Abstract

Article describes electromyographic studies as an important part in the structure of diagnostics and orthopedic dental treatment of patients with increased erasure of teeth. The results of the correlations between elecrocromyography revealed strong reliable connections during the work of the actual chewing and temporal muscles on the right and left. It is confirmed that muscle dysfunction does not occur in the area of only one isolated group, but involves muscles on both sides and often depends on the parafunctional habits of the patient and the use of one side or another during chewing.

Abstract

The article presents data on the relevance of use of adhesive materials when using removable laminar prostheses under various conditions prosthetic bed in patients with complete absence of teeth; on the development of optimal formulations and methods of making adhesive compositions for removable laminar dentures, modified with silver ions, with pronounced antimicrobial activity, bioinertness and high fixing properties to improve fixation and stabilization of removable partial dentures complete dentition in the period of adaptation of patients to newly manufactured prostheses and in the process further their use; and described in detail a number of studies on the physico-mechanical characteristics of the developed material on the basis of technical laboratories: Federal state budgetary educational institution of higher professional education «Voronezh state medical University named after N. N. Burdenko» Ministry of health of the Russian Federation, company for the production of dental materials «Celite» (Voronezh); and in clinical practice: on the basis of the dental clinic of the «VSMU them. N. N. Burdenko» Ministry of health of Russia and in private dental clinics, under which was revealed quite pronounced fixing properties of the modified compositions in relation to the base removable laminar denture and tissues of the prosthetic bed, when proven to this antibacterial activity and bioinertness developed material, thereby clearly defines the relevance of the use of the adhesive composition, modified with silver ions, as an effective means to improve the fixation and stabilization of removable partial dentures in the oral cavity of patients with complete absence of teeth, contributing to more comfortable with their further operation.

RESULTS CHILD ORTHODONTIC REHABILITATION WITH CHRONIC OSTEOMYELITIS MAXILLA HISTORY OF

Abstract

Chronic osteomyelitis in children poorly understood disease. Moreover, insufficient data on patients’ rehabilitation, it underwent. Chronic osteomyelitis is more common in the age of 8-10 years, the more interesting the clinical case of rehabilitation of the child who has had chronic osteomyelitis of the upper jaw to the right at the age of 4 years. In most cases chronic form of osteomyelitis in children occur соmplex dentoalveolar deformations, facial aesthetics violation. To eliminate the disease apply complex orthodontic and surgical interventions, dentures. We explored the possibility of forming a physiological ratio dentoalveolar arches using orthodontic rehabilitation of a child who has had chronic osteomyelitis at an early age. Monitoring and treatment of the patient was carried out at the age of 4-15 years. In the treatment of used removable orthodontic appliances replacement dentition defect, non-removable orthodontic appliances, orthopedic extraoral traction. For the control treatment and the results of the analysis were used different types of X-ray examination: a panoramic picture, computed tomography, as well as calculations of models of the jaws and face and smile detection photometry. During treatment persists infantile type was eliminated, to form a neutral type of jaw relations, create conditions for the eruption of permanent teeth on the modified disease right half of the upper jaw. A detailed analysis of the results showed that the effectiveness of orthodontic treatment depend on its early start, the sequence and scope of the orthodontic treatment, care, timeliness, as well as the activity of a doctor and patient cooperation.

