Medializing calcaneal osteotomy (MCO) is one of the main surgical techniques used to correct such deformities. However, the degree of deformity in different patients can vary significantly; thus, using the above principle, the degree of calcaneal postoperative correction can vary considerably. Based on data from various authors, patients with an insufficient correction of the heel bone axis have a residual valgus in the hindfoot. However, the lack of complete correction may result in the persistence of complaints and corrected limb recurrence.</p> <p><bold><italic>OBJECTIVE: </italic></bold>To improve the surgical treatment of hindfoot malalignment.</p> <p><italic>MATERIAL</italic> <italic>AND</italic> <italic>METHODS</italic><italic>:</italic> The study analyzed treatment results of patients with ankle sprain in the Center Traumatology and Orthopedics (Moscow) between 2012 and 2020. All implantations were performed by two surgeons. The total number of patients is 60. Fifty-five patients with follow-up periods of over 12 months after the procedure were available for a retrospective analysis and assessment of results. The study enrolled 20 men and 35 women, with a mean age of 61.6 (18,5–40,7) years years. The mean follow-up period is 62 (18–80) months.</p> <p><italic>RESULT</italic><italic>:</italic> The mean change in the Foot and Ankle Outcome Score (FAOS) pain subscale was 27.9 (range, −8.3 to 63.9) for the moderate varus group (<italic>n = </italic>16), 41.2 (range, 5.6–66.7) for the mild varus group (<italic>n = </italic>17), and 22.3 (range, −58.3 to 63.9) for the valgus group (<italic>n = </italic>18). In addition, patients with mild varus demonstrated better clinical outcomes than those with valgus; however, this difference was not statistically significant (<italic>p</italic>=0.11). No differences were found between groups in the change in scores for daily activities (<italic>p</italic>=0.26), sports activities (<italic>p</italic>=0.06), or quality of life (<italic>p</italic>=0.17) subscales of the FAOS.</p> <p><bold><italic>CONCLUSION:</italic></bold> Patients with mild varus hindfoot alignment showed significantly greater improvement than those with valgus with respect to the FAOS pain subscale and significantly greater improvement than those with moderate varus in the FAOS symptoms subscale.</p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Обоснование. </italic></bold>Тактика хирургической коррекции вальгусной деформации заднего отдела стопы, обычно включает в себя как костные, так и мягкотканные техники, определяющиеся тяжестью деформации. Одной из основных методик в комплексе хирургических техник коррекции подобных деформаций является медиализирующая остеотомия пяточной кости (МПО). Однако степень деформации у различных пациентов может значительно отличатся и, соответственно, используя вышеперечисленный принцип, степень послеоперационной коррекции пяточной кости может существенно варьироваться. По данным различных авторов, пациенты с недостаточной коррекцией оси пяточной кости имеют остаточную вальгусную деформацию заднего отдела стопы. Отсутствие же полноценной коррекции может привести к сохранению жалоб, а также к рецидиву корректируемой конечности.</p> <p><bold><italic>Цель. </italic></bold>Усовершенствовать оперативное лечение пациентов с посттравматической деформацией заднего отдела стопы.</p> <p><bold><italic>Материалы и методы. </italic></bold>Проведён анализ результатов лечения пациентов с посттравматической вальгусной деформацией пяточной кости, лечившихся в отделении травматологии и ортопедии №4 ФГБУ «НМИЦ ТО им. Н.Н. Приорова» (Москва) в период 2012–2020 гг. Все операции выполнялись двумя хирургами. Общее число пациентов — 60. Ретроспективному анализу с оценкой результатов были доступны 55 человек со сроками наблюдения более 12 мес с момента выполненного вмешательства. Из прооперированных пациентов 20 человек составили мужчины и 35 — женщины, средний возраст — 61,6 (18,5–40,7) года, средний срок наблюдения — 62 (18–80) мес.</p> <p><italic>Результат. </italic>Оценка функциональных результатов по шкале оценки функционального состояния стопы и голеностопного сустава FAОS показала значительное улучшение параметров в послеоперационном периоде по сравнению с предоперационным опросом, это было статистически значимо (<italic>p</italic> &lt;0,05). Среднее изменение субшкалы «Боль» по FAOS для группы «Варус» (<italic>n = </italic>16) составило 27,9 (диапазон от -8,3 до 63,9), для группы «Умеренный варус» (<italic>n = </italic>17) — 41,2 (диапазон от 5,6 до 66,7), для группы «Вальгусная деформация» (<italic>n = </italic>18) — 22,3 (диапазон от -58,3 до 63,9). Кроме того, пациенты группы «Умеренный варус» продемонстрировали более высокие клинические результаты, чем пациенты в группе «Вальгус», но эта разница не была статистически значимой (<italic>p = </italic>0,11).
