Postoperative analgesia with nephopam hydrochloride in plastic surgery

Abstract

The prospective clinical study was conducted in 150 female patients in early postoperative period after plastic and aesthetic surgeries. The analgesic effect and tolerability of nefopam hydrochloride within first hours following the surgery were evaluated. Nefopam hydrochloride was found to be a safe and useful adjunct to therapy for mild and moderate pain after plastic surgeries. Intramuscular injection of nefopam was associated with hemodynamic effects such as tachycardia and high blood pressure. These side effects should be accounted in postoperative pain management in patients with heart rate disorders, coronary heart disease, and hypertension.

Relationships between patient's personality type, postoperative pain and early postoperative period course quality in trauma and ortopeadic surgery

Abstract

The goal of the study was to find correlation between patient personality type from one side and quality of postoperative analgesia and general patient's satisfaction with intensive care phase after trauma and orthopaedic operations from the other side. The study results showed that postoperative pain leads to negative attitudes toward treatment. The patients who are less satisfied with postoperative analgesia have increased risk of hypertension. The most severe pain syndrome should be expected in patients with low body weight and enhanced dieresis. Personality type affects postoperative pain intensity: highly neurotic extroverts are more vulnerable to pain.

The influence of different variants of regional anesthesia on neurovegetative and hemoreological status of patients undergone surgery for foot and ankle joint trauma

Abstract

The goal of the study was to compare the efficacy of three methods of anesthesia in patients operated on distal part of the lower extremities: isolated spinal anesthesia, spinal anesthesia in combination with local anesthesia performed at surgery zone and combination of spinal anesthesia and peripheral nerves blocks. The study was involved 75 patients undergone surgery distal part of the lower extremities stratified for method of anesthesia into three comparable groups. It was proved that the combination of spinal anesthesia with blockade of nerves of the bottom extremity is the most appropriate technique. This type of anesthesia allows not only to quit the pain effectively but also to provides stable indicators of a neurovegetative homeostasis and allow to reache positive hemorheological effects in the early postoperative period.

Regional anesthesia in carotid endarterektomy: benefits and disadventages

Abstract

The goal of the clinical study was to investigate combined anesthesia based on regional blockade in patients undergoing carotid endarterektomiya. Sixty patients were enrolled into the study. At 60 patients with a carotid endarterektomiya studying of the combined anesthesia on the basis of regionarny blockade is carried out. Parameters of brain blood circulation, markers of brain damage, condition of the highest mental functions, neurologic complications were investigated. It is shown that for this anesthesia comprehesible of parameters of a brain blood-groove which doesn't protect from neyronalny damage is characteristic, doesn't cause deterioration of a condition of the highest mental functions.

Conception of analgesia in providing of regional analgesic methods

Anesthetic concentration of local anesthetics is often applied in analgesic regimens, including combination of general anesthesia and epidural or peripheral blocks. The author proposes to apply only analgesic concentrations of local anesthetics during such kind of combined analgesia while performing neural anesthesia if it is indicated individually.

Graphical portrait of meta-analysis results

Abstract

The results of quantitative systematic reviews issued by Cochrame society, so-called meta-ananlysis, are formulated in graphics. This graphical form is arranged strictly in accordance with Cochrane data base standards for this type of investigations. Sometimes these specific graphics may occupy up to 90% of the review. They accumulate all valuable information that is necessary to assess the correctness of conducted meta-analysis, its strategy of searching for articles, date processing methods, pooled sample heterogeneity and statistical and clinical significance of results. To be able to find valuable information in meta-analysis graphical portrait quickly and effectively one needs to know what data is expressed in short signs and abbreviations and what it means. These questions are clarified in the article.

The study is dedicated to Romanian school history of regional anesthesia. The author describes several interesting historical facts that were valuable for successful developing of methods of regional anesthesia at the beginning of XX century in Romania and Europe in general. An outstanding Romanian surgeon Tome Ionnesko made a great contribution in popularizing and developing new methods of anesthesia and applyed them widely in his own surgical practice.
