THE LIFE-STYLE AND MORBIDITY OF MALE ADOLESCENTS OF MILITARY AGE

Abstract

The article deals with data concerning health and social hygienic characteristics of 650 male adolescents of pre-military age. Assessment of health of male adolescents included the results of analysis of morbidity according medical documentation. The social hygienic characteristics of life-style were evaluated according the results of sociological survey of adolescents and their parents with specially designed questionnaire. In our country, during 2000-2010, the number of adolescents decreased from 7 362 thousand to 6 797 thousand (8.3%). The youth males not only make up the national population but directly influence health of posterior generations. On the whole, about 35% of conscripts are getting free of or get the deferment of military service because of health conditions. It should be considered that among persons admitted to military service about 30% have problems with health. This situation demands to apply the comprehensive rehabilitation using well-directed health-improving technologies. The presented data testifies the need to detect the risk factors to be eliminated to promote the improvement of health indicators not only of adolescents but adult population too. Hence, the further enhancement of national social infrastructure and prevention of health issues in mature age.

Abstract

Nowadays, the issues concerning chlamydia infection is not limited only with diseases of urogenital system. At that, the optimal organization and forms of fight against extra urogenital chlamidiosis, infection of respiratory tract included, are still to be developed and this task demands an immediate solution. The article presents scientific substantiation of necessity to develop the complex of activities and main directions to be applied to enhance the organization of medical care to patients with diseases of upper part of respiratory tracts associated with chlamydia infection.

THE INHALATIONS WITH HEATING OXYGEN-HELIUM MIXTURE IN COMPLEX TREATMENT OF PATIENTS WITH COMMUNITY-ACQUIRED PNEUMONIA

Abstract

The article presents the results of study assessing the effectiveness of course of inhalations with heating oxygen-helium mixture (heliox) in complex treatment of patients with community-acquired pneumonia. The heliox supports the restoration of damaged bronchial patency, enhances blood circulation, ventilation and pulmonary gas exchange in lungs. Hence, the preparation can impact on the important components of pathogenesis of pneumonia. The main group (standard therapy combined with course of inhalations with heating heliox) characterized with more promptly disappearing clinical symptoms of pneumonia, more complete roentgenologic resolution and earlier normalization of intensity of chemiluminescence of whole blood. All these processes reflect the rapid reduction of inflammatory process. Therefore, the inclusion of inhalations with heating heliox into complex therapy enhances the effectiveness of treatment of patients with community-acquired pneumonia.

THE CHARACTERISTICS OF FUNCTIONAL CONDITIONS OF RESPIRATORY MUSCULATURE ACCORDING THE DATA OF ELECTRONEUROMIOGRAPHY IN PATIENTS WITH OBESITY

Abstract

The reliable dependence of the degree of alterations of parameters of M-response of diaphragm from degree of obesity and age is established. The amplitude and area of M-response of diaphragm progressively decreased following age increase. At the same time, the duration of M-response increased. The decrease of amplitude of intercostal muscles was detected. Most likely, these alterations testify the alteration of innervation at the level of diaphragm itself and intercostal muscles possibly due to atrophy of neurons and/or decrease of mediator release into neuromuscular synapse.

THE DUODENOGASTRIC REFLUX IN CLINICAL PRACTICE

Abstract

The article presents the data of planned examination of 1660 patients with different dyspepsia manifestations in hospital and polyclinic conditions. The duodenogastric reflux of different degrees of manifestation was verified endoscopically in 39.3% of cases. In patients with duodenogastric reflux the clinical manifestations and diagnostic possibilities of ultrasound examination in gray scale and real-time modes were analyzed. The comparison was made concerning the frequency of occurrence of morphologic alterations of mucous membrane of esophagus and stomach in patients against the background of duodenogastric reflux and without it assessing possible variations of secretory activity of stomach. The date is presented concerning the significance of duodenogastric reflux in patients with esophagitis, gastritis and ulcer of stomach.

