Early detection of brain injury in IUGR enables to predict short-term and long-term outcomes for the development of the central nervous system, that currently remains a serious issue.</p> <p><bold>Objective:</bold> The aim of the study was to assess the role of ultrasound and MRI in diagnosis of FGR in combination with pathological changes in fetal brain.</p> <p><bold>Materials and methods:</bold> The retrospective study included 24 patients with suspected FGR. The mean age of patients (Me; Q1–Q3) was 33 (25–41) years, the average pregnancy length was 27.5 (20–35) weeks. The patients underwent simultaneous diagnostic US and MRI of the fetuses in the second and third trimester of pregnancy to assess fetal head circumference using percentile values of nomograms, and identify comorbidity, including the changes in fetal brain.</p> <p><bold>Results:</bold> No false positive results were found. MRI data and US imaging data were absolutely similar in nomograms for measurement of fetal brain volume using percentile method in 24 fetuses (100%) with FGR. Among them, FGR in combination with congenital diaphragmatic hernia was diagnosed in 3 fetuses (12.5%), and spina bifida in 1 fetus (4.2%). Comparison of two imaging techniques showed that false-negative results of ultrasound detection of malformation of the cortical plate and assessment of sulcation of the fetal brain were found in 3 fetuses (12.5%) versus 7 fetuses (29.2%) using MRI. Also, ultrasound imaging in diagnosing isolated unilateral cerebellar hypoplasia, showed false negative results in 2 fetuses (8.3%) versus false negative MRIs in 5 fetuses (20.9%).</p> <p><bold>Conclusion:</bold> The study showed that diagnostic ultrasound and MRI are comparable techniques in assessing biometry of the fetal brain using centile nomograms. However, MRI helps to perform more careful assessment of the concomitant pathology of the fetal brain.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность: </bold>Синдром задержки роста плода (СЗРП) является частым осложнением беременности и в тяжелых случаях приводит к повышению уровня перинатальной смертности, неонатальной заболеваемости и неблагоприятным прогнозам для жизни в отношении пациентов с пороками развития центральной нервной системы и ишемически-гипоксическими изменениями головного мозга. Раннее выявление повреждения головного мозга при СЗРП позволяет прогнозировать краткосрочные и долгосрочные последствия для развития центральной нервной системы, что остается на сегодняшний день серьезной проблемой.</p> <p><bold>Цель:</bold> Оценка роли методов УЗИ и МРТ при диагностике СЗРП в сочетании с патологическими изменениями головного мозга плода.</p> <p><bold>Материалы и методы:</bold> В ретроспективное исследование включены 24 пациентки с подозрением на СЗРП. Средний возраст пациенток (Me (Q1–Q3)) составил 33 (25–41) года. Средний срок гестации – 27,5 (20–35) недели. Пациенткам были одновременно проведены УЗИ и МРТ плода во II и III триместрах беременности с целью оценки размеров головы плода (с использованием центильных номограмм) и диагностики сочетанной патологии, в том числе изменений головного мозга плода.</p> <p><bold>Результаты: </bold>Ложноположительных результатов обнаружено не было. Данные МРТ и УЗИ полностью совпали при оценке размеров головного мозга плода и при диагностике СЗРП методом центильных номограмм у 24 плодов (100%), при диагностике СЗРП в сочетании с врожденной диафрагмальной грыжей – у 3/24 плодов (12,5%), СЗРП и spina bifida – у 1/24 (4,2%) плода. При сравнении методов при наличии порока формирования кортикальной пластинки и оценке степени сулькации ложноотрицательные результаты УЗИ были обнаружены у 3/24 (12,5%) плодов и МРТ – у 7/24 (29,2%) плодов. При изолированной гипоплазии полушарий/червя мозжечка у 2/24 (8,3%) плодов и 5/24 (20,9%) плодов установлены ложноотрицательные результаты УЗИ и МРТ соответственно.</p> <p><bold>Заключение:</bold> В результате исследования получено, что УЗИ и МРТ сопоставимы друг с другом при оценке размеров головного мозга плода методом центильных номограмм.
