Organization of medical care to children with constipation syndrome

Abstract

The paper discusses the results of delivering medical care to children with constipation syndrome, by analyzing the case histories of 352 patients aged 1 to 17 years. Risk factors contributing to the development of colonic motor evacuatory disorders are identified in children of different age groups. The results of a follow-up of children with acute and chronic constipation are presented, which have given the chance to define the factors that influence the quality of delivering medical care to this category of patients and the causes of its recurrence.

Abstract

The paper describes an original procedure for a comprehensive study of the cardiovascular system in patients with terminal chronic renal failure on continuous ambulatory peritoneal dialysis. It gives the data of examination using this procedure in 50 patients (30 women and 20 men) on peritoneal dialysis at the Moscow City Nephrology Center. The main result of using this procedure is that a group of patients at the highest cardiovascular risk, who have a poor late prognosis and need a continuous follow-up by a cardiologist, can be identified.

Markers of the severity of ankylosing spondyloarthritis

Abstract

Ankylosing spondyloarthritis (AS) is a joint disease that leads to early disability in the able-bodied population. To study the markers of early disability, 86 patients with AS were examined. Their mean age was 32±7.6 years. The study lasted 6 years. Fifty-four (63%) patients had disability and 32 (37%) were nondisabled. The predictors of severe AS with possible disability in the first уear of the disease are its acute onset, systemacy, high clinical functional failure, inefficacy of nonsteroidal anti-inflammatory drugs, and clinical onset of the disease.

Current antipsychotic therapy and metabolic syndrome

Abstract

The paper presents the data of examining 50 patients with paranoid schizophrenia (according to the ICD-10 criteria) and type 2 diabetes mellitus (T2DM). Group 1 comprised patients receiving haloperidol; Group 2 included patients taking atypical neuroleptics. To study latent carbohydrate metabolic disturbances occurring in the use of atypical neuroleptics, the investigators examined 50 patients with their first lifetime psychotic state that met the ICD-10 criteria for paranoid schizophrenia, who had no metabolic disturbances and had received no antipsychotic therapy before inclusion into the study. These patients were randomized into 2 groups, by administering atypical antipsychotics or haloperidol. Their examination involved clinical, catamnestic, biochemical, and anthropometric studies. The paper considers various aspects of the metabolic disturbances occurring in paranoid schizophrenics during current antipsychotic therapy. Trends in the development of T2DM during therapy with classical and atypical neuroleptics are comparatively analyzed. The authors show it possible to use an oral glucose tolerance test to identify latent carbohydrate metabolic disturbances in patients with paranoid schizophrenia during current antipsychotic therapy.

Genetic markers for insulin resistance in gestational diabetes mellitus

Abstract

KCNJ, TCF7L2, PPARG2, ADIPOQ, ADIPOR, and ADIPOR2 gene polymorphisms and hormonal and metabolic markers (immunoreactive insulin, proinsulin, C-peptide, glycemia, lipid spectrum, leptin, glycated hemoglobin) were determined in 185 patients with gestational diabetes mellitus (GDM); of them 80 patients were diagnosed as having GDM during current pregnancy; GDM was diagnosed in 105 patients 4-19 years (14.6±3.8 years) ago. Analysis of the distribution of the frequencies of alleles and genotypes of polymorphic markers in the study genes in healthy versus GDM pregnant women showed a significant increase in the frequency of Lys/Lys genotype of the polymorphic marker Glu23Lys in the KCNJ11 gene, a significant association of the pro and ala alleles of the polymorphic marker pro12ala in the PPARG2 gene and the A and T alleles and AA, AT, TT genotypes of the polymorphic marker A219T in the ADIPOR2 gene with GDM. The findings indicate the genetic features of GDM, which are associated with both abnormal sensitivity to insulin and its impaired secretion.

Use of air-plasma flow during high amputations in patients with obliterating diseases of lower limb arteries

Abstract

The authors analyze the results of treatment in 249 patients with obliterating diseases of lower limb arteries, who underwent hip or upper-third leg amputation due to progressive humid gangrene in 2006 to 2009. In the study group (n = 112), low-power air-plasma flow (APF) was used in various modes at surgery. The Plazon apparatus is a source of plasma power. APF in the cut-coagulation mode was employed in all the patients during major surgical techniques (division of muscle masses and fascia, coagulation of minor vessels, treatment of nerve trunks). Before stump formation, an operation wound was additionally treated with cooled APF in NO-therapy mode. Comparison with a control group (n = 137) that had undergone traditional amputations revealed a significant reduction in the volume of intraoperative blood loss, the incidence of wound complications by 1.8 times, accelerated relief of fever and postoperative pain syndrome, and some clinical improvement, including the length of hospital stay.

Transosseous osteosynthesis in the treatment of victims with leg bone fractures

Abstract

An external fixator was used in combination with interfragmental compression of bone fragments with shape-memory effect tightening clamps in the treatment of 180 patients with shaft fractures (n = 111) and pseudoarthrosis (n = 69). Ring-shaped tightening clamps were applied for the encircling splintage of bone fragments in spiral, oblique, and comminuted fractures and for autograft fixation; S-shaped clamps were employed in transverse fractures and pseudoarthrosis. One hundred and nine (98.2%) patients with fractures and 65 (94.2%) with pseudoarthrosis achieved union of bone fragments in the anatomically correct position and had good functional results after treatment.

Development mechanisms, clinical course, and therapy of rosacea

Abstract

Forty 22-to-79-year-old patients with rosacea were examined, of them 9 (22.5%), 29 (72.5%), and 2 (5%) had erythematous-teleangiectic, papulopustular, and phymatous forms, respectively. All the patients with rosacea were divided into 2 groups. A study group of 30 patients underwent photodynamic therapy (PDT) with activation of endo- and/or exogenic photosensitizers (PS). A 5-aminolevulinic acid alasens derivative was used as an exogenous PS. A control group of 10 patients received conventional therapy. Sixteen (53.3%) patients with elevated endogenous PS levels had 1 to 8 sessions of PDT. And 14 (46.7%) patients with low endogenous PS levels used alasens and received 4 to 10 sessions of PDT. The studies demonstrated that PDT with activation of both endogenous and exogenous PSs was highly effective and permitted achievement of clinical remission and considerable improvement in 86.7% of the patients.

Gastric polyps in childhood

Abstract

Gastroscopy showed that 1.6% of children with chronic gastroduodenitis had antral polyps varying in size. A histological study revealed hyperplastic polyps in 66.6% of patients. The morphological structures of an adematous polyp and the cellular structures of the pancreas were found in the specimens of the other patients. Dynamic endoscopic imaging only is possible in case of small polyps. Gastric polyps measuring 5 mm or more are to undergo endoscopic polypectomy. When children are found to have gastric polyps, a follow-up is indicated for all cases, with repeat gastroscopic monitoring being made at least once a year.