THE APPLICATION OF PLASMA SYSTEM «PLASMAJET» UNDER IMPLEMENTATION OF HIGH AMPUTATIONS OF LOWER EXTREMITIES

Abstract

The article analyzes the results of treatment of 142 patients with diseases of obliteration of arteries of lower extremities having amputation at the level of thigh or higher third of shank because of growing humid gangrene during time period of December 2010-April 2012. The main group consisted of 75 patients. The low-energy argon-plasma flow in different modes was applied during interventions. The apparatus «PlasmaJet» was used as a source of plasma energy. In all observations of implementations of main surgical techniques (cutting across of muscle bodies and fasces, coagulation of minute vessels, d-bridement of ganglions) the argon-plasma flow in coagulation mode was used. Prior to formation of stump, the operational wound was additionally d-bridemented with argon-plasma flow in coagulation mode. The exposition of stimulating argon-plasma impact was not higher than 5-10 seconds per 2 square centimeters. The comparison with the control group of 67 patients with diseases of obliteration of arteries of lower extremities (classical amputations) demonstrated a reliable decrease of volume of intraoperational blood loss and rate of wound complications up to1.5 times. The faster reduction of fever and post-operation pain syndrome and improvement of number of clinical indicators, including duration ща patients stay in hospital were established too.

THE RECONSTRUCTIVE SURGICAL COMPLEX IN TREATMENT OF HALLUX VALGUS

Abstract

The article deals with clinical evaluation of effectiveness of unified technique of surgical correction of deformation of the first radius of foot - reconstructive surgical complex. During January 2010-January 2012, the surgical correction of valgus deformation of first toes was implemented in 86 patients (162 feet). The patients were aged from 18 to 70 years, mean age consisted 48 years. The female patients consisted 98% (n=85) and male patients - 2% (n=1). The mean duration of observation consisted 18 months (from 6 to 30 months). The evaluation of results and comparative analysis of treatment was made using the AOFAS score scale before operation and after 12 and 24 months after operation. In all cases, the operative intervention was applied because of halux valgus III. The reconstructive surgical complex included angular osteotomy and cuneiform resection in the basement of metatarsal bone I, intra-articular release and transosseous capsuloraffy of metatarsophalangeal joint I, cuneiform osteotomy of proximal phalanx of toe I according to Akin. The mean pre-operational angle consisted 47 degrees, M1M2 — 19 degrees, SL — 85%. At the moment of study mean value of HV consisted 15 degrees, M1M2 — 7 degrees, SL — 20%. The mean value according AOFAS scale consisted 86 scores before and 86 scores after the operation. In 8 weeks after the operation about 86% of patients were socially active, used the public transport and turned up for work. The reconstructive surgical complex is an effective technique of treatment of halux valgus III. The major advantage of the complex is in positive correction of varus deformation of metatarsal bone I and valgus deformation of toe I, stabilization of sesamoid gammak under head of metatarsal bone I, maintenance of movements in metatarsophalangeal joint I, shortening of period of medical social rehabilitation of patients.

THE LOCAL FIBROSIS OF CERVICAL INTERVERTEBRAL MUSCLES AND ITS RELATIONSHIP WITH DIAMETER AND BLOOD FLOW OF VERTEBRAL ARTERIES IN HEALTHY AND ILL PATIENTS WITH ARTERIAL HYPERTENSION

Abstract

The study of relationship of local fibrosis of cervical intervertebral muscles and diameter and blood flow of vertebral arteries was carried out. The duplex ultrasound analysis of vertebral arteries was implemented. The sampling included 148 patients with arterial hypertension aged from 34 to 67 years and 282 healthy patients aged from 17 to 67 years. It is established that rate of narrow (less than 2.8 mm) vertebral arteries at one of two sides increases with age. This fact testifies that narrowness of vertebral arteries is not of constitutional but age-acquired character. It is also established that the rate of fibrosis dramatically increased with age and often had asymmetric character. The more fibrosis was manifested more often the narrowing of vertebral arteries was detected. The more was fibrosis more often at the affected side was detected the decrease of velocity of blood flow, volume blood flow and the increase of pulse index. The ill patients have evidence of asymmetry of fibrosis at the left and right side as compared with healthy patients. This condition was especially marked in patients with arterial hypertension and complaints about giddiness.

