Abstract

The authors analyzed the average statistical data on the incidence of congenital pathology of the organ of vision and disability in the Republic of Dagestan. It was established that among the causes of visual disability in children are the following: high complicated myopia, traumas of the organ of vision, retinal pathology, and congenital pathology.

Abstract

The article describes an algorithm of the early detection of perinatal optic atrophy in children with risk factors of its development. The patients underwent hemodynamic monitoring of regional blood flow and electrophysiologic examinations. Diagnostic ischemic-hypoxic index was developed and substantiated; its various values allow with high probability to determine the presence or absence of optic atrophy in children with perinatal compromise.

Abstract

Clinical-statistical analysis of organizational measures on prevention, early diagnostics and treatment of retinopathy of prematurity (ROP) in the Khabarovsk Territory for the periods 2006-2007 and 2008-2009 are listed in the article. It was shown that higher incidence (87.9% of cases) of favourable outcomes in the threshold stages of active RP after conducted transpupillary laser coagulation was obtained due to the introduction into clinical practice of the new organizational system of team-work of Perinatal center and the Khabarovsk affiliate of the Academician S.N. Fyodorov IRTC "Eye Microsurgery" of the Rosmedtechnology (2008-2009).

ROLE OF HEMOTRANSFUSION IN DEVELOPMENT AND COURSE OF RETINOPATHY OF PREMATURITY

Abstract

The authors detected a distinct dependence between the number of hemotransfusions during the first 24 hours of life and the severity of retinopathy of prematurity course (RP): infants with "plus"-disease in the early neonatal period underwent transfusions of erythrocytic mass twice as often as in infants with minimal vascular activity; infants with posterior aggressive RP form underwent the procedure four times more often. The RP course with minimal vascular activity after hemotransfusion remains unchanged. After hemotransfusion in infants with severe RP forms ("plus"-disease, posterior aggressive RP) the caliber and tortuousity of the retinal vessels increase, retinal edema accumulates, exudative and hemorrhagic changes swell as well as extraretinal proliferation. It is necessary to avoid hemotransfusions in such infants at the peak of the disease activity.

STUDY OF CAPILLARY BLOOD GAS INDICES IN THE GROUP OF INFANTS WITH THE RISK OF RETINOPATHY OF PREMATURITY DEVELOPMENT

Abstract

At the department of reanimation and intensive care of newborns the indices of the capillary blood gas partial pressure were followed-up in 45 infants during the neonatal period. The tendency to hypoxia and hypoxemia during the first-second week of life in groups with retinopathy of prematurity was detected. In the group of infants who needed to undergo a preventive laser coagulation of the retina mean values of oxygen partial pressure in the capillary blood were reliably lower during the first and second weeks of life in comparison with the group without RP development (40+5 and 38+6 mm Hg in comparison with 46+5 and 47+6 mm Hg, p < 0.05). The authors showed hypoxemia influence on the development of retinopathy of prematurity severe forms during the first weeks of life. Relative hypercapnia for the moment of the beginning of ophthalmologic follow-up can be a risk factor of retinopathy of prematurity development.

HEMODYNAMIC INDICES IN THE RETINAL VESSELS IN RETINOPATHY OF PREMATURITY IN ACTIVE PHASE OF THE DISEASE

Abstract

An analysis of the quantitative parameters of hemodynamic indices in the vessels supplying the retina (central retinal artery, posterior short ciliary arteries) in children with early features of retinopathy of prematurity (RP) was conducted. The main group included 38 infants (68 eyes) with RP of I-III stages in active phase, and as a control served the indices of linear blood flow velocity in central retinal artery and posterior short ciliary artery of 39 eyes of 21 premature infants without RP features and that of the group of 18 healthy mature infants (36 eyes). Linear velocity of diastolic blood flow (Vdiast) and index of vascular resistance (RI) proved to be the most hemodynamically significant indices. It was ascertained that in clinical features increase accompanied by the pronounced changes on fundus occurred an abrupt drop of the diastolic blood flow linear velocity (Vdiast) and the significant RI increase in comparison with the control group. Quantitative indices depend on the RP stage.