PRENATAL ULTRASONIC DETECTION OF CONGENITAL CLEFTS OF LIP AND PALATE IN KRASNODARSKII KRAI

Abstract

The goal of our study was investigation of prenatal diagnostics of pathology and distribution of associated anomalies in the group of children - patients of clinic with congenital maxillofacial anomalies. Basing on oral poll of mothers given birth to children with the pathology during the period 2012-2014 in Krasnodarskii krai (358 cases), the efficiency of prenatal detection of congenital clefts of lip and palate (diagnosis Q36, Q35 and Q37 by MKB-10) by ultrasonic investigation was studied. Prenatal 2D ultrasonic investigation is performed three times during pregnancy on 12, 18-24 and 32-34 weeks of pregnancy. Cleft lip or palate were detected on the second and third ultrasonic investigation: cleft palate (Q35) was revealed only in 5,6% cases of children born with cleft palate, and cleft palate (Q36 and Q37) - in 33,8% cases. Importance of prenatal ultrasonic diagnostics of pathology is not only predetermined by the necessity of mental conditioning of parents for the most urgent and proper rehabilitation of ill children, but, also, by the fact that patients with clefts of lip or palate frequently have associated pathology of other organs and chromosomal anomaly. For proper prenatal detection of cleft palate 3D ultrasonic investigation is necessary. We also studied the presence of associated pathology distribution for the cases of isolated clefts of lip or palate. Higher efficiency of cleft lip and palate detection in prenatal period by ultrasonic diagnostics would support more successful rehabilitation by well-timed mental conditioning and informing of parents about stages of rehabilitation in the process of child growing. Ethical aspects of probable abortion because of detected pathology during ultrasonic investigation.

CITOKINE PROFILE OF SALIVARY AND GINGIVAL FLUID IN PREGNANT WOMEN

Abstract

Objective: to determine the peculiarities of the cytokine state of the oral salivary (SF) and gingival fluid (GF) in pregnant women with a physiological course of pregnancy in the dynamics of the gestational period. Materials and methods. In 31 healthy pregnant women without dental pathology and in 32 healthy volunteers, the content of interleukins (IL) IL-4 and IL-8 in the SF and GF enzyme immunoassay was studied. Results. The changes in IL-4 and IL-8 in the dynamics of pregnancy in the SF and GF in the clinical group for the directivity were similar: from 1 to 3 trimester, the content of cytokines increased. In healthy donors there were no differences in IL-4 in two biological media. In the clinical group, in all three trimesters, the concentrations of IL-4 in the GF were significantly higher in comparison with SF. The concentration of IL-8 in GF was higher (10-14 times) than in the case of healthy donors, as well as in patients in the clinical group. Conclusion: The content of IL-4 and IL-8 cytokines in the biological fluids of the oral cavity increases in the dynamics of pregnancy, which increases the protective potential of the biological environment of the oral cavity. ASSESSMENT OF BONE METABOLISM IN PATIENTS WITH BISPHOSPHONATE OSTEONECROSIS OF THE JAWS

Abstract

The aim of the study was to evaluate bone metabolism in cancer patients with bisphosphonate osteonecrosis of the jaws. The study included 45 people of the main group (patients with cancer with osteonecrosis of the jaw in patients receiving bisphosphonates) and 25 in the control group (cancer patients treated with bisphosphonates, but did not have osteonecrosis of the jaw), which had a stabilization of the underlying disease. Bone metabolism was evaluated by the level of osteocalcin (OC), bone-specific alkaline phosphatase (bALP), aminoterminal of propeptide of procollagen type I (P1NP), tartrate-resistant acid phosphatase (TRAP5b), calcium (Cа), phosphorus (P) in blood serum before treatment and after 6 months. Compared to the average levels of marker patients of the main and control groups using the Mann-Whitney test for p < 0,05. A baseline level of СТХ (0,23±0,02 ng/ml) and OС (11,58±0,54 ng/ml) in the treatment group was significantly (p < 0.05) lower than control group (0,43±0,01 ng/ml and 17,94±0,83 ng/ml), and the level of osteocalcin in the main group (11,58±0,54 ng/ml) was on average below normal 2,59 times. Recorded significantly higher (p < 0,05) levels bALP (133,24±14,03 U/l) and TRAP5b (3,54±0,38 U/l) in patients with osteonecrosis compared with a control group (73,32±3,41 U/l and 3,12±0,12 U/l). Reliably detected differences in the levels of P1NP, Ca and P were not detected (p > 0,05). In the main group after 6 months of treatment was observed a tendency of growth of СТХ, TRAP5b, OK, bALP, P1NP, Ca, but only for the markers of resorption and СТХ, TRAP5b these differences were significant. Indicators of patients in the control group were stable and did not differ in the dynamics. The development of bisphosphonate osteonecrosis of the jaws is directly related to bone metabolism and occurs with predominance of the processes of bone resorption and inhibition of bone formation processes.

