DYNAMICS OF RETINOPATHY OF PREMATURITY MORBIDITY INDICES AND EFFICIENCY OF LASER TREATMENT IN SVERDLOVSK REGION

Abstract

The article presents the results of examination of 3580 infants of the risk group followed-up at the Regional Center for Retinopathy of Prematurity. Retinopathy of prematurity incidence in different years fluctuated from 30.5 to 34.9% with maximal index (up to 84.5%) registered among infants with extremely low mass of a body at birth. However infants with mass of a body at birth over 1500 g also were diseased (up to 23.8% of cases) and even lost sight (up to 10.9%). The threshold stages developed almost in 31.5% of the diseased infants. The efficiency of laser treatment fluctuated in different years from 77.5 to 100% in retinopathy of prematurity with minimal vascular activity, and from 30 to 86.4% in "plus"-disease and posterior aggressive retinopathy of prematurity. Intensive organizational work with the doctors of regional perinatal centers and increase of the number of patients timely directed for treatment as well as the perfection of laser coagulation technique resulted in decrease of disablement up to 2.2%.

Abstract

Analysis of the myopia incidence according to the appealability data, regular medical check-ups and primary disablement of children in the southern area of the Republic of Bashkortostan for 5 years (2003-2007) is presented in the article. The increase of incidence was detected, and in its structure the myopia ranked as the first through all the analyzed years. The majority of children followed-up for shortsightedness suffered from mild myopia. The increase of primary disablement in children due to myopia was noted.

LASER THERMOKERATOPLASTY AS AN ALTERNATIVE TO THE GLASSES AND CONTACT LENSES CORRECTION IN CHILDREN

Abstract

The article presents the data on the laser technique with use of the laser device "OKO-1" with wavelength 2.12 ?m. The laser thermokeratoplasty procedure is low traumatic, with insignificant lesions of endothelium and can be applied for elimination of anisometropia caused by hyperopia. The author shows that even without special training exercises the visual functions of amblyopic eye abruptly increase contributing to formation of stable binocular vision.

RETROSPECTIVE ANALYSIS OF RETINOPATHY OF PREMATURITY RISK FACTORS

Abstract

The authors performed a retrospective analysis of some risk factors of retinopathy of prematurity: blood gas indices (SpO2, sO2 - 2616 tests, pO2, pCO2 - 844 capillary blood tests, FiO2 - 944 tests) during the first 7 days of life of premature infants who later developed retinopathy of prematurity; SpO2 indices in all three groups of infants exceeded 96% (hyperoxia) but indices of arterialized capillary blood gas (SaO2 and pO2) remained within permissible values. That means that in comparison of these two methods pulseoxymetry is a more adequate method for hyperoxia detection. Simultaneous application of pulseoxymetry and transcutaneous monitoring is the most optimal for hyperoxia detection

CEREBRAL INTRAVENTRICULAR HEMORRHAGES AS A HIGH RISK FACTOR OF RETINOPATHY OF PREMATURITY DEVELOPMENT IN NEWBORNS WITH EXTREMELY LOW MASS OF A BODY AT BIRTH

Abstract

The results of examination of 64 newborns with a mass of a body at birth from 670 to 990 g and gestation term from 24 to 30 weeks that stayed for treatment at the neonatal center of the Municipal Pediatric Clinic ь1 of Saint-Petersburg during 2008 are presented in the article. Among them 13 infants with severe cerebral intraventricular hemorrhage complicated by obstructive hydrocephalus made the main group of follow-up and the control group consisted of 51 infants without neonatal cerebral intraventricular hemorrhage. Comparative analysis of frequency and severity of retinopathy of prematurity manifestations in these groups was performed. The number of infants with retinopathy of prematurity in both groups was the same, at that the disease progressing up to the III stage including the threshold stage demanding preventive treatment was observed 5 times more often (P < 0.05) in infants with cerebral intraventricular hemorrhage in comparison with the control group. The number of unfavourable outcomes characterized by the retinal detachment and loss of vision among infants with severe forms of cerebral intraventricular hemorrhage made 20% that 4 times exceeded this index in the control group (5.1%). Severe forms of cerebral intraventricular hemorrhage, particularly those complicated by the progressing post-hemorrhage obstructive hydrocephalus, should be considered at the neonatal age to be a correctable high risk factor of unfavourable course of retinopathy of prematurity.

