THE ROLE OF CARDIOVASCULAR SYSTEM IN FORMATION OF THE SOCIAL PSYCHOLOGICAL DECONDITIONING IN ADOLESCENTS OF PERM WITH EXTERNAL SYMPTOMS OF DYSPLASIA OF CONNECTIVE TISSUE

Abstract

The sample of 139 adolescents-students of 10-11 grades of high school with external symptoms of undifferentiated dysplasia of connective tissue was examined in Perm. The data of questionnaires, the Beck and Zung scales, Ruffier test, ECG and echocardiography were applied to evaluate the level of social psychological adaptation. The social psychological and cardiovascular deconditioning was established more often in the adolescents with the greatest number of the major (3 and more) and minor (5 and more) external symptoms of dysplasia of connective tissue associated with complains about heart pain.

THE ANALYSIS OF ACTIVITIES OF THE CENTER OF ACUTE INTOXICATIONS OF OMSK DURING 2001-2010

Abstract

The article deals with dynamics of hospitalized morbidity of population of Omsk as a result of acute intoxications of chemical nature in 2001-2010. The quality indicators of the center of acute intoxications of Omsk were involved. It is established that the level of hospitalized morbidity as a result of acute intoxications increased. This process resulted in the increase of number of beds for this category of patients and in the intensification of activities of toxicological service. The increase of numbers of beds for the patients with intoxication up to forty permitted to reduce the average annual bed occupancy up to 11-12%, to stabilize the workload of physician and nurse personnel and to decrease lethality indicator.

THE ORGANIZATION OF CARE OD WOMEN WITH POSTNATAL DEPRESSION ACCORDING TO SURVEY OF MEDICAL STUDENTS

Abstract

The sociological survey was applied to the sampling of 500 medical students (298 students of senior courses and 202 students of junior courses). The respondents diagnosed the postnatal depression in the described situation in 66.3% of cases. The indicators were higher in students who studied psychiatry - 83.2% of right answers against 52.9% of students who didn't study psychiatry. The recommendations of senior courses students became more qualified and grounded, including effectiveness and choice of therapy. However, after completion of studying complete course of psychiatry the number of supporters of psychotherapeutic approach to treatment of postnatal depression increased. This circumstance demands to put forward amendments into the process of students’ education of psychiatry.

THE LOWER ARTERIAL BLOOD PRESSURE LOAD IN WOMEN SUFFERING WITH ARTERIAL HYPERTENSION WITH SUBCLINICAL DEPRESSION AND NORMAL PSYCHO-EMOTIONAL BACKGROUND BEFORE AND AFTER AMLODIPINE TREATMENT

Abstract

The article deals with the issues of changes in women with arterial hypertension of both lower arterial blood pressure load of target organs and dynamics of velocity of morning increase of arterial blood pressure. The psycho-emotional background was normal and amlodipine treatment was applied. It is demonstrated that in both groups, against the background of amlodipine treatment, number of patients with lower systolic blood pressure drastically increases in night time. At that, velocity of morning increase of arterial blood pressure in most of these patients remains high thereby putting forth requirements to autoregulation of cerebral, coronary and renal blood flow. In both groups, against the background of amlodipine treatment, the valid increase of number of patients with increased variability on arterial blood pressure in night time is detected.

THE COMBINED ENDOSCOPIC HEMOSTASIS UNDER ULCEROUS GASTRO-DUODENAL BLEEDINGS

Abstract

The article deals with the results of treatment of 578 patients with ulcerous gastro-duodenal bleedings in 2001-2010. The patients are divided into two groups of 275 and 296 patients. In 2001-2005, the different methods of isolated endoscopic hemostasis were applied to the patients of first group in case of ulcerous bleeding underwent. In 2006-2010, the combined endoscopic hemostasis method developed in the hospital was applied to the patients of second group in case of ulcerous bleeding. The application of combined endoscopic hemostasis method in case of ulcerous gastro-duodenal bleedings decreased number of bleeding relapses from 17.5% to 1.7%, number of operated patients from 26.9% to 9.9% and total lethality from 4% to 1%.

