Antibodies against ACE2 have predictive value for COVID-19 and may contribute to RAS dysregulation and reproductive failure.</italic></p> <p><bold><italic>Objective:</italic></bold><italic> To investigate the prevalence and levels of anti-ACE2 autoantibodies in infertile patients with a history of COVID-19 and in fertile women.</italic></p> <p><bold><italic>Materials and methods:</italic></bold><italic> Serum anti-ACE2 autoantibodies (M, G) were determined by ELISA in infertile patients with a history of COVID-19 (group 1, n=121), without a history of COVID-19 (group 2, n=79), and in fertile women (group 3, n=80). The association between antibodies against ACE2, SARS-CoV-2, thyroid antigens, and hormones was investigated.</italic></p> <p><bold><italic>Results: </italic></bold><italic>Patients in groups 1 and 2 had higher rates of inflammatory gynecologic diseases, pelvic surgery, spontaneous miscarriages, and thyroid pathology than those in group 3. Anti-ACE2 antibodies were detected more frequently (40.5% and 38.8 %) and had higher levels in infertile patients than in fertile women (20%). Women with a history of COVID-19 were more likely to have anti-ACE2 IgG. Antibodies against ACE2 were significantly correlated with those against FSH.</italic></p> <p><bold><italic>Conclusion:</italic></bold><italic> Patients with infertility, irrespective of a history of COVID-19, have a higher prevalence and higher anti-ACE2 antibody levels than fertile women. Anti-ACE2 antibodies are associated with primary and secondary infertility, and may be involved in the pathophysiology of infertility.</italic></p></abstract><trans-abstract xml:lang="ru"><p><italic>Ангиотензинпревращающий фермент 2 (АПФ2) – ключевой компонент ренин-ангиотензиновой системы (РАС), влияющей на репродуктивные процессы у женщин. Антитела к АПФ2 имеют прогностическое значение при COVID-19, могут способствовать дисрегуляции РАС и нарушениям репродуктивной функции.</italic></p> <p><bold><italic>Цель:</italic></bold><italic> Изучить распространенность и уровни аутоантител к АПФ2 у пациенток с бесплодием, перенесших СOVID-19, и у фертильных женщин.</italic></p> <p><bold><italic>Материалы и методы: </italic></bold><italic>С помощью иммуноферментного анализа определяли сывороточные антитела (M, G) к АПФ2 у пациенток с бесплодием с историей COVID-19 (группа 1, n=121) и без COVID-19 в анамнезе (группа 2, n=79), а также у фертильных женщин (группа 3, n=80). Исследовали связь между антителами к АПФ2, SARS-CoV-2, антигенам щитовидной железы и гормонам.</italic></p> <p><bold><italic>Результаты: </italic></bold><italic>Группы 1 и 2 отличались от группы 3 высокой частотой воспалительных гинекологических заболеваний, операций на органах малого таза, самопроизвольных выкидышей и патологий щитовидной железы. У пациенток с бесплодием антитела к АПФ2 выявлялись чаще (у 40,5 и 38,8%), чем у фертильных женщин (у 20%), а их уровень был выше. Вероятность обнаружения анти-АПФ2 IgG была выше у женщин, перенесших COVID-19. Антитела к АПФ2 выраженно коррелировали с антителами к ФСГ.</italic></p> <p><bold><italic>Заключение:</italic></bold><italic> У пациенток с бесплодием, независимо от COVID-19 в анамнезе, отмечаются более высокая распространенность и уровни антител к АПФ2, чем у фертильных женщин.