USE OF MOBILE DIGITAL TECHNOLOGIES OF THE OPHTHALMOLOGIC EXAMINATION OF INFANTS AT THE PREMATURE INFANTS NURSING DEPARTMENT

Abstract

Screening examinations of infants were performed at the premature infants nursing departments of pediatric clinics of Kaluga, Bryansk, Orel and Tula with use of indirect binocular ophthalmoscope and RetCam Shuttle. Total number of the examined infants included 259 infants who underwent initial examination and 141 infants were examined repeatedly. RetCam Shuttle application allows not only examining in details all the sections of fundus oculi but also registering of the obtained findings for the following analysis of the changes of the retinal state in dynamics and thus determining treatment tactics. In comparison with indirect binocular ophthalmoscopy RetCam Shuttle allows a more precise localizing of the process in accordance to the zones of fundus oculi and receiving more informative results of the examination. RetCam Shuttle use is necessary for the profound diagnostics of complicated cases under the conditions of work on traveling with an opportunity of a more precise prognostication of the course of the disease

STUDY OF THE INFECTIOUS STATUS OF INFANTS WITH VARIOUS TYPES OF UNILATERAL CONGENITAL CATARACTS

Abstract

Detection of a range of etiologic features of unilateral congenital cataracts (UCC) resulted from a clinical-immunologic examination of 430 infants with UCC. Infectious, general somatic, congenital compromised history of infants and their mothers, correlation between infectious background of the infants and the character of post-op period were studied. During pregnancy in mothers of infants with UCC transplacental infections were detected in 77.4% of cases, and pathology of pregnancy and delivery - in 78.1% of cases. Various somatic pathology was detected in 58.8% of infants with UCC. High frequency of congenital contamination (in 83.1% of cases) was detected as a result of the immunologic examination of infants with UCC. Herpes-viral (47%), tuberculous (36.1%) and toxoplasmatic (14%) infections were found most often. Comparison of the findings on the contamination of the infants and incidence of the post-op complications of inflammatory character allowed establishing a correlation between them.

ULTRASONIC BIOMICROSCOPY OF THE EYE IN TREATMENT OF THE PROGRESSING FIBROPLASTIC PROCESS IN THE SURGICALLY FORMED DRAINAGE SYSTEM IN INFANTS WITH GLAUCOMA

Abstract

Ultrasonic biomicroscopy findings (UBM) preformed in 40 eyes of 39 infants who underwent antiglaucomatous operations were analyzed. The results of treatment conducted according to the UBM data in 13 eyes of 12 children operated from 1 to 6 months ago are presented. In 8 normotonic eyes intraocular pressure (IOP) made 22.6+1.6 mm Hg), and in 5 eyes - ophthalmic hypertension (IOP 26.2+3.1 mm Hg). In normotonia the main acoustic sign of IOP increase risk was the deterioration of the state of the drainage pathway structures in dynamics on conditions that characteristics of the structures reached marginal values or transcended the diapason of the parameters fluctuations peculiar to IOP compensation. In ophthalmic hypertension the retaining hypoechogenicity of conjunctiva in the zone of surgery, possibility of hypoechogenic tunnel visualization, differentiation of the scleral flap and intrascleral cavity as well as low acoustic density of the intrascleral cavity inclusions became the prognostic signs of the planned treatment efficiency. The treatment conducted according to intrascleral cavity UBM findings allowed achieving stable normalization of intraocular pressure (IOP < 21 mm Hg) even in progressing fibroplastic process. At the same time UBM of 27 eyes with ophthalmic hypertension in 27 infants examined in terms exceeding 6 months after the surgery showed the poor perspectives of intraocular humor outflow activation in the surgical zone.

COMPLEX TREATMENT OF THE CONCOMITANT ALTERNATING SQUINT IN CHILDREN

Abstract

We examined 200 patients with squint having received conventional treatment and 90 patients, which together with conventional treatment were treated by infrasonic pneumomassage, infrasonic phonophoresis and colour stimulation. A course of treatment included 10 sessions, and the duration of treatment made 2 weeks. The treatment resulted in increase of the number of patients with binocular vision up to 74.4%, and the number of patients with monocular vision decreased to 26.6%. The angle of squint diminished in every second patient and completely disappeared in every fifth. Dynamics was absent in 25.6% of cases. Negative results were absent in all the patients. Application of infrasonic pneumomassage, infrasonic phonophoresis and colour photostimulation allows obtaining stable results, and efficiency of the suggested treatment made 52.2%.

