The shortcomings of antibiotic use and methods of their local delivery for surgical and orthopaedic needs are considered. The conception of local use of antibacterial agents and the requirements for the current “ideal” antibacterial agent are given. Classification of the local antibiotic delivery systems on the basis of their physicochemical properties is presented as well as the number of prospective methods for the prevention of microorganisms’ adhesion on the surface of the implanted devices and systems that could be used in traumatology and orthopaedics are examined.</p></abstract><trans-abstract xml:lang="ru"><p>В настоящем обзоре литературы охарактеризовано общее состояние проблемы инфекции области хирургического вмешательства, в том числе применительно к травматологии и ортопедии, рассмотрены существующие известные недостатки применения антибиотиков и ряда средств их доставки для хирургических и ортопедических нужд. Дано представление об оптимальной концепции локального применения антибактериальных агентов наряду с требованиями, предъявляемыми на современном этапе к «идеальному» антибактериальному агенту.
