Abstract

The paper presents review of literature and a case of rare form of B-cell lymphoma involving the skin - lymphomatoid granulomatosis in a 49 years old patient. The diagnosis was confirmed by clinical, histologic, immunohistochemical and molecular-biological examination.

Spectrum of autoimmune disorders in paraneoplastic pemphigus

Abstract

Results of indirect immunofluorescence analysis of curculating autoantibodies in paraneoplastic pemphigus and investigation of their role in development of this disease are presented. It was estimated that a wide variety of IgG autoantibodies play a role in manifestation of paraneoplastic pemphigus. Pathologic process involves not only desmosomal apparatus of stratified squamose epithelium and Hassal's bodies but also of parenchymatous organs, such as heart and liver. Detection of additional antibodies (particularly to antigens of discs in cross-striated muscles, myocardium capillaries, structures of conjunctive tissue in liver, and to nuclei of liver cells) in autoimmune pemphigus can raise a suggestion about paraneoplastic nature of pemphigus . Mosaic character of antibodies allow to consider paraneoplastic pemphigus as a multiorgan syndrome.

Age-specific characteristics of skin diseases development

Abstract

Persons at senile and advanced age apply to dermatologists more often than to other doctors for the correction of age-related changes. Authors analyzed the results of examination of 7795 patients aged from 6 months to 95 years old to detect correlations of involutory skin changes in patients with carbohydrate or lipid metabolism disorders as well as in thyroid gland dysfunction during gerontogenesis. Among them patients exposed to adverse environmental factors were included (n = 328, 60-96 years old). Results of the study showed that influence of anthropogenic environmental factors leads to predomination of such involutory skin changes as senile angiomas (46,3%) and lentigo (39,0%). The study showed that in advanced age a complex of proliferative, pigmented and vascular skin changes develop, which indicate the acting factor. Analysis of somatic comorbidities in skin disorders showed that in skin disorders 1,8 somatic disorder per patient are diagnosed; in diabetes mellitus - 2,2; in hypothyrosis - 2,8; in lipid metabolism disorders - 3,8. Thus, the frequency of somatic diseases increases in metabolic disorders, especially in lipid metabolism disorders.

Treatment of endotoxinemia syndrome in patients with atopic dermatitis

Abstract

The role of endotoxin in pathogenesis of atopic dermatitis (AD) is discussed. A total of 81 patients with AD were examined (68 men and 13 women from 16 to 53 years old). In 60% of patients disorders of gastro-intestinal tract were diagnosed, in 90% of subjects disbalance of gastrointestinal microflora was detected. In all patients mean level of endotoxin (lipopolyssacharide) in blood was above normal. A pathogenetic method of AD treatment is suggested including enterosorbents, probiotics, and hepatoprotectors.

Modern aspects of psychopharmacotherapy in dermatologic practice

Abstract

The most widely used method of treatment of mental disorders in dermatological practice is psychopharmacotherapy which includes a wide variety of psychotropic agents classified into different pharmacologic classes (antipsychotics, antidepressants, anxiolytics). First of all one should assess the structure and severity of mental disorder to decide where (in psychiatric or dermatologic in-patient department or in out-patient department) and how to treat the patient. In a dermatologic in-patient and out-patient departments modern effective and safe medications of first line are recommended. These drugs can be differently used according to a diafnosis of somatoform, depressive and anxious disorders.

Comparative effectiveness of narrow-band 311 nm UVB therapy of psoriasis

Abstract

Results of treatment of 70 patients with psoriasis vulgaris with narrow-band phototherapy (UVB 311 nm) are presented. In 88,6% of patients clinical remission was achieved. Effectivity and safety of this therapeutic method was demonstrated. It was shown that 4 times a week regimen is equally effective as 3 times a week regimen and has a shorter duration.

Abstract

The latest published data about epidemiology, pathogenesis, classification, diagnostics and treatment of pyodermia are presented. Results of own study are demonstrated. Study sample included 25 patients with primary and secondary pyodermia. A complex clinical assessment was conducted in patients with this disorder regarding the effectivity of treatment with "Gexicon" (chlorhexidine) gel. The results of this study allow to draw a conclusion about high effectivity of monotherapy with this drug, which can be recommended for wide use not only in in-patient but also in out-patient departments.