EFFICIENCY OF THE STAGE-BY-STAGE PROSHTETICS IN REHABILITATION OF THE PATIENTS WITH CONGENITAL ANOPHTHALMOS AND MICROPHTHALMOS

Abstract

Efficiency of the stage-by-stage prosthetics method in patients with congenital anophthalmos and microphthalmos for enlargement of conjunctival cavity, osseous orbit and eyelids was studied in this work. In 259 patients with congenital anophthalmos and microphthalmos was performed a bloodless enlargement of the conjunctival cavity by method of stage-by-stage prosthetics by prostheses increasing in size. The age of patients varied from 1 month to 20 years. Follow-up terms made up to 10 years. The results of treatment were evaluated according to the following criteria: possibility of prosthetics, volume and shape of the prosthesis, size of the orbit, length and width of the palpebral fissure. The possibility of prosthetics by standard prostheses was achieved in all cases. The volume of the prostheses in use increased from 0.45 to 2.45 cm?. In 6 months of the prosthetics horizontal and vertical orbital sizes became 0.4-2.7mm larger, palpebral fissure - 1.6-4.1mm longer and 0.7-1.9 mm wider. The best dynamics was noted if the beginning of prosthetics occurred in the early childhood (from the age of 1 month). Thus, the stage-by-stage prosthetics is an effective method of conjunctival cavity, eyelids and orbit enlargement in the early childhood in patients with congenital microphthalmos and anophthalmos

LEVOFLOXACIN IN THERAPY OF BACTERIAL INFECTIONS IN CHILDREN

Abstract

The assessment of clinical efficiency of local application of Levofloxacin 0.5% in eye drops in treatment of inflammatory diseases in children was performed and cytotoxicity of the medication was studied. The authors examined 34 children (62 eyes) with various inflammatory diseases of the organ of vision. It was shown that the terms of complete regression of the disease symptoms were observed at the period from 5-6 to 18-19 days depending on the nosologic form. Taking into account a wide spectrum of drug action and clinical efficiency of Levofloxacin, the absence of clinically pronounced effects and low keratotoxicity of the medication, instillation form of Levofloxacin 0.5% (Oftaquix(r), Santen) can be recommended for wide application as empiric therapy in children at the age of the 1st year of life with inflammatory eye diseases.

Abstract

The results of simultaneous operation in infants with bilateral dense congenital cataract, non-compensated congenital glaucoma and in older children with lens subluxation and Marfan's syndrome in increased risk of general anesthesia are presented. Simultaneous operation in both eyes contributed to acceleration of rehabilitation in children with cataract, IOP normalization in patients with glaucoma and decrease of the general anesthesia risk. No complications were noted during bilateral operation and in the post-op period. A special importance of following asepsis rules in performance of simultaneous cavity interventions in both eyes is emphasized by the authors.

MOLECULAR GENESIS OF CATARACT INITIAL FEATURES

Abstract

The article is dedicated to the study of the initial link of pathogenesis of the cataract. The examinations were performed in 1010 cataract patients (1860 eyes) by the Italian biomicroscope with the registering digital camera SLIT Lamp OTC (magnification x40). It was established that as early as before cataract development protein molecules of the lens denaturate: vacuoles and dilated pores, torn globular protein molecules, transparent and opaque protein canaliculae, etc. appear. It is evidence that the lens protein molecules denaturation in cataract occurs first and leads to increase of the cellular membranes permeability, of which aqueous fissures, dissociation of the lens fibers, aggregations around the dilated pores on the lens membranes and in the intercellular media are the signs. These aggregations appear as a result of penetration of the cellular contents on the surface of the cellular membrane and lead to infiltration and lens opacity.

Abstract

The authors analyzed in details the mechanics of impact on the human body of the pulse low-frequency electromagnetic field and substantiated its application in ophthalmology. The approach with use of the low-frequency pulse electromagnetic field as an informative factor seems to be optimal in connection with the synchronization of the oscillation process of the pulse low-frequency electromagnetic field and rhythm of various systems of the organism functioning that causes bioresonance influences changing functions of the organ or of the system.
