The patients were devided in two groups: open surgery (group 1, n=39) and minimally invasive surgery (group 2, n=15). Different approaches were used based on the type of deformity (according to Berzhano and Lamartine), localization and extent of lumbar stenosis. The evaluation of volume of decompression, blood loss, time of surgery and postoperative treatment, intraoperative complications, pain and patients condition were measured using clinical scales ODI, ZCQ, SF-12 was performed.</italic></p> <p><bold><italic>Results</italic></bold><italic><bold>.</bold> No differences in the type of deformity, the severity of stenosis and clinical manifestations between groups were discovered before treatment. All characteristics of the surgery (except the intraoperative complications) — blood loss, duration of surgery, and hospital stay — were significantly less in the minimally invasive group. Postoperative assessment has shown in group 1 greater regression of axial pain syndrome (p=0.03), in group 2 greater regression of radicular syndrome (p=0.03). Assessment of quality of life based on questionnaires after 2 years has revealed no differences between groups.</italic></p> <p><bold><italic>Conclusion</italic></bold><italic><bold>.</bold> Using of minimally-invasive decompression and decompression-stabilizing surgical technologies in patients with combined stenosis and deformity of the lumbar spine allow to eliminate the syndromes of neural compression, provide sufficient correction of segmental relationships and improve the quality of life.</italic></p></abstract><trans-abstract xml:lang="ru"><p><italic><bold>Цель исследования</bold>: провести сравнительный анализ клинических результатов применения минимально инвазивных и открытых хирургических технологий у пациентов с симптоматическим дегенеративным стенозом и сколиотической деформацией поясничного отдела позвоночника.</italic></p> <p><italic><bold>Пациенты и методы</bold>. Выполнен ретроспективный сравнительный анализ отдаленных клинических результатов 54 пациентов, которым были выполнены открытые (1-я группа, п=39) и минимально инвазивные (2-я группа, п=15) декомпрессивные и декомпрессивно-стабилизирующие вмешательства на позвоночнике в зависимости от типа деформации (по Berjano и Lamartina), локализации и протяженности поясничного стеноза. Изучали объем выполненной декомпрессии и кровопотери, длительность операции и послеоперационного лечения в стационаре, интраоперационные осложнения, а также уровень болевого синдрома и качество жизни исходно и через 2 года после операции с использованием шкал VAS (раздельно для спины и ног), опросников ODI, ZCQ, SF-12.</italic></p> <p><italic><bold>Результаты.</bold> До операции обе группы пациентов были сопоставлены по типу деформаций позвоночника, выраженности стеноза и клиническим проявлениям. Все показатели хирургического этапа, за исключением интраоперационных осложнений, — кровопотеря, продолжительность операции, пребывание в стационаре — были значимо меньше у пациентов в группе минимально инвазивных вмешательств (2-я группа). В послеоперационном периоде у пациентов 1-й группы наблюдался более выраженный регресс аксиального болевого синдрома (р=0,03), во 2-й группе — радикулярного синдрома (р=0,03). Оценка качества жизни на основании анкетирования через 2 года показала отсутствие преимуществ в обеих группах.</italic></p> <p><italic><bold>Заключение.</bold> Использование у пациентов, сочетающих стеноз и деформацию поясничного отдела позвоночника, минимально инвазивных декомпрессивных и декомпрессивно-стабилизирующих хирургических технологий позволяет успешно устранить синдромы компрессии, обеспечить достаточную коррекцию сегментарных взаимоотношений и улучшить качество жизни.</italic></p></trans-abstract><kwd-group xml:lang="en"><kwd>degenerative diseases of the spine</kwd><kwd>symptomatic lumbar stenosis</kwd><kwd>degenerative lumbar scoliosis</kwd><kwd>minimally invasive spinal surgery</kwd><kwd>quality of life</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>дегенеративные заболевания позвоночника</kwd><kwd>симптоматический поясничный стеноз</kwd><kwd>дегенеративный поясничный сколиоз</kwd><kwd>минимально инвазивная спинальная хирургия</kwd><kwd>качество жизни</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Weinstein J.N., Lurie J.D., Tosteson T.D. et al.
