The scientific papers available in the PubMed, MEDLINE, Embase, eLIBRARY, CyberLeninka, and Google Scholar were systematically sought using the search terms “hysterectomy”, “tranexamic acid”, and “use of tranexamic acid during surgical operations” are analyzed. The authors have analyzed six investigations involving 193 616 patients. Hysterectomy belongs to common operations performed in surgical gynecology in the most countries of the world. To reduce blood loss and to prevent possible complications, various mechanical and pharmacological methods are applied. These include the use of TA that a synthetic derivative of lysine and included by the World Health Organization (WHO) into the list of essential medicines. Due to the local and systemic hemostatic effect, this substance is used in bleeding, in the pathogenesis of which a role is played by enhanced fibrinolysis. Fibrinolysis is pharmacologically inhibited by the capacity of TA to link to the binding sites in the molecule of plasmin and so to break its interaction with fibrin and other substrates. TA is well tolerated; its adverse reactions include nausea, vomiting, diarrhea, and allergy; orthostatic collapse is rarely seen. The literature has described the effective use of this agent to reduce blood loss, which finally decreases the need for transfusion and death rates in the patients.</italic></p></abstract><trans-abstract xml:lang="ru"><p><italic>Данный обзор литературы направлен на обобщение результатов применения транексамовой кислоты (ТК), ее пользы и безопасности в практической деятельности при проведении гистерэктомии. Проведены систематический поиск и анализ научных работ в PubMed, MEDLINE, Embase, eLIBRARY, КиберЛенинка и Google Scholar, используя поисковые термины «гистерэктомия», «транексамовая кислота» и «применение транексамовой кислоты при хирургических операциях». Мы анализировали 6 исследований, включающих 193 616 пациентов. К одной из часто проводимых операций в хирургической гинекологии в большинстве стран мира относится гистерэктомия. Для снижения кровопотери и предотвращения развития возможных осложнений используют различные механические и фармакологические способы. К таковым относится применение ТК, которая представляет собой синтетическое производное аминокислоты лизина и включена Всемирной организацией здравоохранения (ВОЗ) в перечень основных лекарственных средств. Данное вещество благодаря местному и системному гемостатическому эффекту используется при кровотечениях, в патогенезе которых играет роль повышение фибринолиза. Фармакологическое подавление фибринолиза осуществляется за счет способности ТК присоединяться к сайтам связывания в молекуле плазмина и тем самым нарушать взаимодействие с фибрином и другими субстратами. ТК хорошо переносится; среди побочных эффектов встречаются тошнота, рвота, диарея, аллергия, редко – ортостатический коллапс. В литературе описано эффективное применение данного средства для снижения кровопотери, что в итоге уменьшает потребность в трансфузии и смертность пациентов.</italic></p> <p><bold><italic>Заключение:</italic></bold><italic> Результаты проведенного нами анализа говорят о безопасности и эффективности применения ТК.
