The role of stellate cells, hepatocytes and endothelial cells in maintaining the normal composition of the extracellular matrix of the liver is presented, as well as the role of these cells in the mechanisms of fibrosis in pathological conditions. Regulatory substances and mediators involved in the activation and transformation of stellate cells in myofibroblasts synthesizing collagen, which may be potential targets for antifibrotic therapy, are presented. Characteristics of repair processes in acute and chronic liver damage are demonstrated. Modern methods and new directions in the treatment of hepatic fibrosis are analyzed in details. The mechanisms of action and efficacy of standardized silymarin (Legalon) in the prevention and treatment of fibrosis in acute and chronic liver diseases are represented.</p></abstract><trans-abstract xml:lang="ru"><p>В обзоре представлены сведения об основных компонентах и локализации интерстициальной ткани печени, а также современные представления о механизмах формирования, прогрессирования и обратного развития фиброза печени. Показана роль стеллатных клеток, гепатоцитов и эндотелиоцитов в поддержании нормального состава экстрацеллюлярного матрикса печени, а в условиях патологии - в механизмах развития фиброза. Представлены регуляторные субстанции и медиаторы, участвующие в активации и превращении стеллатных клеток в синтезирующие коллаген миофибробласты, которые могут быть потенциальными объектами для антифибротической терапии. Дана характеристика процессов репарации при острых и хронических повреждениях печени. Подробно проанализированы современные методы и новые направления в терапии фиброза печени.
