PSYCHIATRY IN ADDICTION MEDICINE’S ERA: NEW DIAGNOSTIC AND THERAPEUTIC REALITIES

Abstract

The article is devoted to the analysis of new psychiatric reality. It is noted that for the last decades the ratio of incidence of mental and addictive disorders which makes one to two has now cardinally changed. The increase in number of psychotic disorders, owing to the use of “new (design) drugs” without dependence signs is noted. The problem of frequency rising of shizophrenoformic psychotic disorders needs special studying. The analysis of a possible role of cannabins and psychostimulans in a schizophrenia etiopathogenesis is important. The question of ethical validity and expediency of studying of therapeutic effects of psychoactive drugs in cases of resistant psychopathological symptoms needs closer attention too.

CORRELATION OF AGGRESSION AND REGIONAL GLUCOSE METABOLISM RATES OF PATIENTS WITH EPILEPSY

Abstract

The article is dedicated to the investigation of the interconnection of and the regional glucose metabolism rates of the human brain in the patients with epilepsy. We have managed to reveal that epileptic patients in the quiet wake state demonstrate a rise of different types of aggression, which is mostly correlated with the glucose metabolism change in the left hemisphere of brain. With the rise of physical and verbal aggression, the increase of the left parietal lobe metabolic rate and the decrease of the left occipital cortex metabolic rate have been registered. Enhanced hostility, soreness, negativism, and suspicion are correlated with the decrease of the left prefrontal cortex glucose metabolism rate.

THE ANALYSIS OF IMPULSIVITY AND ITS CONSEQUENCES IN THE STRUCTURE OF THE PERSONALITY

Abstract

The three forms of the impulsivity are revealed and analyzed: (1) the impulsivity as reaction on the psychotraumatic events that harms the narcissistic image of the grandiosity; (2) the impulsivity as the result of the impact of unconscious triggers in the childhood, and (3) the impulsivity as the end reaction of the accumulation of many micro traumatic episodes that reflect the content of the traumatic episodes during the relative long periods of life. The role of the unsatisfactory mentalization as a significant factor in the appearance of the impulsivity is established. The necessity of the correction of the impulsivity in the process of psychodynamic psychotherapy for the prevention of its dangerous consequences is emphasized.

The analysis of the basic innovations in the classification of substance use disorders is given in the DSM-5 in comparison with the previous (DSM-IV), the present (ICD-10) and the future (ICD-11) guidelines. Scientific discussion including substantial paramedical arguments is noted to have positive effects on productivity-enhancing clinical guidelines. It is stated that Russian narcology is low sensitive to the recent changes.

There was presented a clinical case of a patient with superficial siderosis of the central nervous system. The presence of the clinical triad (sensorineural hearing loss, cerebellar ataxia, pyramidal symptoms), a progressive course of the disease and characteristic neuroimaging features (MRI in T2-gradient echo and SWI - Susceptibility-Weighted Imaging - MRI images, weighted by the inhomogeneity of the magnetic field) in the form of hemosiderin deposits has allowed to establish the diagnosis.

Abstract

There was conducted an observational study in the industrial center of the Republic of Kazakhstan (Pavlodar city), the claim of which was the studying of mental reaction patterns among visitors of bookmaker offices at the moment of hazardous activity. 216 clients from 10 bookmaker offices of the city became object of the research. It was revealed that patterns of mental reaction differed among players with various risks of involvement in a pathological gambling. Players with high risk of gambling addiction were characterized by a variety of neurotic reactions in the current mental status.

There were studied incidence, comorbidity and risk factors of cannabis addiction and its connection with suicidal behavior among 331convicted male population of penal colony of the city of Ufa. The analysis showed high comorbidity of cannabis addiction with depression and attention deficit hyperactivity disorder. Cannabis addiction can be a risk factor of suicidal behavior in male offenders.

PSYCHOMETRIC INDICATORS OF INTERNET ADDICTION OF STUDENTS- COMPUTER USERS

Abstract

Psychometric method of flexible battery was used both for researching qualitative and quantitative features of Internet usage and attitude towards Internet; and the indicators of social adaptation, psychic health and somatization among universities students of Ufa. More than a quarter of respondents are on the eve of Internet-addiction, more than a half show the features of emotional discomfort and nearly 40% demonstrate the features of various socio-psychological disorders and their intensity. Clinical signs of anxious and depression demonstrate an imperceptible number of students-Internet users. But 40% of respondents expressed sufficient problems in neuropsychic sphere; at that the sleeplessness is prevailing. Average number of students-computer users had shown somatization features with prevailing complaints on sense of vision and fatigability, back and arm aches. Research results prove the states on features developing of computer user’s syndrome among students and register sufficient necessity of the further research of Internet-addiction behavior of students.

Abstract

Comparative analysis of clinical and psychodiagnostic data of 155 male and 110 female adolescents under conditions of juvenile correctional facilities was carried out. It was revealed that males as opposed to females began to use psychoactive substances earlier; and long-term substance using has provoked increase in aggressive and hostile reactions in males. Onset of substance use occurs in females in later periods than in males but nature of consumption is more variable and dynamic. The expressed aggressive features in females have promoted their involvement in drug addiction.
