Estimation of the possibility to correct stress-induced changes in tooth pulp tissues and parodontium by modifying the enzymatic activity of NO synthase (an experimental study)

Abstract

This work was designed to study the influence of donors and blockers of NO synthase on the morphological characteristics of tooth pulp and periodontal tissues in laboratory animals exposed to stressful impacts. It was shown that L-arginine, a donor of nitric oxide molecules, can interfere with the development of stress response and thereby reduce the severity of morpho-functional disturbances in periodontal and tooth pulp tissues.

The structure and properties of the surface of glass-infiltrated oxide ceramic restorations

Abstract

Laser profilometry and scanning electron microscopy in conjunction with electromagnetic resonance and determination of the mean free microcrack propagation path were employed to study the microstructure and surface properties of oxide ceramics infiltrated with alumosilicate lanthanum glass, such as "In-Ceram-Alumina" (Vita Zahnfabrik), "In-Ceram Spinell" (Vita Zahnfabrik), and "In-Ceram Zirconia" (Vita Zahnfabrik). The data obtained were compared with the respective parameters of the enamel surface of natural teeth. It was shown that reinforced alumooxide ceramics doped with zirconium dioxide and saturated with alumosilicate lanthanum glass "In-Ceram Zirconia" (Vita Zahnfabrik) as well as dioxide ceramics coated with alumooxide porcelain "Vitadur Alfa" (Vita Zahnfabrik) are possessed of good phase homogeneity characteristics coupled to the high purity of the material and the absence of apparent porosity. These properties of the said ceramics match those of the enamel surface of natural teeth. The close resemblance of structural parameters and surface characteristics of "In-Ceram Zirconia" (Vita Zahnfabrik) with the respective characteristics of natural tooth enamel guarantees good long-term prognosis for the application of these ceramic restorations.

The importance of densitometric examination of the patients presenting with edentulism when planning dental implantation

Abstract

The patients presenting with osteoporosis are known to have significantly reduced bone mineral density of the entire skeleton including jaws which restricts the possibilities for dental implantation. In this context, the accurate diagnostics is of primary importance, e.g. with the use of double photon densitometry. Moreover, the correct choice of medicamental correction of hard bone tissues is crucial. Such diagnostic strategy makes it possible to improve the efficacy of preparing the patient for the forthcoming dental implantation.

The state of salivary glands in the patients presenting with metabolic syndrome

Abstract

A total of 82 patients presenting with metabolic syndrome were examined. All of them were found to have the altered state of salivary glands in the form of dystrophic processes, such as sialadenosis. The principal manifestation of these changes was the compromised function of major and especially minor salivary glands. The authors maintain that these changes may be regarded as a consequence of the integrated pathological processes underlain by insulin resistance playing the role of the common pathogenetic mechanism.

Restoration of canine and incisal guidance with the use of the CAD/CAM technology as an advanced method for high-quality tooth replacement

Abstract

The authors undertook a comparative analysis of the efficacy of restoration of canine and incisal guidance with the use of metal-ceramic and all-ceramic orthopedic constructions and composite restorations. The estimation was based on the objective criteria that allowed to distinguish between clinically acceptable and unacceptable orthopedic constructions. It was demonstrated that neither the material nor the design of these constructions has a significant influence on the early results of the treatment. However, long-term follow-up observations revealed the advantage of the CAD/ CAM technology.

Peculiarities of periodontal tissue lesions in the patients presenting with systemic lupus erythematosus

Abstract

A clinical and laboratory characteristic of periodontal pathology in the patients presenting with systemic lupus erythematosus (SLE) is presented. The factors are considered that complicate lesions of oral mucosa and facial skin (butterfly rash, lupus spots, hard palate enanthema, petechiae, ecchymoses, ulcerations). The clinical symptoms of periodontal pathology are shown to be related to the laboratory characteristics of SLE activity and its severity. Special attention is given to the peculiar features of bone tissue destruction in the jaws of the patients with periodontitis and systemic lupus erythematosus.

Optimization of the methods for the treatment of hard tooth tissues located in the perigingival region

Abstract

Defects in the perigingival region is a commonest pathology of hard tooth tissues that exerts the unfavourable action on the health state of marginal parodontium. The subgingival cavities serve as reservoirs of microorganisms pathogenic for parodontium. The chronic inflammatory process in marginal parodontium developing in response to microbial invasion may be a cause of local periodontitis (periodontal pocket/gum recession). The treatment of perigingival defects must include elimination of the main etiological factors of the disease, such as occlusal supracontacts, cessation of pernicious habits, and normalization of hygienic conditions of the oral cavity. Substitution therapy consists of filling the defect. Composite materials are supposed to be the best choice for this purpose due to their physicochemical and mechanical properties. Optimal filling conditions my be achieved by application of hemostatic and gingival retraction pastes.

The sensory function of parodontium of the front teeth group at various stages of orthopedic treatment with the use of block and single constructions

Abstract

The sensory function of parodontium of the front tooth group under normal conditions and at various stages of orthopedic treatment with the use of block and single constructions was studied with the use of an original method (food item retention and disintegration test) and a Perosensometer apparatus. The patients included in the study had intact periodontal tissues. It was shown that the sensory function of parodontium underwent 3-4-fold deterioration at such stages of orthopedic treatment as anesthesia and fixation of blocks of temporary mouthguides. The sensory function either recovered after fixation of permanent single orthopedic constructions, e.g. veneers, or remained compromised after permanent bridge prostheses had been installed.

Abstract

Restoration of the normal function and movements of the jaws in the patients presenting with temporomandibular joint (TMJ) ankylosis encounters difficulties. We analysed long-term results of TMJ substitution arthroplasty (within the follow-up period of up to 5 years) in young children aged from 1 to 5 years with uni- and bilateral ankylosis. The plastic surgery was performed as a one-step operation in combination with the removal of ankylotic masses and the substitution of the defect with an endoprosthesis. A new type of endoprostheses were employed that substantially improved functional properties of the TMJ head.

Prevention of periodontal inflammatory diseases in pregnant women

Abstract

The objective of the present study was to estimate the efficacy of combined prophylaxis of periodontal inflammatory diseases in pregnant women. The study involved 95 pregnant patients aged from 17 to 36 years. Those included in the study group (group 1) were treated using a complex of preventive measures (CPM) in combination with hygienic preparations based on bee products. The women of the control group (group 2) were treated with the use of CPM alone, those of the reference group received neither CPM nor bee products. The state of the periodontium and the hygienic conditions of the oral cavity were estimated based on dental plaque index (DPI), comprehensive periodontal index (CPI) and index of prevalence and severity of gingivitis (PMA index). The results of the study indicate that the complex of preventive measures comprising professional hygienic treatment of the oral cavity, sanitary education, monitoring the oral cavity condition, and recommendations on rational nutrition makes it possible to stabilize the hygienic status of the oral cavity. However, the best results have been achieved by combining these measures with the use of hygienic preparations based on bee products. This approach resulted in the reduction of DPI, CPI, and PMA indices from the first to the third trimetser of pregnancy by 41.7%, 2.4%, and 14.7% respectively.