Abstract

The article is devoted to the diagnosis and treatment of patients with musculo-articular dysfunction (MSD) of the temporomandibular joint (TMJ) pain syndrome in combination with amplipulsetherapy, fluctuating and orthopedic methods. The purpose of the study - to improve the diagnosis and treatment of patients with chronic generalized periodontitis (present study included) of the DPA and TMJ, with pain, with the help of complex methods of therapy. Made clinical examinations of 98 patients of 35-45 years with MSD and TMJ pain. Identified groups of patients with present study included and MSD with displacement and without displacement of the mandible. Defined electromyographic parameters of bioelectric activity of masticatory and temporal muscles in patients with present study included and MSD. For pain syndrome all patients appointed fluctuating in the area of TMJ. Determined the effectiveness of the treatment, studied diagnostic models in articulator PROTAR (Germany), conducted an electromyography. To eliminate occlusive barriers used mouthguard during sleep. Studies have shown that the use of clear aligners in combination with orthopedic and physiotherapeutic methods is an effective method in the treatment of patients with MDS present study included pain. Okklyuzionny tires change the nature of closing the teeth, affect the periodontium, masticatory muscles and TMJ. Without dental intervention on the occlusal surface they help to diagnose and troubleshoot the DPA in the TMJ due to occlusions violations.

THE MONITORING OF DENTAL ORGANIZATION’S MEDICAL WORKERS COMPETENCES ACCORDING TO THE NEW SYSTEM OF WAGE PAYMENTS

Abstract

Using the method of empirical anonymous sociological research the competence of 630 dentists and health care institutions managers according to the new system of wage payments (NSWP) was studied. On the 7 questions asked, containing about 6138 possible answers from all the respondents 2253 correct answers were retrieved which represents 36,7% of all responses. Found data shows that only every third dental clinic specialist has the competence in NSWP while the majority 63, 3% are not quite oriented in the NSWP. One of the main problems of the NSWP integration is lack of understanding of basic principles and specific criteria of work quality assessment from health care institutions employees thus nullifies the potentially high impact of its introduction. Raise of the transparency of wage payment system will cause significant personnel motivation improvement in achieving qualitative and quantitative labor values that the authority has laid for the organization that are tied to the work goals of the institution itself and the health care system as a whole. Medical institution administration system should use new tools for staff competence improvement nowadays. It is necessary to provide professional communication space for colleagues on the questions of NSWP.

THE ROLE OF SALIVARY GLAND BIOPSY IN DIAGNOSIS OF SYSTEMIC DISEASES AND THEIR LYMPHOPROLIFERATIVE COMPLICATIONS

Abstract

The clinical picture of many systemic diseases (SD) could make its debut in the maxillofacial area with salivary gland lesions. Sjogren’s sicca syndrome, IgG4-related diseases, sarcoidosis, viral hepatitis - C, primary biliary cirrhosis, autoimmune hepatitis, as well as complications of the plurality of abnormalities in the form of lymphoproliferative diseases including AL-amyloidosis and B-cell lymphomas of different malignant tumors. The main diagnostic criteria in this case are the data of histological and immunohistochemistry studies of affected tissues. The aim of this publication is to analyze literature data on the pathological changes developing in salivary glands at different SD. The question about the diagnostic value of limphoplazmatsitarny infiltrate detected in the salivary gland biopsy materials SD and its influence on the prognosis and course of pathological process remains open. Conclusions: salivary glands biopsy has played a significant role in the early diagnosis and follow-up of patients with a variety of SD occurring with salivary gland lesions and such research studies have been required to continue.
