Application of vitamin D, Russian drug Osteo-Vit D3 in the complex prevention of osteoporosis and osteoporotic fractures in children and teenagers

Abstract

Objective of the study. To carry out the analysis of the literature data devoted to the problem of diagnostics, pathogenesis, clinic of osteoporosis and osteoporosis fractures in children and teenagers, complex prevention of osteoporosis and osteoporosis fractures with the use of calcium (Ca), vitamin D, Osteo-Vit D3. Material and methods. The authors have studied 20 foreign articles by authors from India, Australia, Canada, Spain, the USA, Great Britain, the Netherlands, New Zealand, China, Ireland and 20 Russian publications which regard the problem of prevention of osteoporosis and osteoporosis fractures, including in children and teenagers with application of Ca, vitamin D as well as the Russian preparation Osteo-Vit D3. Results. The authors observed that 50% of children under 18 years of age suffered fractures. The study showed that deformities and fractures of the vertebral bodies were a specific feature of the course of osteoporosis in children associated with hormone therapy. They found that 38% of children had vitamin D deficiency. Found associations of low vitamin D levels and recurrent osteoporotic fractures. Revealed that vitamin D is essential for normal body Ca homeostasis and bone metabolism. Reflected data on the importance of Ca and vitamin D preparations in increasing bone mineral density and preventing osteoporosis. Found that vitamin D supplementation was associated with a decreased risk of fracture. Recommended supplementation with Ca and vitamin D to prevent osteoporotic fractures. We studied the effectiveness of the Russian drug Osteo-Vit D3 in comparison with foreign analogues. It was obtained that prescription of Osteo-Vit D3 was associated with an increase in bone mineral density and a reduced risk of fractures. By its properties Osteo-Vit D3 was not inferior to foreign Calcemin preparations. Conclusions. Thus, Osteo vit D3 is a promising preparation for increase of mineral density of bone tissue, strength of bones, prevention of osteoporosis and osteoporosis fractures in children and teenagers. In addition, Osteo-Vit D3 was developed by Russian scientists and its properties are not inferior to the foreign drug Calcemin.

Determination of the Pectorales species No. 2 and its components identity by infrared spectroscopy of attenuated total reflection

Abstract

Introduction. The method of IR spectroscopy is applied in assessing the quality of pharmaceutical substances, drugs of synthetic and natural origin, herbal medicines. Attenuated total reflection IR spectroscopy (ATR IR) is the most modern version of IR spectroscopy. At present, the practice of using IR spectroscopy has not found wide implementation in the assessment of the identity of herbal medicines, despite all the advantages of the method. Pectorales species No.2 is a expectorant multicomponent medicinal herbal preparation. The components of the Pectorales species No.2 are the coltsfoot, plantain leaves, and licorice roots. Despite the fact that these herbal medicines are pharmacopoeial and have a rich medical practice, data on their IR-spectra are not presented in the literature. Objective: to determine the characteristics of the identity of Pectorales species No.2 and its components by the method of attenuated total reflection IR spectroscopy. Material and methods: Dry crushed samples of Pectorales species No.2 and its components were studied on a Tensor 37 Fourier spectrometer (Bruker, Germany). Results. Vibrations characteristic of OH-groups, intermolecular hydrogen bonds of polymers, and NH-groups were found in the range of 3700-3020 cm-1. Vibrations of CHn-groups were revealed in the ranges of 3000-2770 cm-1, 1460-1300 cm-1, 900 cm-1. Vibrations of C=O groups of peptides were detected at 1650 cm-1. Vibrations of NH-groups and CN-groups of peptides, ester group of lipids, OH-groups of polysaccharides, O=C-N groups of peptides were found in the area of “fingerprints”. Conclusion. These data may indicate the presence of phenolic compounds, polysaccharides, lipids, and peptides in the samples. Thus, the characteristics of the identity of Pectorales species No. 2, coltsfoot leaves, plantain leaves, and licorice roots were determined by the method of IR spectroscopy.

Modern conservation methods of medicinal plant raw materials: variability of the content and stability of biologically active substances

