Abstract

Use of laser radiation is especially important in modern clinical practice when it is required to carry out treatment and assessment of its efficiency on the modern principles of «diagnostics at the place of treatment» that objectively has to provide both medical, and diagnostic use of the laser radiation influencing to a biological object. It's necessary for the purpose of identification of positive (negative) physiological and clinical effects, ensuring timely correction of negative effects of laser radiation on a live organism or lack of those. It determines the need and validity of consideration of the all-biological concept of interaction of laser radiation with a biological object as medical-diagnostic. Both primary and secondary effects, and the functional changes induced by them and pathological process can be registered with modern technical means and used for the porpose of optometry of the flow and evaluation of the effectiveness of treatment using laser radiation and laser medical equipment and medical technology. The offered hardware programm complex raman-fluorescent diagnostics will technically and technologically allow high degree ofsensitivity, objectively and authentically provide the most important stage of inspection - diagnosis of a disease, monitoring of his current and process of rehabilitation in general.

PHYSICOCHEMICAL CHARACTERIZATION: COMPARATIVE EVALUATION OF ALLOGRAFT BIOMATERIALS AND AUTOGENOUS BONE

Abstract

Objectives: bone substitutes used in oral surgery include allografts, xenografts and synthetic materials that are frequently used to compensate bone loss or to reinforce repaired bone by encouraging new bone ingrowth into the defect site. The aim of this study was to evaluate a number ofphysical and chemical properties in a variety of allografts biomaterials used in oral surgery and to compare them with those of autogenous bone. Materials and methods: autogenous bone andfive different allograft biomaterials were studied by high-resolution X-ray diffractometry, atomic absorption spectrometry, laser diffraction, and checked for their chemical composition, calcium release concentration, crystallinity and granulation size. Results: the highest calcium release concentration was 24.94 mg/gforPuros® and the lowest one was 4.05 mg/gfor OsteoSponge® compared to 20.15 mg/g to natural bone. The range ofparticles size, in term of median size D50, varied between 394.24 pm for DIZG Spongiosa® and 902.41 pm for OsteoSponge®, compared to 282.1 pm for natural bone. Bone and Puros® displayed a hexagonal shape as bone except and OsteoSponge® which showed a triclinic shape and all the rest showed monoclinic shape. Conclusion: a bone substitute of choice depends largely on its clinical application that is associated to its biological and mechanical performance. These morphological differences between biomaterials greatly influence their in-vivo behavior of biomaterials. Significant differences were detected in terms of calcium concentration, particles size, and crystallinity.

THE RESULTS OF HISTOLOGICAL STUDY OF THE IMPACT OF LASER RADIATION IN THE SELECTIVE PHOTOTHERMOLYSIS MODE ON VASCULAR LESIONS IN EXPERIMENT

Abstract

This article is dedicated to the increasing of the effectiveness of treatment of patients with vascular lesions of the skin using the radiation of KTP-Nd:YAG laser with a wavelength of532 nm. An experimental research was conducted to study the effect of laser radiation with an energy density of 4, 6, and 8 J/cm2 used on blood vessels ofauricles of experimental animals (rabbits) according to the histological study in dynamics. As a result of the experiment, it was found that laser radiation with a radiation density of up to 4 and 6 J/cm2 does not damage the epidermis of the skin, which indicates the selectivity of the effect on the blood vessels of the rabbit auricles. The histological pattern is distinctive for that ofselective photothermolysis. The processes ofneoangiogenesis, proliferation of fibroblasts, collagen production, fibrillogenesis end with the maturation andfibrous transformation ofthe granulation tissue without its cicatrical deformation. The use of KTP-Nd:YAG laser may increase the quality of treatment of patients with skin lesions.

