Сравнительный анализ результатов заднего межтелового спондилодеза (PLIF) и трансфораминального межтелового спондилодеза (TLIF) в сочетании с транспедикулярной фиксацией.

Аннотация
Представлены данные сравнительного изучения результатов использования методик заднего (PLIF) и трансфораминального (TLIF) межтелового спондилодеза при дегенеративном поражении поясничного отдела позвоночника. Прооперирован 101 пациент, из них 47 (1-я группа) выполнены транспедикулярная фиксация, декомпрессия, дополненная межтеловым спондилодезом одним кейджем (TLIF), 54 (2-я группа) - транспедикулярная фиксация из заднесрединного доступа, декомпрессия интраканальных сосудисто-нервных образований и межтеловой спондилодез двумя межтеловыми имплантатами (PLIF). Комплексную оценку результатов хирургического лечения провели у 52 пациентов через 6-12 мес после операции. Показано, что результатом применения обеих методик в 95-98% случаев является формирование межтелового блока. Представлен дифференцированный подход использования данных способов межтелового спондилодеза, обозначены преимущества TLIF перед PLIF: простота и большая безопасность выполнения, меньшие объем интраоперационной кровопотери и время хирургического вмешательства.
