2024, Martynov I.D., Yamshchikova A.V., Fleishman A.N., Petrovskiy S.A.</copyright-statement><copyright-statement xml:lang="ru">Copyright ©; 2024, Мартынов И.Д., Ямщикова А.В., Флейшман А.Н., Петровский С.А.</copyright-statement><copyright-year>2024</copyright-year><copyright-holder xml:lang="en">Martynov I.D., Yamshchikova A.V., Fleishman A.N., Petrovskiy S.A.</copyright-holder><copyright-holder xml:lang="ru">Мартынов И.Д., Ямщикова А.В., Флейшман А.Н., Петровский С.А.</copyright-holder><ali:free_to_read xmlns:ali="http://www.niso.org/schemas/ali/1.0/" start_date="2024-05-29"/></permissions><self-uri xlink:href="https://journals.eco-vector.com/0016-9900/article/view/638580">https://journals.eco-vector.com/0016-9900/article/view/638580</self-uri><abstract xml:lang="en"><p><italic><bold>Introduction.</bold> In occupational polyneuropathies, along with sensory and autonomic disorders, excessive sensory input can lead to the development of excitation foci in various parts of the central nervous system, clinical significance of which is currently insufficiently studied.</italic></p><p><italic><bold>The aim of the study</bold> was the evaluation of the clinical effects of the transcranial magnetic stimulation in the dorsolateral zone of the prefrontal cortex in mine workers with occupational polyneuropathy. </italic></p><p><italic><bold>Materials and methods.</bold> There were examined thirty six workers of coal mining enterprises of Kuzbass with a proven diagnosis of polyneuropathy of the upper extremities, the average age was 50.2±3.7 years, and the average work experience was 23.4±3.5 years. To confirm polyneuropathy, the parameters of sensory conduction along the nerves of the upper extremities were investigated using electroneuromyography, nerve conduction velocity along the median and ulnar nerves was determined. The assessment of autonomic rcontrol was carried out using spectral analysis of heart rate variability. The functional brain status was determined by an electroencephalogram.</italic></p><p><italic><bold>Results.</bold> The patients complained of pain in the extremities, numbness and paresthesia. According to electroneuromyography, there was a decrease in the nerve conduction velocity along the somatic sensory nerves of the upper extremities, a reduction in the power of oscillations in all frequency ranges of the heart rate variability spectrum, which was more pronounced in the high frequency range. Transcranial magnetic stimulation of the prefrontal cortex zone of the right hemisphere was performed with a stimulus with a frequency of 1 Hz for 5 min, over a course of 5 procedures, after which a statistically significant increase in the average nerve conduction velocity along the somatic sensory nerves of the upper extremities, and a rise in the spectral parameters of the heart rate variability were observed. An increase in the alpha rhythm index at the electroencephalogram indicated an amplification of the integrative brain activity, which provided a wide range of regulatory processes. The patients noted a decrease in the intensity of pain, the severity of sensitive disorders, and improvement in general well-being.</italic></p><p><italic><bold>Limitations.</bold> The study is limited to the number of mine workers with a proven diagnosis of upper extremity polyneuropathy, without cardiac arrhythmias, structural damage and metal implants in the brain.</italic></p><p><italic><bold>Conclusion.</bold> Transcranial magnetic stimulation of the dorsolateral zone of the prefrontal cortex is effective in correcting sensory and autonomic disorders in occupationally caused polyneuropathy by activating suprasegmentary autonomic centers involved in systemic adaptation processes. Клиническое значение этого явления в настоящее время недостаточно изучено.</italic></p><p><italic><bold>Цель исследования</bold> — оценка клинических эффектов транскраниальной магнитной стимуляции дорсолатеральной зоны префронтальной коры у горнорабочих с профессиональной полинейропатией.</italic></p><p><italic><bold>Материалы и методы.</bold> Были обследованы 36 работников угледобывающих предприятий Кузбасса с установленным диагнозом «полинейропатия верхних конечностей», средний возраст 50,2 ± 3,7 года, средний стаж работы — 23,4 ± 3,5 года. Для подтверждения полинейропатии исследовали параметры сенсорного проведения по нервам верхних конечностей с помощью электронейромиографии, определяли скорость распространения возбуждения по срединным и локтевым нервам. Оценку вегетативной регуляции проводили с помощью спектрального анализа вариабельности ритма сердца. Функциональное состояние головного мозга определяли с помощью электроэнцефалограммы.</italic></p><p><italic><bold>Результаты.</bold> Пациенты предъявляли жалобы на боли в конечностях, онемение и парестезии. По данным электронейромиографии наблюдалось снижение скорости распространения возбуждения по соматическим сенсорным нервам верхних конечностей, снижение мощности колебаний во всех частотных диапазонах спектра вариабельности ритма сердца, более выраженное в диапазоне высоких частот. Проводили транскраниальную магнитную стимуляцию зоны префронтальной коры правого полушария стимулом с частотой 1 Гц в течение 5 мин (курс — 5 процедур), после чего наблюдали статистически значимое увеличение средней скорости распространения возбуждения по соматическим сенсорным нервам верхних конечностей, повышение спектральных показателей вариабельности ритма сердца. Увеличение индекса альфа-ритма на электроэнцефалограмме свидетельствовало об усилении интегративной деятельности мозга, обеспечивающей широкий круг регуляторных процессов. Пациенты отмечали снижение интенсивности болей и выраженности чувствительных нарушений, улучшение общего самочувствия.</italic></p><p><italic><bold>Ограничения исследования.</bold> Исследование ограничено числом горнорабочих с установленным диагнозом «полинейропатия верхних конечностей», без нарушений сердечного ритма, структурных повреждений и металлоимплантатов головного мозга.</italic></p><p><italic><bold>Заключение.</bold> Транскраниальная магнитная стимуляция дорсолатеральной зоны префронтальной коры эффективна в коррекции сенсорных и вегетативных нарушений при профессионально обусловленной полинейропатии за счёт активации надсегментарных вегетативных центров, участвующих в системных адаптационных процессах.
