Organization of medical rehabilitation of veterans of wars in the context of social support of senior generation in Russia

Abstract

Tasks of the organization of medical care at the present stage lie in the effective and economical use of available health care resources and in the increase of the accessibility and improvement of the quality of medical services. The effectiveness ofprovision of medical services to veterans is based on the systematic monitoring and permanent implementation of health measures aimed on social support of the older generation, along with the maintenance and preservation of health. Emerging trends of population aging as a complex phenomenon of the general civilizational character, reflect at the same time the results of the sequential implementation of legislation in relation to older people in the Russian Federation at Federal, regional and municipal levels. In the current socio-economic conditions on the territory of the Rostov region (RR) there was created and developed and a comprehensive system of rehabilitation ofdisabled persons. In the article there is highlighted the activity of the RR "Hospital for veterans of wars", which in the region is the only the state institution intendedfor the provision of specialized medical care to participants and invalids of the Great Patriotic War and equivalent categories of the population

Scoliosis in children: new approaches to the solution of important medical and social problems

Abstract

Scoliosis, the most prevalent orthopaedic condition affecting children and adolescents, may have long-lasting medical and social consequences. The objective of the present study was to elucidate the possibilities and practicability of the application of a low-frequency electrostatic field for the conservative treatment of grade I and II scoliosis in children. The electrostatic field is shown to promote the development of vibrations of varying strength in biological tissues and their penetration rather deeply into their interior. We examined and treated 94 children divided into two groups matchedfor the age and major clinical manifestation of the disease. All the patients received basic therapy. Patients from the study group were additionally given low-frequency electrostatic therapy. The application of a low-frequency electrostatic field for the conservative treatment of grade I and II scoliosis in the children was shown both to significantly increase its effectiveness and facilitate regression of scoliosis.

Prosthetics in complex medical and social rehabilitation of patients with ocular pathology

Abstract

There is presented an analysis of results of observations in patients underwent different ocular prosthetic procedures: 152 patients with keratoprosthesis; 5456 cases with congenital or acquired anophthalmia, microphthalmus and ocular subatrophy with ocular prosthesis and 63 patients with ecto prosthesis. There are considered clinical, organization and legal aspects of indications for different prosthetic procedures, possible complications and their prevention. There was determined the role ofprosthetics in rehabilitation and medico-social adaptation in patients with severe ocular pathology which helps to improve the quality of life and rehabilitation ofpatients in dependence on their age, kind of the occupational activity and psychosomatic state.

Therapy with valproate preparations in disabled epilepsy patients in the Republic of Komi

Abstract

Epilepsy is the widespread and hard to cure disease. The medico-social importance of the problem is caused by the fact that among patients with epilepsy there is a considerable share of able-bodied age persons, most part of whom fail to receive the adequate treatment. The aim of the work is an assessment of demographic indices in epilepsy patients, being on therapy by preparations of valproic acid and having the group of disability in the Komi Republic. By the method of continuous research there were studied data over the period from 2006 for 2009 in patients from the Komi Republic, being on monotherapy by preparations of valproic acid and polytherapy where one ofpreparations was valproic acid. Out of872 patients disability was established in 36% of cases. Most ofpatients were on monotherapy by preparations of valproic acid. In disabled patients, the symptomatic type of epilepsy was mainly observed and generalized toniko-clonic attacks occurred more often than others.

Dynamics of indices of primary disability of the child population due to diseases of the musculoskeletal system for the period of 2005-2014 in the Altai Territory

Abstract

The level of disability, along with the indices of the infant mortality rate, morbidity rate, physical development and health demographic processes is a basic index of child health, reflecting the level of economic and social well-being of the country. In the Russian Federation children with disabilities make up, according to various estimates, from 1.5-2.5 to 4.5% of the child population. In the structure of the primary disability of children in the Altai Krai the share of disabled children due to musculoskeletal system pathology in 2005-2014 varied from 13.2% to 26.0%. The level of the primary disability of children due to pathology of a musculoskeletal system increased from 5.7 in 2005 to 6.5 per 10 thousands of the child population in 2014. The gain of the intensive index of the primary disability of children due to pathology of a musculoskeletal system is caused by the increase in the disability level due to diseases of the nervous system (from 2.0 to 4.0 per 10 thousands of the child population) and congenital defects of a musculoskeletal system (with 1.1 to 1.6 per 10 thousands of the child population).

