To study the prevalence of different anogenital HPV types in patients with varying severity of HPV-associated cervical intraepithelial neoplasia. Subject and methods. Clinical studies, extended colposcopy; high-resolution anoscopy; cytological examination; target biopsy of the cervix uteri; histological examination of biopsy specimens; HPV typing and qualitative assessment. Results. A total of 159 women (mean age 30±12 years) were examined. Five groups were formed according to the examination results: 1) 38 (23.9%) patients with CIN I-III at that moment; 2) 41 (25.8%) with a history of CIN I-III; 3) 35 (22. 0%) with anogenital condylomas; 4) 29 (18.2%) with HPV carriage; 5) 16 (10.1%) healthy women. Cervical and anal HPV was detected in 79 (49.7%) and 65 (40.9%) patients. HPV group A9 was significantly more common in the cervical canal (OR = 2.8 (1.3-5.9), p = 0.005) whereas HPV group 10 was in the anus (OR = 2.5 (1.1-5.6), p = 0.025). The most common HPV types were 16 (41.7%), 31 (15.1%), 44, 33 (12.6%), and 52, 68 (11.3%) in the cervix and 16 (29.2%), 53 (21.5%), 66 and 44 (16.9%), 31 (13.8%), 56 and 39(12.3%), and 6(10.7%) in the anal canal. In Groups 1, 2, 3, and 4, the cervix was detected to have HPV 16 in 52.6, 4.9, 8.6, and 27.6% of cases; HPV 33 in 7.9, 2.4, 0, and 20.7%; the anal canal more frequently contained HPV 16 in 23.7, 4.8, 8.6, and 17.2%; HPV 31 in 5.3, 0, 5.7, and 17.2% of cases; HPV 66 in 2.6, 4.8, 11.4, and 13.8%; HPV 56 in 15.8, 0, 2.9, and 3.4%; HPV 53 in 13.2, 7.3, 5.7, and 13.8%. Conclusion. In the patients with HPV-associated anogenital diseases, HPV types were ascertained to be different in the cervical canal and anal area. HPV group A9 was significantly more often encountered in the epithelium of the cervical canal than in the anus of patients with CIN, which allows consideration of the presence of CIN as a risk factor for the anal area to be infected with these types of the virus.</p></abstract><trans-abstract xml:lang="ru"><p>Цель исследования. Изучить распространенность различных типов вируса папилломы человека (ВПЧ) аногенитальной области у пациенток с ВПЧ-ассоциированными цервикальными интраэпителиальными неоплазиями (CIN) различной степени тяжести. Материал и методы. Обследованы 159 женщин в возрасте 18-60 лет, обратившихся в связи с патологией шейки матки, с помощью следующих методов: клинический, расширенная кольпоскопия; аноскопия с высоким разрешением; цитологический; прицельная биопсия шейки матки; гистологическое исследование биопсийного материала; типирование и количественная оценка ВПЧ. Результаты. По результатам обследования сформировано 5 групп: 1-я - 38 женщин (23,9%) с CIN I-III на данный момент, 2-я - 41 женщина (25,8%) с CIN I-III в анамнезе, 3-я - 35 женщин (22,0%) с аногенитальными кондиломами, 4 - 29 женщин (18,2%) с ВПЧ-носительством, 5 - 16 (10,1%) здоровых женщин. Наличие цервикального ВПЧ выявлено у 79 (49,7%), анального - у 65 (40,9%) пациенток. В цервикальном канале достоверно чаще встречались ВПЧ группы А9 (OR=2,8 (1,3 -5,9), p=0,005); в анусе - ВПЧ группы А10 (OR=2,5 (1,1-5,6), p=0,025). Наиболее часто встречающимися типами ВПЧ в цервиксе были 16 (41,7%), 31 (15,1%), 44, 33 (12,6%), 52, 68 (11,3%), в анальном канале - 16 (29,2%), 53 (21,5%), 66 и 44 (16,9%), 31 (13,8%), 56 и 39 (12,3%), 6 (10,7%). В1, 2, 3, 4-й группах в цервиксе выявлялись ВПЧ 16 - 52,6%, 4,9%, 8,6%, 27,6%; 33 - 7,9%, 2,4%, 0, 20,7%. В анальной области в 1, 2, 3, 4-й группах чаще встречались ВПЧ 16 - 23,7%, 4,8%, 8,6%, 17,2%; 31 - 5,3%, 0, 5,7%, 17,2%; 66- 2,6%, 4,8%, 11,4%, 13,8%; 56- 15,8%, 0, 2,9%, 3,4%; 53 - 13,2%, 7,3%, 5,7%, 13,8%. Заключение. У пациенток с ВПЧ-ассоциированными заболеваниями аногенитальной области выявлены различия типов ВПЧ в цервикальном канале и анальной области.
