The paper is dedicated to the pathogenetic link between IR and hypertension, IR importance in the development of diabetic cardiomyopathy. It is emphasized that metformin (Siofor) is the best drug for insulin sensitivity restoration and improvement of prognosis in cardiac patients. The presence of a various cardioprotective effects of metformin facilitates therapy optimization for patients with high risk of cardiovascular disease.</p></abstract><trans-abstract xml:lang="ru"><p>Рассматривается природа инсулинорезистентности (ИР), лежащей в основе развития сахарного диабета типа 2 и метаболического синдрома, представлены основные клеточно-молекулярные механизмы ее формирования. Указывается на патогенетическую связь между ИР и артериальной гипертензией, обсуждается значение ИР в развитии диабетической кардиомиопатии. Подчеркивается, что оптимальным препаратом, восстанавливающим чувствительность к инсулину и улучшающим прогноз у кардиологических больных, является метформин (Сиофор).