SOME CLINICAL AND EPIDEMIOLOGICAL ASPECTS OF THE MAXILLOFACIAL AREA PHLEGMONS

Abstract

Introduction. It is known that the number of patients with phlegmons does not tend to decrease, which can be due to both disease pathomorphosis and changes in the epidemiological features of this pathology. In addition, a significant contribution to the development of the disease makes the characteristics of the comorbidity background. The aim of the study was to assess the clinical and epidemiological aspects of the morbidity of the Perm city population with the maxillofacial area phlegmons. Materials and methods. A retrospective analysis of 137 case reports ofpatients with facial phlegmon was carried out. Results. It is shown that young and middle-aged patients dominated in the age structure of the disease. Phlegmons often localized in 1-2 spaces. In more than half the cases, the phlegmons was in the submandibular space. Among the etiological factors of the disease, the leading place is occupied by gram-positive cocci and their associations. An essential role of anaerobic microflora in the pathogenesis of phlegmons has been established. Microorganisms of the Enterobacteriaceae family play a significant role in the formation of inter-microbial associations in the development of this pathology. Often phlegmons of the maxillofacial region are found in patients with concomitant somatic pathology. With a comorbid component, more frequent detection of microorganism associations is indicated. In addition, such patients enter the hospital much later. Conclusion. Thus, when assessing clinical and epidemiological data, it is shown that the phlegmons of the maxillofacial region occur, as a rule, in young and middle age, and also in the presence of comorbid pathology. A significant contribution to the development of the disease is made by associations of microorganisms, which may include a change in the severity ofpathogenicity factors.

Clinical and instrumental diagnostics, including electromyographic (EMG) and axiographic (EA) study of 24 patients to improve the efficiency of planning and predicting the results of their dental treatment was carried out. Correlation links were studied both within EMG and EA studies, and between them. There were no significant correlations between the parameters of EMG and EA of the methods of investigation. SURGICAL-PROSTETIC REHABILITATION PATIENTS WITH MALOCCLUSION AND DENTAL ARCHES DEFECTS

Abstract

Rehabilitation patients with malocclusions and dental arches defect is important separate question. It depends on differences in treatment planning this type of patients. Actually, there is prosthetic stage which replaces orthodontic preparation. There are two possibilities of this algorithm. 1 - orthognathic surgery and prosthetic dental arches reparation. 2 - simultaneously orthognathic and preimplantation surgery and at second stage - dental arches restoration (usually with dental implants). This approach opens many options: simultaneous surgery cut rehabilitation time, combination some operations gives simple approach for bone grafting and big resomves for bone harvesting, preliminary planning of surgery and prosthetic rehabilitation leads to best esthetic and functional results.

USING GALIMETER IN DIAGNOSTICS OF INTRAORAL GALITIOSIS IN PERIODONT DISEASES

Abstract

Diagnosis of the level of halitosis (bad breath) is very important, because this may indicate pathology gastrointestinal tract, upper respiratory tract, endocrine system. The most common cause of its occurrence is oral disease. Objective. To determine the effectiveness of the treatment of periodontal disease, halitosis accompanied by a galimetr apparatus. Materials and Methods. The study involved 118 people with periodontal disease of varying severity, accompanied by periodontitis. In the study group underwent a comprehensive treatment of periodontal disease, accompanied by halitosis, with the inclusion of probiotic complex. The control group consisted of 58 people with periodontitis, accompanied by halitosis, did not receive the probiotic complex. Periodontal status was determined by Silness-Loe index Myullemana index, measuring the depth of periodontal pockets. halitosis level determined by galimetr apparatus. The data were processed using statistical methods. Results. After treatment in both groups showed improvement in the patient's mouth and a statistically significant change in the indicators characterizing it. The average depth of the periodontal pocket of the first group ofpatients has decreased by 2 mm (or 40%) patients of the second group of reduction was 1.9 mm (or 42.2%). After 6 weeks of treatment of halitosis level significantly decreased in both groups. In the first group to 67.7 (6.4 times), the second group - to 157.8 (2.8 times). Level halitosis patients in group 6 weeks after treatment was significantly lower than the patients of the second group of halitosis. The level of halitosis patients of the first group is 2.3 times lower than that of the second group ofpatients, demonstrating the effectiveness of the treatment. Conclusions. The use of probiotic complex in treatment of inflammatory periodontal diseases, accompanied by halitosis increases the efficiency of traditional therapy. A galimetr is a necessary tool to measure and evaluate the level of halitosis.

Abstract

Studied the nature of healing the wounds of post-resection in 36 patients with carcinoma of the tongue before and after the surgical stage of treatment in the local application of 2% solution of chitosan acid in the postoperative period. The clinical healing process of language, in terms of local effects 2% acid chitosan solution through all phases of regeneration and at the same time as in the comparison group. Cytologic evaluation of the prints of the wound surface is established that wound healing on the background of the use of chitosan occurs with less microbial contamination and prevalence ofpolyblasts and fibroblasts.

