ASSESSING THE QUALITY OF TIMELY DETECTION OF PATIENTS WITH RESPIRATORY TUBERCULOSIS FROM THE CRITERIA OF EFFICIENCY IN THE CITY OF MOSCOW

Abstract

The time course of changes in the major parameters characterizing the efficiency of timely detection of patients with respiratory tuberculosis in general health care facilities (HCF) of Moscow and in 16 specialized tuberculosis dispensaries was analyzed over 12 years (1997-2008). Radiation (X-Ray fluorography) is a main method for detecting tuberculosis among adults. The role of timely X-ray fluorographic examinations is shown. The criteria proposed by the authors to assess the operations of photofluorographic units reflect the real state of the timely detection of tuberculosis by goal-oriented X-ray fluorography and should be used in the work of general and specialized HCFs.

ORGANIC AND FUNCTIONAL DIASTOLIC DYSFUNCTION OF THE LEFT VENTRICLE IN PATIENTS WITH ARTERIAL HYPERTENSION DURING DIFFERENT TYPES OF NEGATIVE STRESS MAINTENANCE AND TYPES OF CARDIAC REMODELING

Abstract

Left ventricular diastolic dysfunction was studied in patients with arterial hypertension, by using a cold functional test. The study was conducted in terms of the autonomic nervous system, by applying variation intervalometry. Gender differences in left ventricular geometry were found in patients with arterial hypertension. Organic diastolic dysfunction of the left ventricle was also shown to be significantly more frequently observed in male hypertensive patients with a sympathetic autonomic response to the cold test as compared to those who had developed a parasympathetic response to cold stress. When the sympathetic nervous system was activated with a cold test, organic diastolic dysfunction of the left ventricle was more common in patients with arterial hypertension in men than in women.

THE FUNCTIONAL STATE OF CEREBRAL HEMISPHERES IN RELATION TO THE LATERALIZATION OF AN EPILEPTIC FOCUS AND THE SEVERITY OF AN EPILEPTIC PROCESS

Abstract

The authors studied the steady-state and dynamic manifestations of the functional state of the brain, the characteristic features of its topography and changes in the propagation of epileptic activity in patients with epilepsy in its progression, estimated interhemispheric asymmetric parameters in the primary and secondary generalization of epileptic activity in patients with left and right hemispheric foci, and found correlations of the parameters of organization of bioelectrical processes reflecting the functional state of the brain with the specific features of the cognitive and emotional personality sphere in different patient groups as compared to healthy individuals. One hundred and ten epileptic patients with varying epileptic focus lateralization and 20 healthy individuals were examined; the role of the spatial synchronicity of the cortical biopotentials reflecting the enhanced functional activity of cortical regions in the left hemisphere, which allows it to be regarded as leading in the genesis of generalized seizures in right-handed people. As the epileptic process progressed the level of cognitive functions significantly decreased, this decreasing level being higher in the patients with a left hemispheric focus than in those with a right hemispheric one. The determination of the severity of cognitive disorders makes it possible to predict the degree of impaired adaptation and to follow up changes in antiepileptic treatment.

FACTORS INFLUENCING THE SOCIAL ADAPTATION OF CAREGIVERS OF PATIENTS WITH THE ADVANCED AND END STAGES OF PARKINSON’S DISEASE

Abstract

The status of caregivers of patients with the end and advanced stages of Parkinson’s disease (PD) has been poorly studied. The aim of the study was to investigate the factors influencing the social adaptation of caregivers. The scale of social adaptation (Zh. M. Glozman, 1998) was used to estimate its level. The Unified PD Rating Scale, Schwab and England Activities of Daily Living scale, Barthel index, Autonomic Impairment Scale, Addenbrooke’s Cognitive Examination, Cornell Scale for Depression in Dementia, and Neuropsychiatric Inventory were used for the clinical evaluation of the patients’ state. The main determinants of the social adaptation of caregivers were professional, family-domestic, and leisure limitations and emotional changes, which were determined by the severity of symptoms of parkinsonism, particularly axial motor impairments and psychotic, autonomic, and cognitive disorders in patients with PD.

The paper describes a procedure for using vacuum therapy in traumatology to treat type IIIb open fractures of long bones according to the Gustilo-Andersen classification. Vacuum drainage surpasses current bandaging materials in its properties, which allows this procedure to be used at all stages of the wound process even in the fixation of fragments in external fixation apparatuses.

ORGAN-SAVING OPERATIONS FOR OBSTETRIC PERITONITIS

Abstract

The paper analyzes the efficiency of organ-saving operations, by using as an example 12 puerperas with separated uterine sutures after cesarean section in the presence of obstetric peritonitis. Metroplasty was proposed to use in patients with these postpartum complications as an alternative to the classical obstetric procedure uterine extirpation. The paper also describes a clinical case of disseminated seropurulent peritonitis in a puerpera, which has developed in the presence of uterine suture failure after cesarean section. This patient underwent reconstructive surgery on the uterus with sanitation and drainage of abdominal cavity.

