Гидравлическая теория патогенеза посттравматического остеоартрита коленных суставов и перспективы лечебных мероприятий

Аннотация
Предложена оригинальная гидравлическая теория патогенеза остеоартрита (ОА) коленных суставов (КС). Гидравлическая теория предполагает наличие первоначального травматического или бытового повреждения менисков, крестообразных связок и синовиальной оболочки с последующим посттравматическим увеличением объема синовиальной жидкости и, соответственно, гидростатического давления внутри синовиальной полости. Повреждение клеточного слоя синовиальных макрофагов, составляющих основу интимы и синовиального барьера, приводит к просачиванию синовиальной жидкости в субинтиму с развитием клеточной альтерации и последующего разрастания фиброза и нарастания бесконтрольных гиперплазии, клеточной гипертрофии и новообразования сосудов и нервных окончаний. Более позднее повреждение мощного костно-хрящевого барьера приводит к интрузии синовиальной жидкости в костную ткань с формированием костных кист и остеоартритному повреждению/отеку костного мозга. Восстановление барьеров путем применения репарационных, в том числе клеточных технологий в настоящее время представляется невозможным ввиду отсутствия у синовиальных макрофагов и хондроцитов регенерационного потенциала. Повреждение синовиального и костно-хрящевого барьера предполагает различную медицинскую тактику ведения пациентов с ОА КС. Авторы обосновывают нежелательность любого вида обезболивающей терапии при повреждении синовиального и сохранении целостности костно-хрящевого барьера. Безальтернативным эффективным и безопасным методом лечения на всех стадиях заболевания является лечебная физкультура с акцентом на упражнения, ускоряющие лимфатический и венозный отток с целью достижения антифиброзного и антигипоксического эффектов.
