Embryological indicators and treatment effectiveness were assessed. The prospective study of metabolome in cumulus cells and culture media after cumulus cells co-culture was performed by high performance liquid chromatography and mass spectrometry using 90 samples of culture media after culturing cumulus cells, 90 samples of cumulus cells, as well as 10 control samples of culture media without cumulus cells.</italic></p> <p><bold><italic>Results:</italic></bold><italic> The prospective study of the influence of embryo-cumulus cells co-culture on the morphology of blastocysts and possibility of their implantation versus standard-based method using conventional culture media was performed. The study showed that autologous embryo-cumulus cells co-culture reduces the chances of implantation in women of late reproductive age by 1.2 times, and is not advisable for young patients. At the same time, co-culture significantly increases the number of high-quality blastocysts suitable for cryopreservation in the general cohort. This results in a greater likelihood of having a child with the use of a single ovarian stimulation protocol. Assessment of metabolism of cumulus cells showed that in culture media, co-cultivation increases the concentration of some amino acids (leucine, valine, serine) and amino-acid containing dipeptides. The obtained data suggest that cumulus cells are involved in lipid metabolism, initiating fatty acid oxidation, which is essential for normal embryonic development.</italic></p> <p><bold><italic>Conclusion:</italic></bold><italic> The results obtained in this study suggest that it is reasonable to refuse embryo-cumulus cells co-culture in women aged over 36 years, who had multiple IVF failures in anamnesis. Autologous embryo-cumulus cells co-culture significantly increases the number of high-quality embryos in young women.</italic></p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Цель: </italic></bold><italic>Оценить эффективность аутологичного сокультивирования эмбрионов с клетками кумулюса в программах лечения бесплодия методами вспомогательных репродуктивных технологий (ВРТ), а также проанализировать метаболический профиль отработанных культуральных сред после культивирования клеток кумулюса.</italic></p> <p><bold><italic>Материалы и методы:</italic></bold><italic> Были обследованы 127 супружеских пар, проходящих лечение бесплодия методами ВРТ. Проведена оценка эмбриологических показателей и результативности лечения. Проспективное исследование метаболома клеток кумулюса и отработанной культуральной среды после сокультивирования в ней клеток кумулюса методами высокоэффективной жидкостной хроматографии и масс-спектрометрии было выполнено на 90 образцах отработанных сред после культивирования клеток кумулюса, 90 образцах самих клеток кумулюса, а также 10 контрольных образцах сред культивирования без кумулюса.</italic></p> <p><bold><italic>Результаты:</italic></bold><italic> В данной работе проведено проспективное исследование влияния сокультивирования эмбрионов с клетками кумулюса на морфологию бластоцист и вероятность их имплантации в сравнении с классическим методом при использовании стандартной среды. Было показано, что аутологичное сокультивирование эмбрионов с клетками кумулюса в 1,2 раза снижает вероятность имплантации у женщин позднего репродуктивного возраста, а у молодых пациенток аутологичное сокультивирование нецелесообразно. При этом в общей когорте сокультивирование достоверно увеличивает число бластоцист отличного качества, пригодных для криоконсервации. Это приводит к большей вероятности рождения ребенка с одного протокола стимуляции. Оценка метаболизма клеток кумулюса показала, что в культуральных средах при сокультивировании повышается концентрация ряда аминокислот (лейцина, валина, серина) и дипептидов на их основе. Данные позволяют говорить о том, что клетки кумулюса активно участвуют в метаболизме липидов, инициируя, среди прочего, механизм бета-окисления жирных кислот, работа которого необходима для нормального развития эмбриона.</italic></p> <p><bold><italic>Заключение: </italic></bold><italic>Результаты полученного исследования позволяют рекомендовать отказаться от аутологичного сокультивирования эмбрионов с клетками кумулюса женщинам старше 36 лет с множественными неудачными попытками ВРТ в анамнезе.
