Abstract

The article describes the emergence in the legislation of the Russian Federation the concept of "professional sport", as well regulatory framework, current problems in this area and the problem of occupational diseases. It provides research, which was aimed to give scientific evidence for the role occupational medicine in professional sports and the development of methodological approaches to the complex hygienic evaluation of working conditions, diagnostics, and expertise of the correlation between profession and the prevention of occupational diseases among professional athletes.

Abstract

On the basis of analysis of the regional traumatologic and orthopedic service condition and that of medico-social expertise service negative moments were detected as follows: high indices of traumatism, musculoskeletal system sickness rate, invalidity, ineffective outpatient assistance and absence of in-patient orthopedic care the adults, poor material and technical basis of treatment-and-rehabilitation center. Recommendations on the regional program of medico-social rehabilitation of invalids with consequences of trauma and musculoskeletal system diseases are proposed.

Abstract

Various groups of invalids differentiated according to the level and character of locomotor system lesions and other diseases and their consequences, whose anatomo-functional opportunities demand to administer special clothes wearing, were considered in the article. Special clothe as rehabilitation means were developed in our country to present day for the armless invalids only. However the practice has show that other contingents of invalids also need special functional-aesthetic clothes. Development of special clothes able to constructively compensate the lost functions or to create comfort and to hide by illusion the parts of the body damaged by the aliment is quite topical and task both in medical and in social aspect.

Abstract

Results of study of the opinions of specialists medico-social expertise of Moscow, Omsk and Regions on the problem of invalids with mental disorders in cerebro-organic diseases after discharge from the hospital after compulsory treatment were presented. The data of questionnaire of 34 doctors-experts of the above territories were summarized. It was established that social factors in the vital activity of invalids and prevalence of social measures in individual rehabilitation programs were the most significant in expert tactics of the specialists. The means of quality of life improvement in the period after completed in-patients compulsory treatment were revealed. The authors arrived at the conclusion on the necessity of a more thorough study of social problems of the category of invalids.

Abstract

Problem of development and application of the complex, differentiated rehabilitation methods for the patients of gerontopsychiatric profile staying at the in-patient institutions of social protection was covered. Correlation of evaluations of clinical status and that of the level of social activity of various groups of patients in rehabilitation measures development was shown. The evaluation criteria of the indices of social activity of the patients were considered. A specially developed flow sheet of rehabilitation programs with rational approach to the maintenance of social activity of the patients depending on the revealed condition was presented.

THE CURRENT STRATEGY OF RISK ASSESSMENT AND PREVENTION OF BONE FRACTURES AMONG THE EMPLOYEES AND WORKERS OF MOSCOW

Abstract

The present study was designed to develop an optimal strategy for risk assessment and prevention of bone fractures among the employees and workers of Moscow region, who were not previously examined, and did not receive anti-osteoporotic therapy. The study included 737 women aged 45 to 64 years (mean age 53,9, ± 4,7 years). Evaluation of ten-year risk of bone fractures was carried out using the mathematical analysis of the FRAX system (John A. Kanis, 2008). Evaluation of bone mineral density (BMD) was performed using dual energy x-ray absorption densitometry (DEXA) «HOLOGIC» with the definition of BMD in the three areas. The condition of thoracic and lumbar spine was evaluated with the help of morphometric vertebral semi-quantitative Dzhenanta method (IVA). X-rays of thoracic and lumbar spine in lateral projection were carried out to confirm the presence of a fracture. Analyzing the data obtained it was established that in the age group 45-65 years, the method of determining the ten-year fracture risk FRAX has low sensitivity compared with standard diagnostic methods of osteoporosis and can not be recommended as the primary method for clinical examination of women of this age in spite of its low-cost and availability.

The conducted examination of 189 patients and invalids with cardiac rhythm disturbances due to the ischemic heart-disease after and without acute myocardial infarction using correlation and regression analyses allowed considering rhythm disturbances possessing linear correlations with clinic- functional, psychological, social and professional indices to be a significant evaluation indicator of myocardium functional state allowing objectively evaluate the degree of vital functional limitation in patients with ischemic heart-disease.

Abstract

Model of prognostication of assessment according to Bartel scale in 1, 3, 6 and 12 months after stroke including initial assessment according to Bartel scale and National Institutes of Health Stroke Scale (NIHSS) was developed. In patients with brain infarction in the medial cerebral artery basin an assessment according to ASRECTS scale additionally included into the model was expedient. In prognostication of adverse outcome (5-6 numbers on Rankin scale) an assessment according to the Glasgow Coma Scale (GCS) was the most significant on admission. Application of complex model gave some improvement: Complex index = 7.14 - 0.07 NIHSS - 0.548 GCS- 2.91 Bartel (0) - 0.005 x (cross dislocation, mm) - 1.03 x (axial dislocation, mm).

FORMATION OF DISABILITY DUE TO CEREBRAL PARALYSIS IN CHILDREN IN THE REPUBLIC OF DAGESTAN 2002-2010

Abstract

These are the results of disability formation regularitites study in 3445 children as a result of cerebral paralysis in the Republic of Dagestan in 2002-2010. The high proportion of persons with disabilities due to cerebral paralysis in a class of diseases of the nervous system (49.2%). The level of primary disability was clearly reduced and amounted to 2.4 per 10 thousand child population. In the structure of primary disability by age children of the first 3 years of life are dominating — 73.5%. The highest level ofprimary disability is also observed in children during the first three years of life, and was 7.8 per 10 thousand of the corresponding children population. In the structure of primary disability by sex in all years, girls (51.5%) were dominating. The level of children re-disability due to cerebral paralysis in the RD for the years 2002-2010 was 11.1 per 10 thousand children population. The number of children re-registered as disabled due to cerebral paralysis was 4-6 times higher than that initially recognized as disabled children. The peak frequency of re-examination was observed among disabled aged 8-14 years. There was a high proportion of re-inspected children aged 4-17 years (88.6).

Abstract

Sports cardiology is a part of the general cardiology and examines the positive and negative changes in the circulatory system, arising under the influence of intense physical activity, also addresses issues of diagnosis, treatment and prevention of pathological changes that result from excessive physical exertion. A correct understanding of the sports heart physiological mechanisms, i.e. heart, which has a high ability to adapt, allows sport doctors to make a diagnosis "healthy", an objective assessment of the cardiovascular system of athletes and observe favorable changes appearing in the rational ongoing training load. Proper understanding of the ways and laws of development of pathological "sports heart" development, provides the ability to timely diagnosis, management and early treatment and prevention of pre-pathological conditions and pathological changes in the circulatory system of athletes arising during various unfavorable conditions, of the physical labor associated with sports activities.
