with the identification of four main phenotypes and a variant of the COPD + bronchial asthma (BA) phenotype. For a comprehensive assessment of the condition of COPD patients, the indices of disease severity – ADO and comorbidity – Charlson were calculated. A moderate positive correlation was established between the ADO and Charlson indices in patients with emphysematous (<italic>r</italic> = 0.407, <italic>p</italic> &lt; 0.04) and mixed (<italic>r</italic> = 0.537, <italic>p</italic> &lt; 0.001) phenotypes. The severe degree of the disease (<italic>p </italic>&lt; 0.01) was noted in emphysematous and mixed phenotypes, respectively 65 and 54 %. With bronchitis and COPD+In the phenotypes, the similar severity of the disease was 31 and 28 %. When analyzing the features of the construction of personalized pharmacotherapy in patients with different COPD phenotypes, clinically significant disadvantages of the treatment process in the region were established. A stereotypical mistake in the tactics of prescribing bronchodilators is the insufficiency of basic therapy in outpatient settings, which amounted to 34.7 % of cases, mainly in bronchitic (42.9 %) and mixed (54 %) phenotypes, which, in turn, leads to a decrease in the main goal of COPD treatment. Personalization of pharmacotherapy of patients of various phenotypes with chronic obstructive pulmonary disease optimizes the ten-year prognosis of patient survival.</p></abstract><trans-abstract xml:lang="ru"><p>Выполнено фенотипирование пациентов с выявлением четырех основных фенотипов и варианта фенотипа ХОБЛ + бронхиальная астма (БА). Для комплексной оценки состояния больных ХОБЛ были рассчитаны индексы тяжести заболевания – ADO и коморбидности – Charlson. Установлена умеренная положительная корреляционная связь между индексами ADO и Charlson у пациентов с эмфизематозным (<italic>r</italic> = 0,407,<italic> p</italic> &lt; 0,04) и смешанным (<italic>r</italic> = 0,537, <italic>p</italic> &lt; 0,001) фенотипами. Тяжелая степень заболевания (<italic>p</italic> &lt; 0,01) отмечена при эмфизематозном и смешанном фенотипах, соответственно 65 и 54 %. При бронхитическом и ХОБЛ + БА фенотипах подобная тяжесть заболевания составила 31 и 28 %. При анализе особенностей построения персонализированной фармакотерапии у пациентов при разных фенотипах ХОБЛ установлены клинически значимые недостатки лечебного процесса в регионе. Стереотипной ошибкой в тактике назначения бронхолитических препаратов является недостаточность базисной терапии в амбулаторно-поликлинических условиях, составившей 34,7 % случаев, преимущественно в бронхитическом (42,9 %) и смешанном (54 %) фенотипах, что, в свою очередь, приводит к снижению основной цели лечения ХОБЛ.
