Оценка рецептивности эндометрия по уровню малых некодирующих РНК в маточном аспирате у женщин на фоне циклической гормональной терапии

Аннотация
Современные методы диагностики рецептивности эндометрия, введенные в клиническую практику (ERA, Igenomix; ER-Map, iGLS; ERPeak, CooperGenomics), основаны на транскриптомном анализе ткани эндометрия при инвазивном способе его получения, что отменяет перенос эмбриона в том же цикле, что и выполнение биопсии эндометрия.
Цель: Оценка рецептивности эндометрия путем количественного определения малых некодирующих (мнк)РНК в маточной жидкости (МЖ) в день переноса криоконсервированного эмбриона (КЭ) на фоне циклической гормональной терапии (ЦГТ) и построение модели логистической регрессии расчета вероятности готовности эндометрия к имплантации эмбриона путем сопоставления транскриптома МЖ от пациенток с положительным и отрицательным исходом программы ВРТ.
Материалы и методы: В исследование были включены 54 женщины, которым проводили аспирацию МЖ в объеме 5–50 мкл в зависимости от уровня ее секреции непосредственно перед переносом КЭ с использованием катетера (COOK, Австралия). мнкРНК, выделенные из МЖ набором miRNeasy Serum/Plasma Kit (Qiagen), были проанализированы методом глубокого секвенирования на платформе NextSeq 500/550 (Illumina, USA) c последующей валидацией полученных данных методом количественной ПЦР в реальном времени наборами miScript II RT Kit и miScript SYBR Green PCR Kit (Qiagen, Hilden, Germany).
Результаты: Образцы МЖ были классифицированы на 2 группы – «рецептивный эндометрий» и «нерецептивный эндометрий» – в зависимости от результатов проведенной программы ВРТ (наличие и отсутствие имплантации соответственно). По профилю мнкРНК в образцах МЖ и толщине эндометрия на момент переноса эмбриона в полость матки были построены семь моделей логистической регрессии, среди которых наиболее точной является комбинация содержания miR-1180-3p в МЖ и толщины эндометрия (71% чувствительность, 88% специфичность) ввиду отсутствия зависимости включенных в нее переменных по данным корреляционного анализа по Спирмену (r=0,02; p=0,9) и статистической значимости всех входящих в модель коэффициентов (р<0,05).
Заключение: Индивидуализация подхода в определении «окна имплантации» при проведении ЭКО на фоне ЦГТ в соответствии с уровнем мнкРНК в МЖ может играть важную роль в повышении эффективности программ ВРТ. Ограничением использования в клинической практике модели логистической регрессии, построенной в настоящем исследовании на обучающей выборке пациенток, является отсутствие информации об имплантационном потенциале переносимой в полость матки бластоцисты. Отрицательный исход программы ВРТ может быть обусловлен не отсутствием рецептивного эндометрия, а сниженным качеством самого эмбриона. Одновременное определение рецептивности эндометрия и имплантационного потенциала эмбриона по профилю мнкРНК приведет к снижению процента ложноотрицательных результатов и улучшению качества модели. Необходимо расширить обучающую выборку и проверить точность построенных моделей на независимой тестовой выборке.
