Traumatologists and Orthopedists of Russia (ATOR) and the Association of Pediatric Orthopedists and Traumatologists of St. Petersburg (APOT SPb), a scientific and practical conference was conducted for the first time with international participation regarding “Congenital and acquired pathology of the upper extremities in children (diagnosis, treatment, rehabilitation)” in accordance with the Regulation on the basic principles of continuous medical education. The topic of discussion was relevant; 59 reports were presented and 3 master classes were held. The invited lecturers from Finland, Austria, and the United States presented the results of their research. The conference was attended by more than 200 doctors from all regions of Russia. After answering the test questions to assess the knowledge gained, the participants received certificates with 12 credit points [7].
Thus, it can be stated that the experience of training activities conducted by the Turner Scientific and Research Institute for Children’s Orthopedics complies with the basic principles of the Regulation on continuous medical education of traumatologists, orthopedists and surgeons who treat the injuries and diseases of the musculoskeletal system in children.
