The main group included 96 patients who received the local treatment as follows: fenticonazole 600 mg twice with an interval of 72 hours, then 600 mg once every 10 days for 3 months (subgroup 1, n=36); fenticonazole 600 mg twice with an interval of 72 hours once a month for 3 months (subgroup 2, n=30); fenticonazole 600 mg once every 10 days + 2 courses of platelet-rich plasma therapy (subgroup 3, n=30). The control group included 110 women who received induction therapy with fluconazole 150 mg orally on days 1, 4, 7, and then 150 mg once a week for 3 months. The patients of the groups matched for age, body mass index, somatic and gynecological diseases.</p> <p><bold>Results: </bold>There were no significant differences in the recurrence rate of candida vaginitis in patients who received systemic and topical therapy (p=0.66), but the recurrence rate was significantly higher in the control group during three-month follow-up (p=0.043). The effectiveness of therapy was 94.55% in the control group and there was no recurrence of the disease in 76.36% of patients over the next 3 months. The effectiveness of therapy was 92.71% in the main group and recurrence was not observed in 87.5% of patients.</p> <p><bold>Conclusion: </bold>Topical fenticonazole is effective in relieving and preventing recurrence of vulvovaginal candidiasis. The administration of an induction and prolonged course of fenticonazole may be an acceptable alternative to systemic therapy.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель: </bold>Изучение эффективности продленного курса местной терапии фентиконазолом для профилактики рецидива кандидозного вульвовагинита у пациенток с его хроническим рецидивирующим течением.</p> <p><bold>Материалы и методы: </bold>Проспективное обсервационное исследование включило 206 пациенток с рецидивирующим кандидозным вульвовагинитом, стратифицированных на 2 группы. В группах проводилось следующее лечение: в основной группе (n=96) местное лечение – фентиконазол 600 мг дважды с интервалом 72 ч, далее 600 мг 1 раз в 10 дней 3 месяца (1-я подгруппа, n=36); фентиконазол 600 мг 2 раза с интервалом 72 ч 1 раз в месяц 3 месяца (2-я подгруппа, n=30); фентиконазол 600 мг 1 раз в 10 дней + 2 курса PRP-терапии (3-я подгруппа, n=30). В группе сравнения (n=110) индукционная терапия проводилась флуконазолом 150 мг перорально в 1–4–7-й дни, далее по 150 мг 1 раз в неделю 3 месяца. Пациентки сформированных групп были сопоставимы по возрасту, индексу массы тела, соматическим и гинекологическим заболеваниям.</p> <p><bold>Результаты: </bold>Не было выявлено значимых различий в частоте рецидивов кандидозного вульвовагинита на фоне системной и топической терапии (р=0,66), но в течение трех месяцев дальнейшего наблюдения частота рецидивов была значимо выше в группе сравнения (р=0,043). В группе сравнения эффективность терапии составила 94,55%, в течение последующих 3 месяцев отсутствие рецидивов отмечалось у 76,36% пациенток. В основной группе эффективность терапии составила 92,71%, и у 87,5% пациенток в последующем не было рецидивов.</p> <p><bold>Заключение: </bold>Топическая терапия фентиконазолом эффективна в купировании рецидивов вульвовагинального кандидоза и в профилактике его рецидивов.
