Patients were categorized into three groups based on histologically verified diagnoses. Group 1 comprised 186 patients with chronic cervicitis, Group 2 included 341 patients with low-grade squamous intraepithelial lesions (LSIL), and Group 3 included 292 patients with high-grade squamous intraepithelial lesions (HSIL). HPV DNA testing was conducted by real-time PCR to determine the number of genomic equivalents of the virus. The HPV Kvant-21 reagent kit, designed for the detection, typing, and quantification of human papillomavirus DNA with low carcinogenic risk (HPV types 6, 11, 44) and high carcinogenic risk (HPV types 16, 18, 26, 31, 33, 35, 39, 45, 51, 52, 53, 56, 58, 59, 66, 68, 73, 82), was used for analysis.</p> <p><bold>Results: </bold>The study found that detection of a high viral load was associated with a statistically significantly higher risk of HSIL than LSIL, regardless of HPV genotype (OR=5.26; 95% CI 3.19–8.64). A similar trend was observed with moderate viral loads (OR=2.32; 95% CI 1.78–3.03). Furthermore, LSIL was significantly more common than HSIL when a low viral load was detected (OR=3.09; 95% CI 1.91–4.99). Additionally, a statistically significant positive relationship between the viral load and the degree of cervical involvement was identified for HPV genotypes 16, 18, 31, 39, and 44.</p> <p><bold>Conclusion: </bold>This study highlights the significance of HPV load as an important diagnostic marker. This suggests that the value of HPV load is currently underrated owing to challenges in interpreting the results of quantitative HPV DNA determination and the complexity of conducting research in this domain.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель: </bold>Изучение связи между количественным определением вирусной нагрузки вируса папилломы человека (ВПЧ) и степенью поражения шейки матки.</p> <p><bold>Материалы и методы: </bold>Проанализированы данные 819 ВПЧ-позитивных женщин в возрасте от 18 лет до 81 года. Пациентки были разделены на 3 группы в зависимости от гистологической верификации диагноза: 1-я группа – 186 пациенток с хроническим цервицитом; 2-я группа – 341 пациентка с LSIL; 3-я группа – 292 пациентки с HSIL. Тестирование на ДНК ВПЧ низкого (ВПЧ 6, 11, 44 типов) и высокого (ВПЧ 16, 18, 26, 31,33, 35, 39, 45, 51, 52, 53, 56, 58, 59, 66, 68, 73, 82 типов) канцерогенного риска выполнялось с определением количества геномных эквивалентов вируса методом ПЦР в режиме реального времени на наборе реагентов «HPV Квант-21».</p> <p><bold>Результаты: </bold>Было установлено, что при определении высокой вирусной нагрузки, вне зависимости от выявленного генотипа ВПЧ, риск обнаружения HSIL статистически значимо выше, по сравнению с LSIL (ОШ=5,26; 95% ДИ 3,19–8,64); при определении умеренной вирусной нагрузки наблюдается та же закономерность (ОШ=2,32; 95% ДИ 1,78–3,03). При этом при низкой вирусной нагрузке статистически значимо чаще будет встречаться LSIL по сравнению с HSIL (ОШ=3,09; 95% ДИ 1,91–4,99).