Abstract

Introduction. To preserve the maximum content of biologically active substances (BAS) in medicinal plant raw materials during storage, it is necessary to subject it to conservation. Modern methods of preservation are drying and freezing. However, in both cases, the content of BAS will vary due to various factors. Thus, it is necessary to research the choice of a priority conservation method to preserve the highest content of BAS in medicinal plant raw materials. Objective: to study the effect of freezing and drying of medicinal plant raw materials on the content of BAS. Material and methods. Fresh lingonberry fruits (Vaccinium vitis-idaea L.), collected on the territory of the Tver region during the fruiting period in 2020 and subjected to conservation. Determination of BAS content (ascorbic acid, organic acids, tannins, flavonoids, anthocyanins) in fruits was carried out according to the methods set out in pharmacopoeia monographs. Results. In the course of a comparative study of BAS, it was found that in frozen fruits the content of ascorbic acid, organic acids, flavonoids decreased by 7, 8 and 26%, respectively, and the content of tannins and anthocyanins even increased slightly. The drying conditions (natural and artificial drying) did not affect the content of BAS. A significant decrease in the content of ascorbic acid was observed in the dried fruits. During the storage of frozen fruits for 6 months, there was a tendency to decrease ascorbic acid and anthocyanins, the number of organic acids practically did not change. Data on the storage of dried fruits indicate the stabilization of the content of BAS after 3 months of storage. Conclusion. It is shown that freezing, as a conservation method, is the most preferable for preserving the content of BAS in the medicinal plant raw materials. The stability of BAS in frozen and dried fruits during storage has been studied. The recommended shelf life of frozen fruits is 6 months and of dried fruits is 12 months.

Composition of biologically active compounds of three-part beggarticks herb

Abstract

Introduction. Three-part beggarticks herb is a pharmacopoeial medicinal plant raw material that has found wide application in clinical practice as an external remedy for skin manifestations of exudative diathesis, pyoderma and as an internal remedy for acute respiratory viral infections. In Russian Federation, anti-inflammatory medicinal herbal preparations based on it (herb crushed for the preparation of infusion, herb powder in filter bags) are produced and sold. Despite the high degree of scientific knowledge of this medicinal raw material, the improvement of approaches to standardization remains an urgent issue. Objective: identification of the biologically active compounds composition of the industrially produced three-part beggarticks herb. Material and methods. Biologically active compounds were analyzed using ultra-high-performance liquid chromatography with photodiode array and tandem quadrupole mass-selective detection in the gradient elution mode. Extraction was carried out with 70% aqueous methanol in an ultrasonic bath. Results. The presence of the main groups of biologically active compounds characteristic for three-part beggarticks herb was confirmed. 18 compounds were identified, including sulfuretin, okaniin, luteolin, glycosides, hydroxycinnamic acids, polyacetylenes. Conclusion. The proper quality of medicinal herbal raw materials and the drugs based on them can only be guaranteed if the composition of marker compounds is reliably determined, which makes it possible to confirm its identity. In this regard, a detailed analysis of the phytochemical composition, performed in this study, is a prerequisite for scientifically sound improvement of regulatory documentation. Polyacetylenes are a rather rare and specific group of biologically active compounds in medicinal plant raw materials. In order to standardize the three-part beggarticks herb, it is also recommended to determine them along with other compounds of the polyphenolic complex (flavonoids, hydroxycinnamic acids) and polysaccharides.

Quantitative determination of the mangiferine in Hedysarum caucasicum M.Bieb.by HPLC method

Abstract

Introduction. The species of the genus Hedysarum L., Fabaceae family are of considerable interest from a pharmacognostic point of view. In these species, one of the main biologically active substances is xanthones, xanthone glycoside mangiferin, having pronounced antibacterial and antiviral activity, with respect to DNA-containing viruses. Due to these pharmacological properties, the search for additional raw sources of mangiferin is promising. Objective: development of a methodology for quantitative determination of mangiferin in the aerial part of Hedysarum caucasicum M. Bieb. by HPLC. Material and methods. The objects of the study were samples of the aerial part of Hedysarum caucasicum M. Bieb. collected on the territory of Kabardino-Balkary and Karachai-Cherkessya republics. Electronic spectra were recorded on Shimadzu Prominence LC-20AD liquid chromatograph, with a DGU-20A3R degasser. Mangiferin Sigma-Aldrich was used as a standard sample. The mobile phase was a mixture of buffer solution, acetonitrile and methanol in a ratio of 81:16:3. Results. The technique takes into account the basic physical and chemical properties of xanthones, is characterized by reproducibility, high accuracy, simplicity of execution, allows to carry out both screening evaluation of various raw objects containing mangiferin, as well as standardization of prepared medicinal plant raw materials. As a result, 0.13-0.15% of mangiferin was found in the aerial organs of Hedysarum caucasicum. Conclusion. The results show that there is sufficient mangiferin in the aerian part of the Hedysarum caucasicum, which makes it possible to recommend mangiferin as an additional raw material source as an antiviral agent of domestic origin.

