All patients were male, with a mean age of 34.5±7.82 years. The mean time from injury to elbow replacement surgery was 193±39 days. The range of motion in the elbow joint was assessed. The QuickDASH and the Oxford Elbow Score were used to subjectively assess treatment outcomes after 3 and 6 months. CT examinations were performed at each stage.</p> <p><bold>RESULTS: </bold>A follow-up examination after 6 months showed an improvement in the range of motion in the majority of patients. The following criteria indicated a positive outcome: range of motion without limitations (Table 4, RF Government Decree No. 565 of July 4, 2013, On Approval of the Regulation on Military Medical Examination), QuickDASH score &lt;45, and Oxford Elbow Score &gt;70. The outcome was good in 17 (68%) patients and satisfactory in 5 (20%) patients. Three (12%) patients had postoperative complications (one patient’s elbow implant component was destroyed, and two patients developed deep periprosthetic infections), resulting in poor outcomes. Thus, the elbow joint function was successfully restored using additive technology in 22 (88%) patients.</p> <p><bold>CONCLUSION: </bold>In patients with upper extremity gunshot wounds that have destroyed the elbow joint, additive technology rapidly and completely restores the upper extremity function. However, the long-term outcomes of this treatment need to be investigated further.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Проблема лечения пациентов с огнестрельными ранениями верхних конечностей, сопровождающимися разрушением локтевого сустава, остаётся актуальной в травматологии и ортопедии по настоящее время.</p> <p><bold>Цель. </bold>Оценить среднесрочные результаты хирургического лечения пациентов с огнестрельными ранениями верхних конечностей с разрушением локтевого сустава в комплексном лечении с применением аддитивных технологий.</p> <p><bold>Материалы и методы.</bold> За период с 2022 по 2024 г. в Центре травматологии и ортопедии ФГБУ «ГВКГ им. Н.Н. Бурденко» Минобороны России были пролечены 25 пациентов с огнестрельными ранениями верхних конечностей, сопровождающимися разрушением локтевого сустава, с применением аддитивных технологий. Все пациенты были мужского пола, их средний возраст составлял 34,5±7,82 года. Средний срок установки индивидуального протеза локтевого сустава от получения ранения составил 193±39 суток. Оценивали амплитуду движения в локтевом суставе, субъективную оценку результатов лечения пациентов определяли по двум шкалам — Quick DASH и Oxford Elbow Score — через 3 и 6 месяцев. На всех этапах проводили КТ-исследование.</p> <p><bold>Результаты. </bold>При контрольном обследовании через 6 месяцев у большинства пациентов отмечается улучшение амплитуды движения. За хороший результат мы приняли следующие критерии: амплитуда движения без ограничений по таблице 4 Постановления Правительства РФ № 565 от 04.07.2013 г. «Об утверждении Положения о военно-врачебной экспертизе», по функциональным шкалам — менее 45 баллов по Quick DASH, более 70 баллов по Oxford Elbow Score. У 17 (68%) пациентов получен хороший результат, у 5 (20%) пациентов — удовлетворительный. У 3 (12%) пациентов в послеоперационном периоде развились осложнения (у 1 пациента — разрушение локтевого компонента эндопротеза, у 2 — глубокая перипротезная инфекция), что в итоге привело к неудовлетворительному результату. Таким образом, количество пациентов, у которых было достигнуто успешное восстановление функции локтевого сустава с применением аддитивных технологий, составило 22 (88%).</p> <p><bold>Заключение. </bold>Применение аддитивных технологий в лечении пациентов с огнестрельными ранениями верхних конечностей, сопровождающимися разрушением локтевого сустава, позволяет в короткие сроки вернуть полноценную функцию верхней конечности, однако отдалённые результаты данного лечения требуют дальнейшего изучения.</p></trans-abstract><kwd-group xml:lang="en"><kwd>post-gunshot defect</kwd><kwd>3D implants</kwd><kwd>additive technologies</kwd><kwd>elbow joint replacement</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>постогнестрельный дефект</kwd><kwd>3D-имплантаты</kwd><kwd>аддитивные технологии</kwd><kwd>эндопротезирование локтевого сустава</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Khominets VV, Kudyashev AL, Dresvyannikov AS, et al.