ANALYSIS OF THE RISK FACTORS OF RETINOPATHY DEVELOPMENT IN INFANTS AT THE DEPARTMENT OF REANIMATION AND INTENSIVE CARE OF NEWBORNS

Abstract

At the department of reanimation and intensive care of newborns 165 premature infants from the group of high risk of retinopathy development were examined. Concomitant pathology such as pneumonia, enterocolitis and cerebral ischemia of II and III degree was detected in all the infants of the examined group. In 100 cases development of retinopathy of prematurity was noted. In the group of infants with high risk of retinopathy of prematurity development a high incidence of ischemic structural lesions of CNS was registered. Periventricular leukomalacia develops more frequently. Hemorrhages of the III degree prevail in the structure of intraventricular hemorrhages. Long-term disturbance of hemodynamics with increase of the resistance indices of the cerebral and ophthalmic arteries over 0.8 is a significant risk factor of severe retinopathy of prematurity forms development.

Abstract

The article pioneers describing morphologic features of the macular zone structure in infants with hypoplasia of the macular zone studied with the help of the modern method of intravital visualization, i.e. optical coherent tomography. Two types of hypoplasia are distinguished. Characteristic concentric reflexes in the macular zone in infants with the second type of hypoplasia are detected in ophthalmoscopy in polarized light. Visual functions and their prognosis in infants of both groups are compared. The authors show an opportunity to make a prognosis of visual functions for the infants with hypoplasia of the macular zone in use of ophthalmoscopy in polarized light in the first years of life when optical coherent tomography is failed to be conducted due to the age of the infants.

NITRIC OXIDE IN PATHOGENESIS OF DIABETIC RETINOPATHY

Abstract

The work is dedicated to detection of correlative dependence between the content of nitric oxide (NO) metabolites in lacrimal fluid and clinical condition of the retina in patients suffering from diabetes mellitus (DM) of the 2nd type at preclinical and non-proliferative stages of diabetic retinopathy (DR). It was established that mean level of NO metabolites in lacrimal fluid in patients with DR is significantly higher than in patients without DR (p < 0.01). In the control group NO metabolites in lacrimal fluid were not found (the level was lower than the sensitivity threshold of the method). The study of the final NO metabolites in the lacrimal fluid also showed that in DM their high level is already noted at the early stage of DR that is a compensatory vascular reaction supporting metabolism and providing blood supply, and NO plays an important pathogenetic role in the mechanics of diabetic complications forming.

Abstract

The authors developed a method of assessment of treatment efficacy in penetrating wounds of the eyeball with help of apparatus-programmed complex "IKAR", which is a broadband 9-zone spectrophotometer of low intensiveness working at the diapason of wavelengths from 2 to 12 ?m (3500-963 cm-?) combined with personal computer allowing to register dispersion of the transmission indices of infrared spectrum of lacrimal fluid (LF). Apparatus-programmed complex allows to determine the similar changes in the dynamics of dispersion of the LF transmission indices and to compare it with the clinical picture and course of inflammatory process.

Abstract

The assessment of efficacy of Tobrex 2x (r) drops instillations into conjunctival cavity twice a day in treatment of 36 children at the age from 1 to 15 (23 eyes) with conjunctivitis and anterior uveitis of bacterial and mixed etiology was preformed. The conducted analysis confirms the clinical efficacy of eye drops Tobrex 2x (r) corresponding to the following compliance: wide spectrum of action and clinical efficacy, fair tolerability, absence of the clinically pronounced side effects, convenience in use. Tobrex 2x (r) can be recommended for the wide use in inflammatory eye diseases in children from 1 year of age.

Abstract

The analysis of the results of prosthetics in 311 children with anophthalmos (acquired and congenital) and subatrophy of the eye is presented in this work based on the materials of the Helmholtz Moscow Eye Diseases Scientific Research Institute laboratory for eye prostheses selection. Specifics of the method of eye prostheses selection depending on the type of pathology, clinical picture, age of the patient as well as terms of replacement and characteristic of the prostheses were determined. It is emphasized that eye prosthetics allows not only obtaining good cosmetic results but also rehabilitating the child socially and psychologically. The article reflects certain legal matters related to the problem of unilateral anophthalmos.