Abstract

Since conservative methods of treatment of chronic apical periodontitis are not always effective, the operation of resection of the apex of the tooth root does not lose its relevance. To make up for the lost bone tissue, osteoplastic materials are used, which make it possible to accelerate the regeneration process in the defect zone. These materials must be biocompatible, have osteoinductive and osteoconductive properties. MULTI-SEGMENT OSTEOTOMY OF UPPER JAW AT THE LEVEL OF LE FORT I IN PATIENTS WITH DYSGNATHIAS COMPLICATED EDENTULOUS UPPER JAW

Abstract

The article presents clinical examples of the application of methods of multi-segment osteotomy of upper jaw at the level of Le Fort I, as a preparatory stage before the prosthesis on dental implants in 6 patients with dysgnathias complicated edentulous upper jaw. We examined and operated on 6patients agedfrom 28 to 55 years with designtime complicated edentulous maxilla, c complete growth of the facial skeleton. The average period of postoperative follow-up was 24 months. The method of multi-segment osteotomy of upper jaw at the level of Le Fort I, used in the treatment of such patients, allowed to normalize the ratio of the alveolar bases of the upper and lower jaws and to create the conditions to achieve functional and stable result of implantation and prosthetics.

EVALUATION OF THE STRENGTH PARAMETERS OF TONGUE MUSCLES IN PATIENTS WITH ANKYLOGLOSSIA

Abstract

Thefunctional model ofthe neuromuscular apparatus ofthe tongue, its power characteristics, and also the parameters ofendurance play an important role in the formation of the majority ofmalocclusion disorders. The aim of the study was to compare the strength parameters of tongue muscles in patients with ankyloglossia before active complex therapy and at the first stage of treatment. The study program involved a group ofpatients with ankyloglossia, which included 39 children from 7.5 to 9.3 years. The control group included 20 people of similar age. The strength parameters of the tongue were measured through the IOPI Medical system. It was established that in patients with ankyloglossia of the 1 st and 2 nd degree, the decrease in the force parameters of the muscles of the tongue responsible for vertical excursions is determined. The strength activity of the muscles - protractors of the tongue in the individuals with ankyloglossya before treatment were significantly higher in comparison with the similar parameters of the control group. Frenulotomy, combined with subsequent orthodontic therapy and miogymnastics leads to an increase in the strength activity of tongue muscles that determine vertical excursions and reach control values within 1-2 months. It is determined that the use of the game model, which increases the motivation ofpatients for complex therapy, enhances the endurance of the neuromuscular apparatus of the tongue, as well as intensifies the normalization of the functional activity of the tongue muscles.

ANALYSIS OF QUALITY OF LIFE OF ELDERLY PATIENTS AT THE STAGE OF REHABILITATION WITH REMOVABLE ORTHOPEDIC DESIGNS

Abstract

In the elderly and senile age, the need for medical care, including dental care, is increasing. The untimely appeal for dental care entails a change in the KPI index, as a result of which a partial or complete loss of teeth occurs. Complete absence of teeth is accompanied by morphofunctional changes of all elements of the dentoalveolar system, a significant decrease in chewing ability. Providing orthopedic dental care to the population of elderly, senile and old age is not easy due to the peculiarities of adaptation to removable dentures. In this study, the impact of removable dentures on the quality of life of a dental patient was assessed. The study involved 217 people aged 75 to 95 years. All patients needed removable prosthetics. Before the prosthetics, a questionnaire was conducted using the OHIP-14 questionnaire. The questionnaire showed the level of satisfaction with their dental health of the patients being examined. Then proceed to prosthetics. In the first group, the prosthesis was manufactured using a standard method. In the second group, the impression was removed using a modified individual spoon, which should improve adaptation to a removable denture. One month after the prosthesis, when all patients had complete adaptation to a removable denture, a second questionnaire was conducted using the OHIP-14 questionnaire. In order to reveal the dynamics of the quality of life of the patients being examined. The results showed the effectiveness of the proposed method. The number of patients with a good standard of living in group 2 increased by 58.8%, and in group 1 only by 34.5%. In group 2, adaptation to removable dentures was more rapid.

Abstract

Singing and playing various musical instruments affect not only the condition of the body, but also the dental status: for example, the violinists change occlusal relationships, dysfunction of the temporomandibular joint develops. Musicians playing wind instruments are susceptible to diseases of the lips and require a special approach to prosthetics. The dental status of vocalists has been least studied, although there are studies that have revealed their dry mouth, more frequent recurrences of herpes labialis and dysfunctional disorders of the temporomandibular joint.

Abstract

This review aims to analyze the domestic and foreign professional literature, and it shows the main positive and negative properties of the materials for the manufacture of membranes used in modern dental practice in bone-grafting interventions and guided tissue regeneration. Emphasis is placed on the importance of developing a new individual membrane made of polytetrafluorethylene, due to high relevance of such researches owing toreduce the risk of infection of surgical wounds and high degree of adaptation to the geometry of the bone defect.
