Abstract

The article presents analysis of modern concepts concerning value of health from point of view of human capital and from positions of competitiveness. It is noted that health capital comes out as investment project for human capital. The multiple-aspects of health conditions, dynamics of alterations of these conditions in many ways depend on internal stimulating motives of individual, life conditions, specifics of social environment, life situation. From this point of view, health is a socially conditioned occurrence playing dialectic role in relationships of human with society, with others and with oneself.

The social portrait of patient with arterial hypertension

Abstract

The study was carried out related to social characteristics of 318 patients with arterial hypertension. The obtained data are compared with opinions of patients concerning accessibility and quality of provided medical care. It is established that patient with arterial hypertension is most often a male (54.1%), aged 55-64 years (56.9%), younger than 45 years (38.7%), with such harmful habits as tobacco smoking (63.5%) and alcohol consumption (60.1%), married (83.3%), residing in municipal apartment (more than 70%), has children residing separately (47.5%), works in industrial sphere (41.2%), with income as 20-40 thousands roubles per family member (39.6%) of lower than 20 000 rubles (30.2%) paying attention to activity on dacha parcel (34.4%), communicates mainly with family members and neighbors (51.6%), emphasizes hard stress situations all along last two years (59.6%). As a rule, patient with arterial hypertension has medical organization in place of one's residence (55%) but frequently reaches it by municipal transport (31,4%), rather frequently visits medical organization (53.1%) and expresses satisfaction with quality of medical care (76.7%). The predictors decreasing probability of satisfaction with quality of medical care are older age of patient, alcohol consumption, more frequent visits to medical organizations. The conclusion is made that study of social demographic characteristics is to be used in practice for increasing efficiency of prevention of arterial hypertension in able-bodied population.

The goals of sociology of medicine in studying technologies of “human enhancement”

The technologies of “enhancement of human” are issue of discussion at large due to absence of clear-cut conception in society and professional community concerning related with risks for human individual and society. For the present, these technologies just began to be applied only in medicine. Therefore, namely medical professionals are this social group that is to be more informed in the area of “enhancement of human” and hence more adaptive to their application. The paradox of inconsistency of level of technologies and level of professionalism at their application includes high social risks to be foreseen and to be eliminated. To implement these task is possible with means of sociology of medicine.

The characteristics of perception, awareness and formation of inner picture of disease in girls and boys

Abstract

The article presents data of study of perception and awareness of disease in girls and boys. The children0specific characteristics of development of inner picture of disease are identified. The model of components of inner picture of disease of child is presented as system of factors of positive and negative impact. The structure of attitude to health is presented including cognitive, emotional and behavioral components. In ill children sexual and gender differences reaction to disease are established. These differences are manifested in that girls in a more adequate way perceive hospitalization, adapt faster to new life conditions and new life stereotype though they more often experience apprehension for future. The attributes of image of health and disease are described. In girls they are characterized by emotional attributes and in boys by instrumental activity and object-concrete attributes that condition their behavioral repertoire. In girls, the social aspect in damage of health is more significant than physiological one. In boys both aspects are of equal worth. The ideas of disease and their intellectual and emotional processing are more objective in girls. The particular issue of study became identification of intra0family relationships and relationship healthy/unhealthy children with parents and parents with children determined by specific of nosology of children. It is established that parental evaluation of attitudes to child depend on gender of parents and is related with sex of child that is related with social gender roles of males and females and not with their individual psychological characteristics. The reaction of parents to prognosis of disease passes several stages and at that behavior of mother and father differ. The mothers of ill children faster than fathers alter their attitude to developed situation and they play major role in maintaining hope for convalescence of children and assist them to adapt to new life conditions.

The experience of expired century was used as a basis of presenting certain philosophical and sociological foundations of sociology of physician. The main methodological defects of comprehension of the system “physician-patient” are revealed. The particular methodological aspects of status position of physician in social space are exposed. The original structure of status of physician is proposed as oneness of one's social (personality) and biological (individual) aspects. The absolute and relative differences of status of physician from other social units are described.

