Selecting an optimal therapy for such psychoses remains challenging. Despite the critical importance of managing synthetic cathinone-induced psychoses, existing studies typically describe therapeutic strategies and lack a comparative analysis. This article systematizes current approaches to managing psychoses associated with the use of synthetic cathinones.</p> <p><bold>AIM:</bold> This study aimed to determine potential therapeutic strategies for cathinone-induced psychoses and to identify associations between treatment approaches, psychosis subtype, duration of psychotic symptoms, changes in positive symptoms, and severity of cognitive impairment.</p> <p><bold>METHODS:</bold> The study included 98 patients &gt;18 years with psychotic disorders. Synthetic cathinones or their metabolites were detected in biological samples using gas chromatography–mass spectrometry. The Brief Psychiatric Rating Scale (BPRS) was used to assess the severity and trends in psychotic symptoms on days 1 and 10 of psychosis. Upon resolution of psychosis, defined as cessation of delusions and/or hallucinations, cognitive impairment was evaluated using the Mini-Mental State Examination (MMSE) scale.</p> <p><bold>RESULTS:</bold> Four principal therapeutic strategies were identified: a basic strategy incorporating infusion therapy and benzodiazepines; two strategies combining basic regimen with either typical or atypical antipsychotics; and a strategy involving forced diuresis and propofol. Psychosis duration was found to be determined primarily by psychosis subtype rather than therapeutic strategy. BPRS scores on days 1 and 10 differed significantly (<italic>F</italic> = 347.5; <italic>p</italic><sub>ANOVA</sub> &lt; 0.0001). BPRS scores decreased by 52.1% with the basic strategy (<italic>q</italic> = 2.732; <italic>p</italic><sub>Tukey</sub> &lt; 0.0001); by 45.0% with typical-antipsychotic strategy (<italic>q</italic> = 3.020; <italic>p</italic><sub>Tukey</sub> &lt; 0.0001); by 22.2% with atypical-antipsychotic strategy (<italic>q</italic> = 3.874; <italic>p</italic><sub>Tukey</sub> = 0.0074); by 53.3% with the strategy combining forced diuresis and anesthetic agents (<italic>q</italic> = 3.139; <italic>p</italic><sub>Tukey</sub> &lt; 0.0001). Analysis of MMSE scores showed that after the resolution of psychotic symptoms, the mean score was 25.9 ± 0.75. Between-group ANOVA revealed no significant differences (<italic>F</italic> = 0.6731; <italic>R<sup>2</sup></italic> = 0.02060; <italic>p</italic><sub>ANOVA</sub> = 0.5706).</p> <p><bold>CONCLUSION:</bold> Standardizing therapeutic approaches to psychoses induced by psychoactive substances is an essential step toward establishing effective clinical pathways for these patients. This study systematizes therapeutic approaches and compares selected psychometric parameters across different treatment strategies.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Психотические расстройства, вызванные употреблением психоактивных веществ — это гетерогенная группа психических расстройств, включающая в себя как интоксикационные, так и эндогенные психозы, манифестировавшие на фоне приёма психоактивных веществ. Выбор оптимальной терапии таких психозов — сложная задача. Несмотря на чрезвычайную актуальность вопроса терапии психозов, вызванных синтетическими катинонами, в существующих исследованиях лечебные тактики представлены лишь в виде описания, без дополнительного анализа. В данной статье представлена систематизация используемых подходов к лечению психозов, связанных с употреблением синтетических катинонов.</p> <p><bold>Цель исследования </bold>— определить возможные тактики лечения катинон-индуцированных психозов, а также выявить взаимосвязь между используемой тактикой, формой психоза, его длительностью, динамикой продуктивной симптоматики и выраженностью когнитивных нарушений.</p> <p><bold>Методы.</bold> В исследовании приняли участие 98 пациентов старше 18 лет с психотическими расстройствами, у которых с помощью газовой хромато-масс-спектрометрии в биологических средах были обнаружены синтетические катиноны или их метаболиты. Для объективизации оценки тяжести и динамики психотической симптоматики в 1 и 10-е сутки психоза применяли шкалу Brief Psychiatric Rating Scale (BPRS). После разрешения психоза, то есть после исчезновения бреда и/или галлюцинаций, когнитивные нарушения оценивали по шкале Mini-Mental State Examination (MMSE).</p> <p><bold>Результаты.</bold> Описаны четыре основные терапевтические тактики: базовая тактика с применением инфузионной терапии и бензодиазепинов; две тактики, в рамках которых базовая тактика дополняется применением типичных или атипичных антипсихотиков; тактика, включающая применение форсированного диуреза и пропофола. Обнаружено, что длительность психоза в основном обусловлена его формой, а не лечебной тактикой. Баллы по шкале BPRS на 1 и 10-е сутки лечения статистически значимо различаются (<italic>F</italic>=347,5; <italic>p</italic>ANOVA &lt;0,0001). При использовании базовой тактики баллы по шкале BPRS снижаются на 52,1% (<italic>q</italic>=2,732; <italic>p</italic>Tukey &lt;0,0001), тактики типичных антипсихотиков — на 45,0% (<italic>q</italic>=3,020; <italic>p</italic>Tukey &lt;0,0001), тактики с применением атипичных антипсихотиков — на 22,2% (<italic>q</italic>=3,874; <italic>p</italic>Tukey=0,0074), тактики с применением форсированного диуреза и средств для наркоза — на 53,3% (<italic>q</italic>=3,139; <italic>p</italic>Tukey &lt;0,0001). Анализ оценок по шкале MMSE показал, что после купирования психотической симптоматики средний балл составляет 25,9±0,75; межгрупповой дисперсионный анализ не выявил статистически значимых различий (<italic>F</italic>=0,6731; <italic>R</italic><sup>2</sup>=0,02060; <italic>p</italic>ANOVA=0,5706).</p> <p><bold>Заключение.</bold> Стандартизация подходов к лечению психозов, вызванных употреблением психоактивных веществ — важный этап формирования траектории помощи таким пациентам.
