In turn, it is known that in the presence of bone-cartilaginous defects in knee, hip and ankle joints, the autologous matrix-induced chondrogenesis is quite successfully used. In this regard, we have proposed to use this technique in patients with hallux rigidus.</p> <p><bold>The aim of the study</bold> was to evaluate the clinical efficacy of the 1<sup>st</sup> MTP joint chondroplasty using the induced chondrogenesis technique in patients with HR, to analyze the immediate and medium-term results of the operations in terms of pain and function.</p> <p><bold>Materials and methods.</bold> The 1<sup>st</sup> MTP joint chondroplasty has been performed in 21 patients with hallux rigidus. Before the surgery the range of motion (ROM) in 1<sup>st</sup> MTP joint was measured; the foot condition was evaluated using such scales as VAS of pain, AOFAS, VAS FA. The 1<sup>st</sup> MTP joint chondroplasty was performed using the technique of the induced chondrogenesis with collagen matrix. The results of surgical treatment were evaluated within 3, 6 and 12 months after surgery.</p> <p><bold>Results:</bold> 3 months after the operation, a significant decrease in pain, an increase in ROM in 1<sup>st</sup> MTP joint and an improvement in the foot function were observed. Subsequently, a moderate positive dynamic was observed.</p> <p><bold>Conclusion:</bold> the results of the operations showed that the 1<sup>st</sup> MTP joint chondroplasty can be an effective method of surgical treatment, which allows to relieve pain and significantly improve the quality of life of patients with hallux rigidus, both young and elderly. Also, this technique can be used in the treatment of patients with rheumatic diseases of the low activity or remission.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> В настоящее время единого подхода к выбору тактики оперативного лечения hallux rigidus (HR) не существует. С другой стороны, известно, что при наличии дефектов хряща в голеностопном, коленном и тазобедренном суставах успешно используется техника аутологичного индуцированного матрицей хондрогенеза. В связи с этим нами предложено применение данной техники в лечении пациентов с HR.</p> <p><bold>Цель исследования</bold> — дать оценку клинической эффективности хондропластики I плюснефалангового сустава (ПФС) по технике аутологичного индуцированного матрицей хондрогенеза у пациентов с HR, проанализировать ближайшие и среднесрочные результаты проведенных операций с точки зрения боли и функции.</p> <p><bold>Материалы и методы.</bold> К настоящему времени хондропластика I ПФС выполнена 21 пациентам с HR. При обследовании определялся объем движений в I ПФС; состояние оценивалось по таким шкалам, как ВАШ боли, AOFAS, VAS FA. Хондропластика I ПФС осуществлялась по технике индуцированного хондрогенеза с использованием коллагеновой матрицы. Результаты проведенного хирургического лечения оценивались через 3, 6 и 12 мес. после операции.</p> <p><bold>Результаты.</bold> Уже через 3 мес. после операции отмечено выраженное значимое уменьшение боли в I ПФС, увеличение объема движения в нем и улучшение функции оперированной стопы. В дальнейшем наблюдалась умеренная положительная динамика.</p> <p><bold>Заключение.</bold> Результаты проведенных операций показали, что хондропластика I ПФС может являться эффективным методом хирургического лечения, позволяющим купировать боль и существенно улучшать качество жизни пациентов с HR как молодого, так и пожилого возраста.