CURRENT CAPACITIES OF FREE TISSUE AUTOTRANSPLANTATION IN THE RECONSTRUCTIVE SURGERY OF THE EYELIDS AND PERIORBITAL REGION

Abstract

The authors analyzed the results of surgical treatment and revealed its most effective procedures in 120 patients with congenital and acquired deformities and defects of the eyelids and periorbital region, in whom free tissue autotransplantation was used. In surface defects of the lower eyelid with damage to its support structures, it is expedient to combine free skin flap autotransplantation with eyelid fixation using an autochondral graft taken from the auricle. A free deepidermized skin-fat graft and adipose tissue autotransplantation (lipofilling) are most effective to correct contour deformities of the periorbital region if autotransplantation techniques are strictly observed. Autotransplantation of hair follicles taken from the eyebrow is an optimal way for restoring the ciliary edges of eyelids. A differential approach to using the effective methods for transplantation of the skin, fat, cartilage, hair follicles, or a deepidermized skin-fat graft could considerably extend the capacities of reconstructive blepharoplasty, enhance the efficiency of treatment, and achieve early medical and social rehabilitation of patients with congenital and acquired deformities and defects of the eyelids and periorbital region.

PSYCHOLOGICAL CHARACTERISTICS OF WOMEN WITH COLORECTAL CANCER

Abstract

The paper describes the psychological features and leading personality patterns of women with colorectal cancer. Hypochondria has been found to be the most common personality trait (70.2%) in these patients. Varying degrees of depressive disorders are stated in 15.5%. The other patients have developed protective responses as hypomanic trends (a disease denial reaction) and hysteroid and neurosis-like reactions. The study demonstrates that it is necessary to elaborate a complex rehabilitation program to correct the psychological state of the patients at all treatment stages.

TO OPTIMIZATION OF THE TREATMENT OF CHRONIC URETHRITIS CAUSED BY SEXUALLY TRANSMITTED INFECTIONS

Abstract

The epidemiological features of individual pathogens of sexually transmitted infections, which cause urogenital tract inflammatory diseases, their susceptibility to antibacterial drugs, and β-lactamase activity were studied. Gonorrhea was ascertained to run as an associated infection in 68% of cases with the predominance of microbial associations with Staphylococcus aureus (52%) in the epidemiological structure. There was a substantial rise in the proportion of Neisseria gonorrheae and S. aureus strains producing β-lactamase (95.8 and 93%, respectively). There was a high antibiotic resistance of S. aureus and Escherichia coli strains in association with gonococcus, an increase in the resistance of Ureaplasma urealiticum, Mycoplasma hominis, and M genitalium strains isolated in mixed infections (by 1.25-5.8 times), and a decrease in gonococcal antibiotic susceptibility (by 4 times) in mixed infections. The findings substantiate it necessary to examine for concurrent microflora simultaneously with the identification of a pathogen and the regular monitoring of antibiotic susceptibility in all the isolated pathogens of mono and mixed infections.

EFFICACY OF PSYLLIUM DIETARY FIBERS IN PATIENTS WITH METABOLIC SYNDROME

Abstract

Metabolic syndrome is characterized by increased visceral fat mass, decreased peripheral tissue insulin sensitivity, and hyperinsulinemia, which induce carbohydrate, lipid, and purine metabolic disturbances, an activated systemic inflammatory response, and prothrombogenic shifts in the hemostatic system. Patients with metabolic syndrome have a combination of cardiovascular risk factors, such as arterial hypertension, overweight, and dyslipidemia, which causes us to regard metabolic syndrome as a very high cardiovascular death risk state. The purpose of this study is to make a complex evaluation of the efficiency of long-term use of psyllium (Mucofalk, Dr. Falk Pharma, Germany) in patients with metabolic syndrome during combination therapy. Sixty patients with metabolic syndrome were examined. Thirty patients received standard therapy and 30 patients took psyllium. Addition of psyllium to therapy could bring about an additional weight loss. Blood glucose levels were found to be reduced. During psyllium therapy, there was a decrease in the level of blood fibrinogen and C-peptide. Incorporation of psyllium dietary fibers into combination therapy could provide an additional fall in plasma lipid level in the treatment of patients with metabolic syndrome. At the same time psyllium showed a positive effect in losing weight, regulating eating behavior, relieving the symptoms of intestinal dyspepsia, and recovering the rate of defecation and the levels of fasting glycemia and lipid profile in patients with metabolic syndrome just at 3 months of treatment. The drug exhibits good tolerability and long-term safety.
