To comparatively analyze the course of 167 pregnancies in 95 women with Ph-negative myeloproliferative neoplasms (MPN) who have been treated and untreated for the underlying disease. Material and methods. Complications of pregnancy, delivery, and postpartum period and neonatal health status were evaluated in both groups. Results. The creation of a program for pregnancy preparation and management in women with Ph-negative MPN led to a significant reduction in reproductive losses from 76.3% to 3.3% and to a downward trend in the number of pregnancy complications from 84,6% to 74.2%. The health status and frequency of malformations in children born to the mothers with MPN who had been treated during pregnancy did not differ from those in the healthy population. Conclusion. Pregnant women with Ph-negative MPN belong to a high-risk group. The results of the investigation have shown that pregnancy has no impact on the course of myeloproliferative disease. The algorithm elaborated by the authors could substantially reduce reproductive losses and the number of pregnancy complications in women with MPN. The health status of the children does not differ from that in the general population.</p></abstract><trans-abstract xml:lang="ru"><p>Цель исследования. Провести сравнительный анализ течения 167 беременностей у 95 женщин с Ph-негативными миелопролиферативными новообразованиями (МПН), как получавших терапию основного заболевания, так и находившихся без лечения. Материал и методы. В обеих группах проведена оценка осложнений течения беременности, родов и послеродового периода, а также состояние здоровья новорожденных. Результаты. Создание программы подготовки, ведения беременности у женщин с Ph-негативными МПН привело к статистически значимому снижению репродуктивных потерь с 76,3 до 3,3% и тенденции к снижению числа осложнений беременности с 84,6 до 74,2%. Состояние здоровья и частота пороков развития детей, рожденных у матерей с МПН, которым проводилось лечение во время беременности, не отличаются от детей в здоровой популяции. Заключение. Беременные женщины с Ph-негативными МПН относятся к группе высокого риска. Результаты нашей работы показали, что беременность не оказывает влияния на течение миелопролиферативного заболевания. Разработанный нами алгоритм позволил существенно снизить репродуктивные потери и число осложнений во время беременности у женщин с МПН.
