Rol' uchastkovogo pediatra v formirovanii zdorov'ya detey rannego vozrasta

Abstract

The paper presents the results of studying the sociomedical characteristics of children of the first three years of life, analyzes their parents' medical activity, and identifies risk factors for the development of poor health indicators in the children. Particular emphasis is laid on the results of teaching the parents by pediatricians in the methods of interactive improvement of their infants' health. The impact of medical literacy of the parents who bring up their infants on positive changes in their health is shown. The findings suggest that only the active participation of family members, which includes timely complete compliance, can improve health indicators in children.

Abstract

The paper describes an organizational model of a day hospital (DH) that operates as a structural subdivision on the basis of a therapeutic-and-prophylactic institution. The authors present the structure of DH and its provision with medical staff. The paper analyzes these issues by age, gender, and reasons for patients' hospitalization to DH, provides a comparative analysis of medium-term stay at DH and round-the-clock hospital, and gives the results of treatment. Particular emphasis is placed on a physician's work quota setting and a load on a DH physician has been calculated.

Abstract

The influence of a number of factors and their combination on the development of recurrent atrial fibrillation (AF) was studied in patients with arterial hypertension (AH). A combination of female sex, over 60 years of age, grade 3 AH, prior AF, current coronary heart disease (CHD) and type 1 diabetes mellitus promotes recurrent AF. A combination of AH and left ventricular diastolic dysfunction with CHD or that of AH with prior AF is prognostically less favorable in recurrent AF than the influence of each of these factors singly.

Abstract

To diagnose community-acquired pneumonia, in parallel with routine diagnostic criteria the investigation used high-resolution computed tomography (HRCT) of the lungs, which allows fine pulmonary structures to be analyzed approximately to the data of a postmortem study. It has been shown that HRCT can reveal more extensive and long preserved lung tissue changes than can standard X-ray study.

Gnoyno-septicheskie oslozhneniya u lits, stradayushchikh parenteral'noy narkomaniey

Abstract

The paper analyzes the results of treatment in 297 patients with pyoseptic diseases of soft tissues and drug abuse. There has been recently an increase in the number of admissions for postinjection complications from 0.8 to 9.3% of the treated patients. Thirty-nine (13.1%) patients were observed to have severe anaerobic clostridial and non-clostridial infection with extensive soft tissue involvement as necrotic fasciocellulitis and myositis. Eighty-one (27.3%) patients developed sepsis caused mainly by aurococci and anaerobes. In addition to purulent changes, there were postinjection complications associated with vein or arterial vascular bed lesion in 61 (20.5%) patients. The paper also shows the specific features of ultrasound and microbiological diagnosis in this category of patients.

Abstract

Much severer gestosis in Yemen versus Russian women engaged attention. In Russian and Yemen women, severe gestosis is notified in 4% and 46.7% of cases, respectively. Risk factors for gestosis, accepted in our literature, such as primilabor, incomplete family, study at a higher educational establishment, intellectual labor, and bad habits, are likely to be urgent only when the population has a relatively highly medium level of living conditions.

Endoskopicheskaya i morfologicheskaya kharakteristika sostoyaniya zheludka pri sklerodermii u detey

Abstract

A gastric endoscopic study performed in 260 children with scleroderma revealed chronic gastritis in 245 (94.2%) cases. A histological examination of gastric mucosal biopsy specimens verified the endoscopic diagnosis of antral gastritis associated with Helicobacter pylori in 81.3% of cases. Vivid hyperemia and diffuse mucosal edema are characteristic signs in 88 (35.9%) patients with scleroderma. Of them 33 (37.5%) patients were found to have gastric mucosal erosions and 10 children had erosions and submucosal hemorrhage. In 147 (60%) patients with scleroderma, the stomach displayed abundant congestion contents as thick dark bile and turbid mucus, in individual cases with debris of the food taken before. Delay in emptying the stomach contents was substantially more frequently observed in patients with scleroderma than in those with gastroduodenitis from a control group. Delayed evacuation of stomach contents and multiple antral mucosal hemorrhagic erosions are likely to be a nonrandom phenomenon associated with systemic sclerosis.

Vliyanie povtornykh respiratornykh zabolevaniy na biokhimicheskie pokazateli krovi u detey

Abstract

The impact of frequent respiratory diseases on the peripheral blood levels of lipids and protein fractions was studied in fifty 4-to-7-year-old children going to organized pediatric collective bodies. Frequently ill preschool children were ascertained to have significantly elevated levels of α2-globulin and γ-globulin fractions and accordingly a decreased albumin/globulin ratio, which suggests the possible subacute course of the chronic inflammatory process in these children in the absence of clinical manifestations or may be associated with a slower recovery of biochemical parameters during late convalescence after prior infection. There was a significant increase in the level of total cholesterol at the expense of all existing fractions, but predominantly at the expense of low and very low lipoproteins that are endogenous immunomodulators.

Lokal'naya gipotermiya v kompleksnom lechenii ostrogo destruktivnogo pankreatita

Abstract

The authors have developed a fundamentally new design of a pancreatic hypothermia device. In 2007-2008, a drain was inserted for local hypothermia in 32 patients with pancreonecrosis. The results of blood biochemical tests and pancreatic ultrasound studies are analyzed in these patients. These results are compared with data on patients undergoing laparoscopy or laparotomy without setting a drain for local hypothermia. It is concluded that local hypothermia is a rather effective technique and may be used in multimodality treatment for acute destructive pancreatitis.
