Терапия эстрогенами: от науки к практическим решениям

Аннотация

В обзоре представлены последние данные о безопасности и эффективности гормональной терапии эстрогенами. Авторы разделяют понятия «заместительная гормональная терапия» и «менопаузальная гормональная терапия», акцентируя внимание на принципиальных различиях в лечебных подходах к женщинам с эстрогенодефицитом в репродуктивном периоде и в постменопаузе. В статье обсуждаются особенности подбора терапии у женщин с гинекологической патологией и некоторыми экстрагенитальными заболеваниями. Статья содержит информацию об основных принципах назначения менопаузальной гормональной терапии для минимизации связанных с ней онкорисков и рисков неблагоприятных сердечно-сосудистых событий, которые включают использование трансдермальных форм, в частности, 17β-эстрадиола в виде 0,1% геля, в минимально эффективных дозах, и натурального прогестерона, начало лечения в пределах «окна терапевтического воздействия» длительностью до пяти лет, оценку пользы/риска от вмешательства в динамике. В статье приведены некоторые клинические ситуации, при которыхрекомендуется избегать назначения пероральных форм эстрогенов вследствие особенностей фармакокинетики: гипертриглицеридемия, высокие базовые риски венозных тромбоэмболических осложнений и инсультов, заболевания желчного пузыря, прием противосудорожных препаратов и тироксина, употребление женщиной алкоголя, а также, вероятно, коронавирусная инфекция. Кроме того, в обзоре анализируются обновленные рекомендации по назначению заместительной гормональной терапии у женщин с преждевременной недостаточностью яичников, основываясь на данных международных клинических протоколов. Заключение. Гормональная терапия является в целом безопасным фармакологическим вмешательством, поскольку назначается либо женщинам в репродуктивном возрасте при снижении/утрате активности яичников, либо молодым относительно здоровым женщинам с менопаузальными расстройствами в возрастном диапазоне от 40 до 50лет.