Evaluation of the quality and biological activity of the «BioChaga» birch fungus extract

Abstract

Introduction. «BioChaga» is a natural extract of the birch mushroom chaga, produced according to the author's technology of the Irkutsk company SibPribor LLC under the trademark «Baikal Legend ». The high quality of «BioChaga» is due to the combination of environmentally friendly chaga medicinal plant raw materials rich in biologically active substances of the Baikal region and th e unique author's technology of its processing. The result is a fast-soluble «BioChaga» powder with low density, high biological activity and bioavailability. The purpose of the study: a comprehensive study of the composition and content of BAS, biological activity of the «BioChaga » extract in connection with the peculiarities of its production technology. Material and methods. To determine the physicochemical properties of «BioChaga» extract we used the following methods: extraction / gravimetry, spectrophotometry, the qualitative composition was studied using high-performance liquid chromatography. To determine the biological activity of «BioChaga », the antiradical activity of the extract to organic and inorganic radicals of natural and artificial origin was determined by spectrometry and coulometry. The antioxidant potential was evaluated using spectrophotometry and fluorimetry to determine the total antioxidant potential and the ability to absorb oxygen radicals. The iron-reducing and iron- chelating potential of «BioChaga» was determined by spectrophotometry. Results. The physico-chemical characteristics of the «BioChaga» composition had a high level of compounds characteristic of the chaga: low molecular weight phenolic compounds - 57,00±1,48 mg/kg, high molecular weight phenolic compounds (melanin complex) - 24,9±1,2%. The nature of the distribution of fragments of the melanin complex shows a predominant content of o-dioxybenzoyl fragments, which suggests the presence of high biological activity in the «BioChaga». It has been shown that the «BioChaga» has antioxidant, antiradical and iron-reducing activity. In addition, the extract has proven to be a good inactivator of nitric oxide molecules, which indicates its anti-inflammatory properties. Conclusion. «BioChaga» is a highly active natural complex agent characterized as an effective antioxidant created using a unique author's technology. Its high biological activity indicates the effectiveness of the extract and allows it to be recommended as a means of prevention and protection against oxidative stress

Model of a risk-based approach in the implementation of pharmaceutical activities

Abstract

Introduction. This paper analyzes the general situation of drug supply and builds a mathematical model for improving the quality of pharmaceutical services based on an analysis of licensing requirements and conditions using the Sverdlovsk region as an example. Objective: to identify risk-based points in the implementation of pharmaceutical activities and build a mathematical model of the impact of the quality of education of pharmaceutical specialists on the pharmaceutical services provided. Material and methods. Data from the Unified Register of Licenses of Roszdravnadzor. The analysis was carried out by the method of content analysis. Results. It is shown that in the pharmaceutical market there are certain features, expressed in a legal nature, associated with licensing and the risk-based approach used in organizing inspections. The paper describes situations related to the stages and issues of licensing, and schemes with risk points for obtaining a license for pharmaceutical activities, the possibility of self-assessment of compliance with licensing requirements. The organizational and legal forms and their attractiveness for entering the market of the Sverdlovsk region are described. It is noted that conditionally these requirements can be divided into four stages. The first requirement relates to the assessment of the premises, the organization of their functioning, compliance with sanitary and hygienic requirements. The second requirement relates to the formation of a system for accounting and preparation of equipment for the placement of medicines. The third scheme describes the training of specialists involved in pharmaceutical activities, and the fourth one deals with the validation of the main business processes. A mathematical model has been developed that links consumers, the demand for medicines, and the professional qualifications of a specialist with a pharmaceutical education. It is concluded that licensing requirements set a barrier that improves the quality of drug provision and increases the social well-being of the population. Conclusion. Currently, it is necessary to improve the quality of drug provision by improving the quality of education of pharmaceutical workers through the accreditation procedure.

Evaluation of pharmacotherapy in pediatric patients with acute intestinal infections in a hospital setting

Abstract

Introduction. Acute intestinal infections (AII) remain the most common diseases in pediatric patients. The analysis of scientific literature has shown that pharmacotherapy of these diseases in pediatric patients requires an integrated approach and the appointment of drugs of different groups. Therefore, it is important to study the rationality of drug therapy for pediatric patients with AII in a hospital setting. Objective: evaluation of the pharmacotherapy rationality for pediatric patients with AII and development of optimal procurement lists. Material and methods: official sources of information about medicines, orders of the Ministry of Health of the Russian Federation, statistical collections, clinical records of pediatric patients with AII (400). Methods: logical, systematic, grouping, statistical, clinical and economic methods. Results. The results of the analysis of the therapy of pediatric patients with AII are presented. In particular, the range of drugs prescribed in inpatient conditions is determined. Combining the results of ABC-, XYZ- and VEN-methods of assortment analysis, as well as comparing the results of therapy with approved drug lists, allowed to select of the most important drugs for the procurement lists formation. Conclusion. Procurement lists have been developed, including 28 international generic names assigned to various priority groups. The choice of priority positions in the range of purchases makes it possible to acquire the most necessary and important drugs, depending on the volume of financing, and to predict stocks in risk conditions.