THE EXPERIENCE OF SURGICAL TREATMENT OF PATIENTS WITH PYOINFLAMMATORY PATHOLOGY OF CARPAL JOINT

Abstract

The article deals with analysis of the results of operative treatment of 57 patients with pyoinflammatory diseases of carpal joint in specialized department of surgery of pyoinflammatory diseases of hand of the Moscow state clinical hospital №4 o the base of the general surgery clinic of pediatric faculty of the Russian research medical university. The classification of pyoin-flammatory diseases of carpal joint was proposed to systematize a wide specter of clinical manifestations of pathology and to diagnose and to determine policy of treatment. The active policy of treatment of pyoinflammatory diseases of carpal joint was developed too. The rational and anatomical operational accesses, differential approach to issues of patient treatment depending on acuteness and propagation of inflammatory process, alternatives of operation completion and characteristics of rehabilitation measures were included. The application of this policy resulted in improvement of immediate and distant outcomes of treatment of pyoinflammatory diseases of carpal joint.

THE ENDOSCOPIC CHARACTERISTICS OF COURSE OF CHRONIC GASTRODUODENITIS UNDER JUVENILE DERMATOMYOSITIS

Abstract

The article deals with the results of esophagogastroduodenoscopy in 65 children with dermatomiositis and in 210 children with chronic gastroduodenitis. The comparative evaluation of severity of course of esophagitis, gastritis and duodenitis in both groups of children was carried out. In all patients the single-type endoscopic signs of chronic gastroduodenitis are diagnosed. However, in patients with juvenile dermatomyositis the more expressed exacerbation of gastroduodenitis with erosions and contact vulnerability of mucous membrane is observed reliably more frequent. In certain cases, the more expressed exacerbation of gastroduodenitis with mucous membrane ulcer and focuses of destruction of esophagus wall, multiple ulcers of stomach and duodenum, submucosal hemorrhage is established. The accumulation and delay of evacuation of gastric contents significantly more often was diagnosed in children with dermatomyositis.

THE DECREASE OF EMIGRATION OF T-LYMPHOCYTES FROM THYMUS UNDER THYMOMEGALIA IN CHILDREN OF EARLY AGE

Abstract

The article demonstrates that in children aged less than 5 years the content of so called T-receptor excisional rings in mononuclears of peripheral blood is significantly (more than six-fold times) decreased. The T-receptor excisional rings are formed in the process of changes of genes of T-cell receptor in thymocytes. In the peripheral part of immune system (blood included) T-receptor excisional rings are present only in T-cells, recently migrated from thymus. The content of T-receptor excisional rings in mononuclears reflects the intensity of T-lymphocytes maturation and their emigration from thymus. Therefore, the content of T-receptor excisional rings in mononuclears of blood can be considered as an indicator of thymus functional activity. With age, the content of T-receptor excisional rings in peripheral T-cells steadily decreases. The research data signify that in children aged less than 5 years with thymomegalia the thymus functional activity is decreased as compared with age norm.

THE EXPERIENCE OF ORGANIZATION OF TREATMENT OF PATIENTS WITH VIRAL PNEUMONIA

Abstract

The article discusses the intensive therapy policy in patients with viral pneumonia in the department of reanimation in 2009-2011. Among 75 hospitalized patients prevailed persons aged 22-45 years (41 patients, 55%), including 4 pregnant women (period of 16, 29, 38 and 39 weeks). The period of patient stay in the reanimation department was 6.2 bed-days. The urgent transfer to artificial ventilation of the lungs was needed for 33 patients (44%). The base of antiviral therapy consisted of osel-tamivir. The empirical antibacterial therapy was applied using meropenem, cefepim, levofloxacin. The deceased persons consisted of 18 patients (18%), among them in 11 patients (61%) the flu virus A(H1N1)2009 was separated. There were no lethal outcomes among pregnant women.
