The lipidome signature of follicular fluid was analyzed using high-performance liquid chromatography coupled with mass spectrometry (HPLC-MS). Orthogonal projections to latent structures discriminant analysis (OPLS-DA) were used to create a differential model.</p> <p><bold>Results:</bold> Comparative analysis of FF samples from women who achieved pregnancy versus those with negative result revealed differences in lipidome signature within each group that correlated with IVF outcomes. Based on these findings, a predictive OPLS model was developed. The model allows to differentiate pregnant and non-pregnant women based on the levels of lipids belonging to the following classes: cholesteryl esters, phosphatidylcholines, sphingomyelins and triglycerides.</p> <p><bold>Conclusion:</bold> Lipidome profiling of follicular fluid using HPLC-MS allows for fairly accurate differentiation between women who achieve pregnancy and those who do not. These results are consistent with international data and confirm the potential for using this method to predict IVF outcomes in patients with ovarian endometriomas.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность: </bold>Липидное профилирование фолликулярной жидкости (ФЖ) у пациенток с эндометриоидными кистами яичников (ЭКЯ) может предоставить ценную информацию о потенциале ооцита к оплодотворению, жизнеспособности эмбриона и, соответственно, исходе программы вспомогательных репродуктивных технологий (ВРТ).</p> <p><bold>Цель:</bold> Создание модели предикции наступления клинической беременности в программах ВРТ у пациенток с ЭКЯ на момент овариальной стимуляции на основании липидного профиля ФЖ.</p> <p><bold>Материалы и методы: </bold>Проведен анализ клинико-анамнестических данных, липидного профиля ФЖ в циклах овариальной стимуляции и исходов программ ВРТ у пациенток с наличием ЭКЯ (n=41). Анализ липидного профиля образцов ФЖ, полученных в день пункции яичников, был проведен методом высокоэффективной жидкостной хроматографии с масс-спектрометрическим детектированием (ВЭЖХ-МС). При использовании дискриминантного анализа ортогональных проекций на скрытые структуры (OPLS-DA) выполнялось построение дифференциальной OPLS-модели.</p> <p><bold>Результаты:</bold> Сравнительный анализ образцов ФЖ женщин, у которых наступила беременность, и женщин, у которых беременность не наступила, выявил различия липидного профиля внутри каждой из исследуемых групп, которые коррелировали с исходом цикла ВРТ, что легло в основу построения прогностической модели. Полученная OPLS-модель позволила дифференцировать пациенток с наступившей и не наступившей беременностью на основании липидов следующих классов: холестериновые эфиры, фосфатидилхолины, сфингомиелины, триглицериды.</p> <p><bold>Заключение:</bold> Анализ липидного профиля ФЖ с помощью ВЭЖХ-МС выявил значимые различия в липидном составе образцов ФЖ пациенток с наступившей и не наступившей беременностью. Полученные результаты свидетельствуют о том, что липиды играют важную роль в фолликулярном микроокружении, обеспечивая оптимальную среду для созревания ооцита.
