Decompression of the necrotic zone in different variations is the most widely used, but the effectiveness of it is debated.</p> <p><bold><italic>AIM:</italic> </bold>We evaluated the effectiveness of classic decompression of the necrotic zone and decompression using a percutaneous expandable reamer combined with bone graft.</p> <p><bold><italic>MATERIAL AND METHODS: </italic></bold>Fifty patients were included in our study. The inclusion criteria were decompression of the necrotic zone in AVN of the femoral head at stages I–II and the possibility of assessing the effectiveness of surgical treatment after 12 months. Depending on the method of decompression, the patients were divided into two groups. Group 1 included 25 patients who underwent decompression using a percutaneous expandable reamer combined with bone graft. Group 2 consisted of patients who underwent classic decompression of the necrosis area. The groups were comparable in all major clinical characteristics. The efficacy of surgical interventions was assessed after 12 months by comparing pre- and postoperative assessment of the functional state of the hip joint using the Harris Hip Score and the intensity of pain syndrome using the visual analog score (VAS). The main criterion for ineffectiveness of AVN decompression of the femoral head was the need for total hip arthroplasty.</p> <p><bold><italic>RESULTS:</italic> </bold>Twelve months after surgical treatment of femoral head AVN, group 1 patients average Harris Hip Score was 63.9, group 2 patients average Harris Hip Score was 74.1 (versus 59.1 and 63.9 before surgery, respectively); VAS was 2.7 in both groups (versus 5.5 and 4.8 before surgery, respectively). Three patients (12%) from group 1 and four patients (16%) from group 2 underwent total hip arthroplasty, to persisting pain syndrome and progression of osteonecrosis of the femoral head to the subchondral fracture stage. The differences between the groups were statistically insignificant.</p> <p><bold><italic>CONCLUSION:</italic> </bold>Decompression of the necrosis zone is an effective method of treatment of stages I and II of AVN of the femoral head, significantly reducing the intensity of pain syndrome and slightly improving the functional characteristics of the hip joint. Studies in this direction should be continued with the involvement of more profiled patients and with the analysis of the effectiveness of other joint-preserving surgical techniques.</p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Обоснование.</italic></bold> В отношении методов хирургического лечения ранних стадий аваскулярного некроза (АВН) головки бедренной кости не существует единого мнения. Наиболее широко применяется декомпрессия очага некроза в различных вариациях, эффективность которых обсуждается.</p> <p><bold><italic>Цель. </italic></bold>Оценка эффективности классической декомпрессии очага некроза и декомпрессии с использованием чрескожного раздвижного лезвия с комбинированной пластикой костного дефекта.</p> <p><bold><italic>Материалы и методы.</italic> </bold>В исследование были включены 50 пациентов. Критерии включения: декомпрессия очага некроза при АВН головки бедренной кости при I–II стадиях и возможность оценки эффективности хирургического лечения по истечении 12 мес. В зависимости от метода декомпрессии пациенты были разделены на 2 группы. В группу 1 вошли 25 пациентов, которым была выполнена декомпрессия с использованием чрескожного раздвижного лезвия и комбинированной костной пластикой. Группу 2 составили пациенты, которым выполнялась классическая декомпрессия очага некроза. Группы были сопоставимы по всем основным клиническим характеристикам. Эффективность оперативных вмешательств оценивалась через 12 мес путем сравнения до- и послеоперационной оценки функционального состояния тазобедренного сустава по шкале Харриса и интенсивности болевого синдрома по визуальной аналоговой шкале (ВАШ). Основным критерием неэффективности декомпрессии АВН головки бедренной кости считали необходимость выполнения тотального эндопротезирования тазобедренного сустава.</p> <p><bold><italic>Результаты.</italic> </bold>Через 12 мес после хирургического лечения АВН головки бедренной кости у пациентов группы 1 средняя оценка по шкале Харриса составила 63,9, у пациентов группы 2 — 74,1 (против 59,1 и 63,9 до операции соответственно); оценки по ВАШ составили 2,7 в обеих группах (против 5,5 и 4,8 до операции соответственно). В связи с сохранением болевого синдрома и прогрессированием остеонекроза головки бедренной кости до стадии субхондрального перелома 3 пациентам (12%) из группы 1 и 4 пациентам (16%) группы 2 проведено тотальное эндопротезирование. Различия между группами статистически незначимы.</p> <p><bold><italic>Заключение. </italic></bold>Декомпрессия очага некроза является эффективным методом лечения I и II стадий АВН головки бедренной кости, значительно снижая интенсивность болевого синдрома и несколько улучшая функциональные характеристики тазобедренного сустава.