Dispersion of the lacrimal fluid transmission indices in the eyes of patients with sympathetic inflammation by method of infrared spectrometry was studied. Apparatus-programmed complex "Ikar" was used, that is a broadband 9-zone spectrophotometer of low intensiveness working at the diapason of wavelengths from 2 to 12 ?m (3500-963 cm-?) combined with personal computer. It was established that dynamics of dispersion of the lacrimal fluid transmission is evidence of expediency of sympathizing eye enucleation that has a favourable action on the character of the sympathetic inflammation course in the fellow eye.

CLINICAL EXPERIENCE OF IRIS-CLAW IOL "ARTISAN" APPLICATION BEHIND THE IRIS IN POSTERIOR CHAMBER IN COMPLICATED CASES

Abstract

We followed-up 105 patients (107 eyes) at the age from 19 to 89 years (mean age 73.7+10.1 years) among them 63 (60%) females and 42 (40%) males. The follow-up term made from 4 to 96 months, in average - 20.4+19.8 months. Iris-claw IOL "ARTISAN" was implanted in all the patients behind the iris in posterior capsule. Visual acuity 0.5 and more was obtained in 90 (84.11%) cases. Iris-claw IOL implantation was accompanied by minimal complications. The position of the implanted lens remained stable during the whole follow-up period. Analysis of the clinical material and obtaining high functional results is evidence of safety and reliability of iris-claw IOL "ARTISAN" implantation behind the iris in posterior chamber in complicated cases; this procedure can be an operation of choice for aphakia correction.

Abstract

Peroxiredoxin VI (PRDX6) is a protein marker of age-related cataract. It can be considered an attribute of untimely ageing and usual mechanics of protection from oxidative stress in surgical intervention in ophthalmology. PRDX6 content is stably increased in the cells protected from oxidative stress. Decrease of protein concentration leads to aggravation of apoptosis mechanics in patients suffering from diabetic cataract and PVR and as a consequence to a large number of post-op complications. Absence of the marker in tear allows considering any cataract as complicated.

Abstract

The article is dedicated to the study of biological properties of S.aureus strains egested from the conjunctival, nasal and pharyngeal mucous tunic in children with bacterial conjunctivitis in its various course. We examined 110 children and teenagers at the age from 3 to 18. The following biological properties of S.aureus strains were studied: antilysozyme, anti-complementary activity according to O.V. Bukharin method and adhesive activity according to V.I. Brilis method. S.aureus strains were egested reliably more often in chronic course of bacterial conjunctivitis than in acute one. High and moderate antilysozyme and anti-complementary activity and low adhesive activity were determined reliably more often in chronic conjunctivitis both in strains egested from conjunctival and nasal mucous tunic in comparison with acute conjunctivitis.

Abstract

In traditional approach to cataract management in children high percentage (73.8) of repeated interventions as laser discission remains. Introduction of posterior capsulorrhexis in combination with anterior vitrectomy allowed decreasing by a factor of five the risk of visual axis opacities and creating favourable conditions for the more efficient amblyopia treatment. At that no additional expenses were needed for realization of this surgical method. Decrease of laser discissions in number provided economic effect. Expenses for the laser treatment in the group of children with posterior capsulorrhexis and anterior vitrectomy were reduced more than by a factor of five.

Based on the model of experimental developing axial myopia we worked out a new method of treatment for progressing myopia in children - distraction scleroplasty specifying activation of synthetic processes in fibroblasts and fibrocytes, improvement of the graft fixation to the scleral surface, decrease of the stretching tension in the exterior equatorial area of the sclera towards anterior-posterior axis of the eyeball.

Abstract

Circular segments were implanted into the corneal stroma of experimental animals according to the standard method. As explants were used the materials as follows: structured copolymer collagen and polymethylmethacrylate both with thickness of 200 ?m. As a result of the conducted experimental studies an opportunity of controlled modelling of refractive effect after intrastromal tunnel circular keratoplasty with YAG-laser was confirmed. The result is determined by the properties of the material: ratio of hydrogel, its density and thickness.

Abstract

The article is dedicated to clinical syndrome caused by thrombosis of cerebral venous sinuses with a characteristic picture of the optic discs edema on fundus as one of the manifestations of this condition. In case of detection of the optic disc edema picture for excluding of the diagnosis of cerebral sinus thrombosis it is necessary to conduct a spiral computer tomography with bolus dosing of the contrast medium (in the mode of angiography and venography) or magnetic resonance imaging in the vascular mode (venography).
