It is noted that among the antibacterial agents used for treatment of UTI, III generation cephalosporins, particularly cefixime, have attracted particular attention in terms of efficiency and good tolerability. The results of clinical studies of cefixime are presented. It is emphasized that cefixime can be considered as first-line treatment for most variants of UTI, including acute cystitis and acute pyelonephritis. Particularly, its use is justified in the presence of β-lactamase-producing pathogens. Cefixime can be considered as a treatment of choice in poor tolerance of other treatment options, as well as in groups of patients with highest risk of adverse events (children, elderly patients, pregnant women).</p></abstract><trans-abstract xml:lang="ru"><p>Рассматриваются причины инфекций мочевыводящей системы (ИМС), обсуждаются принципы их рациональной антибактериальной терапии. Отмечается, что среди антибактериальных препаратов, применяемых при ИМС, цефалоспорины III поколения, в частности цефиксим, привлекают особое внимание с точки зрения высокой эффективности и хорошей переносимости. Представлены результаты клинических исследований цефиксима. Подчеркивается, что цефиксим может рассматриваться как средство первого ряда лечения большинства вариантов ИМС, в т. ч. острого цистита и острого пиелонефрита. Его применение оправданно, в частности, при наличии возбудителя, продуцирующего β-лактамазу.