The self-safety of patient in surgical practice: sociological aspects

Abstract

The article considers from positions of sociology of medicine relatively new for Russian medical science notion “self-safety of patient”. Such concrete sociological techniques as voluntary anonymous questionnaire survey and siple questionnaire were applied. The determination of notion is proposed from sociological and legal points of view. The level of competences from the side of patients of surgical clinic hospitalized in routine procedure for surgery treatment is investigated. The certain organizational activities in context of considering problem promoting prevention of occurrence of conflict in the system “physician-patient” are presented. The results of implemented study make it obvious that patients, from one hand, are insufficiently aware about risks related to surgical intervention and from the other hand they have extremely low indicator of self-safety preferring for-no-reason to trust attending physician and at that to impose on him whole responsibility for the results of treatment.

The evaluation of degree of accessibility of health centers at municipal level

Abstract

In the Zabaikalskii kraii function 5 health centers providing free services to population and 4 out of them are in the municipal okrug “Gorod Chita”. The main purpose of their functioning is development of complex of activities targeted to health promotion through propaganda of healthy life-style, motivating citizen to personal responsibility for one's health and health of one's children, struggle with risk factors of development of diseases, education and information of population about harm of tobacco smoking and alcohol abuse. The assessment was made concerning accessibility of health center in municipal okrug “Gorod Chita”. The questionnaire survey and interviewing were applied. The reliability of results was achieved due to representative sampling. The respondents are presented by residents of Chita and aged 18-72 years. It is established that population of municipal okrug evaluates project positively, in spite of occurring problems. The most of the respondents evaluate quality of rendering of medical services in health centers as high. The deficiency of information is considered as one of main problems on the way of achieving purposes of Health centers. Second place goes uncomfortable schedule of work. And third place goes to feeble motivation of population. To overcome occurring difficulties more active advertising was proposed and development of succession with other medical and health promotion organizations.

The characteristics of development of relationships in the system “physician-patient” in modern conditions

Abstract

The implementation of modern technologies in diagnostic, treatment, prevention of various diseases, level of satisfaction of population with quality of rendered medical services are within framework of negative estimates. This assumption is supported by the results of sociological surveys carried out by VCIOM, Levada-Center, the Minzdrav of Russia and also by initiative of the authors of the present article. The theoretical approach of sociologists to studying characteristics of functioning of social institutes of modern society was used to establish instability of confident attitude of patients to its main actors and structures as one of the main causes of paradoxical development of institute of medicine in Russia. The article considers types of confidence in medicine and factors effecting their development. Among them the social status of physician in modern society, his economic and legal position, stereotyping of public consciousness by mass media and development of negative image of worker of medical sphere and underdevelopment of health-preserving behavior of population are positioned as key ones. The particular attention is paid to problem of interpersonal confidence that is a key determinant in development of relationship in the system “physician-patient” in modern conditions of transformation of hierarchic structure of social interactions into linear one that is conditioned by objective processes of further informatization of society. of the chair of modern sociology of the sociological faculty.

Certain results of studying quality of life relevant to health inpatients with arterial hypertension

Abstract

The article presents the results of study of quality of life of patients of able-bodied age with arterial hypertension. The study was carried out om the basis of ambulatory polyclinic institutions of Saratov in 2013-2015. The questionnaire WHOQOL-100 was applied. The respondents with arterial hypertension had statistically reliable decreasing of total index of quality of life, index of total value of life perception and also indices of quality of life in such areas as "Physical", "Level of Independence", "Environment" that testify somatic determination of index of quality of life of the given areas. The impact is established related to the level of incomes and related deprivations on both total index of quality of life of patients with arterial hypertension and indices of quality of life in particular sections of questionnaire WHOQOL-100 (mainly such sections as "Level of Independence", "Psychological" and "Physical" sections). This occurrence substantiates assumption about social determination of quality of life of patients with arterial hypertension.
