The patients underwent laser drilling of the uterus with a holmium laser using a laparoscopic approach.</p> <p><bold>Results:</bold> During the follow-up period after organ-preserving surgery, clinical symptoms and reproductive outcomes were assessed. Ultrasound and MRI were used to objectify the data. Six months after surgery most patients demonstrated a significant reduction in the severity of dysmenorrhea (from 8.10 to 2.0 according to the NRS scale). Menstrual blood loss also markedly decreased from 153.1 (80) ml to 67.0 ml. Pregnancy occurred in 127/337 patients under the follow-up. No intra- or postoperative complications were noted.</p> <p><bold>Conclusion:</bold> The obtained data demonstrate a significant improvement in clinical symptoms (decrease in the severity of dysmenorrhea, menstrual blood loss, and uterine size) in patients with adenomyosis. Reproductive outcomes also improved, which did not show statistical significance after Bonferroni correction, therefore requiring further confirmation with control groups.</p></abstract><trans-abstract xml:lang="ru"><p>Аденомиоз представляет собой медико-социальную проблему, связанную не только с ухудшением качества жизни женщин репродуктивного и перименопаузального возраста, но и является одной из причин бесплодия и невынашивания беременности у пациенток молодого возраста.</p> <p><bold>Цель: </bold>Оценить динамику клинических симптомов и репродуктивных исходов после лазерного дриллинга матки с помощью гольмиевого лазера у пациенток репродуктивного возраста.</p> <p><bold>Материалы и методы: </bold>В исследование включены 470 пациенток с бесплодием маточного происхождения, обусловленным наличием диффузного и/или узлового аденомиоза 2 и 2–3 степени распространения (MUSA 2022), пролеченных в период с 2000 по 2024 гг. Пациенткам был проведен лазерный дриллинг матки с помощью гольмиевого лазера лапароскопическим доступом.</p> <p><bold>Результаты: </bold>При динамическом наблюдении после органосберегающего вмешательства оценивались клинические симптомы, репродуктивные исходы. Для объективизации данных применялись ультразвуковые методы исследования, МРТ. Через 6 месяцев после хирургического вмешательства у большинства пациенток отмечалось значительное снижение выраженности дисменореи (по шкале NRS с 8,10 до 2,0). Объем менструальной кровопотери также значимо снизился с 153,1 (80) мл до 67,0 мл. Беременность наступила у 127/337 продолжавших наблюдение пациенток. Интра- и послеоперационных осложнений отмечено не было.</p> <p><bold>Заключение: </bold>Полученные данные демонстрируют значимое улучшение клинических параметров (уменьшение выраженности дисменореи, объема менструальной кровопотери, сокращение размеров матки) у пациенток с аденомиозом.