The given article raises questions about the application of questionnaires for the assessment of the quality of life, inability to the use the self-assessment systems in the classic sense in patients with craniopharyngiomas, and the need for the doctor himself to deliver systems for the evaluation of results of the management and outcomes.

In the article there are analyzed external links of the large multidisciplinary institution of the social support of the population «Scientific and Practical Center of Rehabilitation» from the point of view of the identification of relationships of social partnership in them. These relationships were shown to have diverse forms and are implemented in terms of the participation of representatives of various sectors in the activity of collective bodies of institution’s management. There is demonstrated the significant role of relationships of the social partnership in the determination kinds, volumes, quality and circumstances of the provision social welfare services to the population.

Legal aspects of the work with request containing medical secrecy of the citizen in the institution carrying out the medical expertise

Abstract

The scope of present research is the civil-law protection of health (medical) confidentiality of the citizen in the activity of the institution, carrying out a medical expertise. Mentioned category is considered in this article in aspects of the work with requests to obtain or provide, without the consent of the citizen, information bound by his medical secrecy. There are proposed algorithms of actions for processing mentioned information. As the ingredient of the topic there are considered rationales for its delivery (assignment). It is widely acknowledged that in the construction of legal norms regulating relations concerning medical confidentiality, one of the main task of the legislator is to maintain a balance between the interests of the participants of legal relations - as holders of intangible benefits and public interests of the society, through the formation of a special legal regimes. By determination of the legalframework of the execution of the medical expertise, the State sets: conditions of the execution of expertise; conditions for the recognition the citizen as disabled; conditions for the establishment of his disability, etc. In the case of the establishment of the incapacity for work, disability, it enables the citizen a certain legal and social status, but at the same time obliges him to undergo periodic medical examinations (checkups) for the possibility of saving or cancel (change) provided benefits that state is normal practice for the social welfare state. In these and other cases, the citizen is supposed to disclose certain information constituting his medical secrecy - both to employees of medical institutions, as well as to the members of the expert committees, the employer (by authorized persons), etc. The right to disclosure ofsuch information without the consent ofthe citizen, as an exception according to the federal law may be afforded to other persons - within the limits set by law.

The availabilities of thermal waters in the Mostovsky district of the Krasnodar krai in a complex system of rehabilitation and medico-social rehabilitation

Abstract

Restorative medicine (RM) is based on principles of sanology, concepts of the preservation of human health (valeology) including occupational one. The origins of VM are SPA methods and systems of the restorative treatment (medical rehabilitation) within the framework of the fundamental principle of national health care are the prevention. The main stages of the development of RM are related to studies of regularities of the preservation and restoration of health, investigation of the process of adaptation and natural aging of the body, the elaboration of advanced methods for the premorbid diagnosis and correction of disturbed functional state of the human. In the article there are presented the availabilities of the using of the mineral springs of the Mostovsky district of the Krasnodar region in the system of modern RMfor a wide variety of somatic pathology.

Requirements of stroke patients in medical and social rehabilitation and modern technologies of its implementation (according to foreign literature)

Abstract

There is presented the analysis of the incidence rate and outcomes of different types of strokes. Depending on the level of average income per capita, these indices have significant differences, which should determine the strategy of medical care, prevention and rehabilitation. The basic element contributing to the restoration of brain structure and functional capabilities of the brain after lesions of the different character, is the phenomenon of neuroplasticity. Neuroplasticity varies widely depending on the location of the lesion and the time elapsed since the end of the impact of the damaging factor. In addition to traditional methods of rehabilitation, more and more attention of scientists is attracted to new technologies associated with both the development of the technical devices used for recovery of cognitive and motor functions, as well as with the improvement of the organization of the rehabilitation process. Out of technological innovations in the review there are presented games controlled by the movement (gesture), including the Nintendo Wii and Xbox Kinect, brain-computer interfaces, wearable robotic gait control system. The organization of the management of rehabilitation process by the patient can play a major role in the enhancement of the motivation in patients. Just the combination of the mobilization of intact functions, and the patient’s motivation with the use of high clinically tested technologies can ensure the success of rehabilitation.