THE FIRST EXPERIENCE OF EXTRACTION OF CATARACT AFTER TRANSPLANTATION OF DESCEMET’S MEMBRANE

Abstract

The article deals with the presentation of short-term results (6 months) of the first case of extraction of cataract and implantation of intraocular lens in patient with Descemet’s membrane transplant. The examination included visiometry, biomicroscopy, refractometry, endothelial microscopy, estimation of central thickness of cornea, optical coherent tomography, analysis on Scheimpflug-analyzer of front segment of eye and photo recording. The patient 53 years old, 6 months after repeated transplantation of Descemet’s membrane was applied phacoaspiration and implantation of intraocular lens because of development of steroid cataract and deterioration of vision. During the post-operational period, visual acuity increased up to 0.8. The impairment of cornea transparence and transplant conformity were not detected. The significant alterations in density of endothelium cells and central thickness of cornea as compared with pre-operation indicators were not established. For the first time implemented the extraction of cataract with implantation of intraocular lens applied to the eye with Descemet’s membrane transplant demonstrated the possibility of its secure implementation both for position of Descemet’s membrane and density of endothelium cells and for cornea transparence on the whole.

THE POSSIBILITIES OF CONFOCAL LASER ENDOMICROSCOPY IN DIAGNOSTICS OF BARRETT ESOPHAGUS

Abstract

The endomicroscopy using confocal laser scanning system Cellvizio (Mauna Kea Technologies, France) was applied to 51 patients with suspicion on Barrett esophagus during standard esophagogastroduodenoscopy. This is a new non-invasive technique which by the use of diminutive confocal endomicroscope in the real-time mode provides an opportunity to visualize various types of epithelium cells and to detect cell and vessel alterations typical for Barrett esophagus and related with it neoplastic transformations of epithelium. The data is compared concerning the endomicroscopic and morphologic reports. The Barrett esophagus and associated neoplasia can be detected with sensitivity of 97.1 and 92.7% and specificity of 94.1 and 98.2% correspondingly (accuracy 96.6 and 97.2%).

The article deals with the results of fundamental applied studies concerning the possibility of chrome nickel unload of human organism through muscle exercises applied in relatively ecological clean place. The surplus of nickel and chrome in human organism conditions the carcinogenesis. The accumulation of chrome and nickel in in human organism results in «breakdown» of microelement homeostasis. The implementation of chrome nickel unload of human organism into practice provides the defense against chemical carcinogenesis conditioned by the mentioned metals.

Abstract

The express-evaluation of degree of physical health according Apanasenko was implemented during the identification of the level of physical capacities (Martinet-Kushelevsky test). It is one of the most informative because in case of person not having sufficient health energy potential the possibility to demonstrate higher workability is close to zero. The physical development was determined on the ground of body length and body mass, vital capacity of lungs, dynamometric characteristics of right hand. От the basis of these indicators, the body mass index, life index and force index were calculated. Summarily, these indicators reflect the integral picture of physical development level. The comparative analysis was made in studied groups concerning the level of physical capacities and the level of physical development.

Abstract

The number of patients with purulent complications in area of electric cardiostimulator is increasing. The maximal danger represents bacterial endocarditis and sepsis though 90% of patients have local purulent process. The main mode of treatment of purulent complications in area of electric cardiostimulator is the deimplantation of stimulation system. This technique is traumatizing and highly unprofitable and demands alternative modes of treatment. Nowadays, the reports exist concerning such successful interventions as surgical d-bridement of area of electric cardiostimulator without deimplantation of stimulation system in patients with local purulent process. This information needs to be comprehensively analyzed.

Abstract

The arterial hypertension is an actual problem in children with chronic disease of kidneys. In children having kidney transplantation arterial hypertension conditions the affection of target-organs - left ventricle of heart in the form of its hypertrophy and lesion of kidney transplant. Due to this cause, arterial hypertension is a leading predictor of survival in this category of children. The technique of choice permitting to control arterial hypertension in these children is the per-day monitoring of arterial hypertension. The children having kidney transplantation need to undergo the per-day monitoring rarely than once a year.
