IR and compensatory hyperinsulinemia play a key role in the pathogenesis of disorders of carbohydrate metabolism, which is confirmed by the numerous clinical and experimental studies, but they have multi-faceted potential for a cascade of other metabolic and vascular disorders. In particular, the relationship between IR and hyperinsulinemia and lipid metabolism has been studied extensively. The modern approaches to the treatment of DM2 with metformin, incretinomimetics, and tiozolidindiones are discussed. Currently, the sole representative of the tiozolidindiones on the medicinal market is pioglitazone.</p></abstract><trans-abstract xml:lang="ru"><p>Рассматриваются патогенетические механизмы развития сахарного диабета типа 2 (СД2) с упором на роль инсулинорезистентности (ИР). ИР и компенсаторная гиперинсулинемия играют ключевую роль в патогенезе нарушений углеводного обмена, что подтверждают данные многочисленных клинических и экспериментальных работ, однако они имеют многогранный потенциал для развития каскада других метаболических и сосудистых нарушений. В частности, активно изучается взаимосвязь ИР и гиперинсулинемии с нарушениями липидного обмена. Обсуждаются современные подходы к лечению СД2 с использованием метформина, инкретиномиметиков и тиазолидиндионов.
