SKIN INVOLVEMENT IN CHRONIC LYMPHOID LEUKEMIA

Abstract

About 25% patients with CLL develop skin lesions. These lesions can be classified as specific (including skin infiltration by leukemic cells and the cutaneous form of Richter’s syndrome), secondary cutaneous tumors, and concomitant cutaneous diseases. Leukemic infiltration of the skin in CLL patients is called specific in volvement of the skin (SIS) or skin leukemia. Many authors attribute the unfavorable prognosis to the CLL transformation with specific infiltration of the skin into Richter’s syndrome and to emergence of SIS before the diagnosis of CLL. The risk of cancers in CLL patients is 3-fold higher, of skin cancer 8-fold higher than in normal subjects. The most frequent secondary tumors of the skin in CLL are basal-cell carcinoma, squamous-cell carcinoma, melanoma, Merkel’s tumor. Nonspecific cutaneous changes vary greatly and are detected in 30—50% CLL patients. The most prevalent secondary changes in the skin in CLL are those of infectious nature. These patients can also develop hypersensitivity reactions to insect bites, generalized itching, exfoliative erythroderma, nodular erythema, paraneoplastic pemphigus, bullous pemphigoid, toxi coderma, etc.

PEMPHIGOID LICHEN PLANUS

Abstract

The paper presents the clinical and morphological characteristics of a rare pemphigoid form of lichen planus, a cross-syndrome — lichen planus and bullous pemphigoid. The pathogenesis is unclear. Clinically this form is presented by the initial manifestation of cavitary elements with a compact cap and subsequent emergence at the site thereof of polygonal papulous elements of typical lichen planus. Of 279 patients with lichen planus, observed at Department of Dermatovenereology and Dermato-Oncology of M.F.Vladimirsky Institute over the recent decade, this form was diagnosed in 6 (2.2%). A clinical case with pemphigoid lichen planus is described, effectively treated by a combination of delagil, tetracycline, and elocome.

UVA 370 NM PHOTOTHERAPY OF ATOPIC DERMATITIS

Abstract

The UVA (l = 370 nm) mechanism of action and its use in atopic dermatitis, therapeutic modes, indications and contraindications are discussed. The authors present the results of treatment of 24 patients by UVA 370 nm phototherapy. The patients were distributed into 2 groups. Group 1 consisted of 14 patients with moderate atopic dermatitis, receiving UVA monotherapy. Group 2 consisted of 10 patients with moderate to severe atopic dermatitis, receiving UVA phototherapy in combination with conventional therapy (antihis taminic drugs, adsorbents, emollients). The SCORAD index of group 1 patients reduced from 15.6 before therapy to 3.4 after therapy, in group 2 from 28.6 to 7.5, respectively. No side effects, except moderate dry ness of the skin, were recorded. The data indicated high efficiency and safety of the method.

Abstract

Studies of the somatic, psychological, and social components of the quality of life in 125 patients showed that periods of exacerbations were characterized by the anxiety syndrome, blue moods, and loneliness. Only 30.4% patients with psoriasis were completely satisfied with their life style; the disease did not disturbed everyday activities in 44.8% patients with psoriasis.

NECROBIOSIS LIPOIDICA AND THE METABOLIC SYNDROME

Abstract

Manifestation of necrobiosis lipoidica, running the course of granuloma annulare, was observed in a male patient of 56 years with the metabolic syndrome under conditions of photodermatitis relapse. The diagnosis was confirmed histologically. The studies revealed early disorders in carbohydrate and porphyrin metabo lism and insulin resistance. The patient received combined therapy including metformine. The treatment results are discussed. A total of 54 relevant publications are reviewed and modern concepts on the pathoge netic mechanisms, clinical polymorphism in necrobiosis lipoidica, and therapeutic strategy in this patient population are discussed.

ROLE OF ANTIBODY PRODUCTION IN THE FORMATION OF RESISTANCE TO BOTULOTOXIN A DRUGS

Abstract

Injections of botulotoxin A are among the most frequently used procedures in esthetic medicine. Botulo toxin A is a protein, the main mechanism of its elimination is antibody production. Hence, the production of antibodies specific to botulotoxin drugs can become the cause of botulin therapy inefficiency. However, some authors claim that the development of botulin therapy resistance is not caused by antibody production. An original method for detection of antibodies to botulotoxin A drugs is suggested, based on enzyme immu noassay. Antibodies to botulotoxin A were found in 64% of 119 patients (114 women and 5 men, mean age 40 ± 9 years), who received low-dose botulin therapy for esthetic indications (disport 60—200 Units, botox 20—100 Units). Antibodies were found in patients with good effect and with failure of therapy. Antibodies to botulotoxin A were also found in 24% donors without history of contacts with botulotoxin. The results indicate that antibody production is a factor essential for the efficiency of botulin therapy, as it can reduce its efficiency. However, antibody production is presumably not the main factor and not the only one, and further studies are needed to detect these other factors.

RATIONAL THERAPY OF ACNE

Data on the pathogenesis of acne and its modern therapy are presented. The molecular mechanisms of retinoid activity are reviewed and the authors’ hypotheses on their impact for the development of retinoid dermatitis are discussed. A comprehensive approach to disease therapy making use of Zerkalin solution with consideration for the severity and prevalence of eruptions is discussed. Special attention is paid to acnecu tan, an acne control drug.

SUPPRESSIVE EFFECTS OF PSORALENE PHOTO-OXIDATION PRODUCTS ON CONTACT SENSITIVITY REACTION IN MICE: LYMPHOCYTE PROLIFERATION INHIBITION AND APOPTOSIS INDUCTION

Abstract

The effects of psoralene photo-oxidation products (PPP) on 2,4-dinitrofluorobenzene (DNFB) contact sensi tivity of mice were studied. Injections of PPP to DNFB-sensitized mice led to a decrease in the cell counts in the regional lymph nodes and to reduction of the cellular proliferative activity. Proliferation reduction was paralleled by changes in the lymph node cell cytokine balance: lesser production of IL-2, IL-4, and INF-γ and higher production of IL-17. Interestingly that PPP did not change the production of IL-10 and IL-6 by the lymph node cells in vivo. In vitro incubation of the lymph node cells of DNFB-sensitized mice with PPP led to induction of these cells’ apoptosis. It seemed that modulation of the lymphocyte cytokine profile under lay the mechanism of PPP immunosuppressive activity, this leading to reduction of these cells’ proliferative activity and apoptosis induction.
