Violation of incretine's secretion is one of the important mechanisms for the development of hyperglycemia in type 2 diabetes mellitus. Incretines are hormones acting on β- and α-cells of the pancreas, increasing insulin secretion and suppressing glucagon secretion, thereby maintaining normoglycemia. The dipeptylpeptidase type IV (DPP-4) is enzyme inactivating incretines. DPP-4 inhibitors are used in the treatment of type 2 diabetes mellitus. The article presents the results of clinical trials of a new DPP-4 inhibitor saxagliptin. These clinical studies have confirmed the efficacy and safety of saxagliptin in patients with type 2 diabetes mellitus both as monotherapy and in combination with other oral hypoglycemic agents.</p></abstract><trans-abstract xml:lang="ru"><p>Современная фармакотерапия сахарного диабета типа 2 (СД2) должна быть направленной не только на снижение уровня гликозилированного гемоглобина, глюкозы плазмы натощак и постпрандиальной гликемии, но и на все патогенетические звенья заболевания. Одним из важных механизмов развития гипергликемии при СД2 является нарушение секреции инкретинов - гормонов, которые, воздействуя на β- и α-клетки поджелудочной железы, увеличивают секрецию инсулина и подавляют секрецию глюкагона, тем самым способствуя поддержанию нормогликемии. В лечении СД2 нашли применение препараты группы ингибиторов дипептилпептидазы IV типа (ДПП-4) - фермента, инактивирующего инкретины. В статье представлены результаты клинических исследований нового ингибитора фермента ДПП-4 - саксаглиптина.
